Requirements in the Issuance and Endorsement of

STCW ’95 Certificate to Marine Deck Officers

MASTER MARINER & CHIEF MATE

I. Certificate of Registration/STCW Certificate of Competency


II. Medical Certificate from DOH accredited medical clinic/hospital with QSS , in place, using the DOH approved
seafarers pre-employment medical form, which must not be more than one(1) year from the date of
issuance. If submitted during application for STCW’95 Certificates, should not be least than fifteen (15) days
prior to expiry date.
III. One (1) year accumulated approved seagoing service during the preceding five (5) years.
A. Company Certification of Sea Service-shall Include the type of vessel, rank occupied, vessels gross tonnage,
vessel’s trading area and inclusive dates of sea service (letterhead with dry seal)
B. Seafarer’s Identification and Record Book (SIRB)-one photocopy each of pages one and two and other pages
reflecting sea service. (In case of lost/damage SIRB, any acceptable substitute documentary proof of sea service
and affidavit of loss shall be required.
IV. National Assessment Center (NAC) Certificate for Regulations V&VI

NOTE: NAC Certificates under Regulation V of the 1978 STCW w/ less than year validity shall be revalidated first form
the Issuing agency.

CATEGORY A- Marine Deck Officers who are holders of STCW ’95 Certificates In the operational level

1. Medical Care-NAC
2. Proficiency In fast Rescue Boat (If applicant will board a ship equipped with fast rescue boat)
3. E CDIS (If applicant will board a ship equipped with ECDIS)
4. Management Level Course (mandatory after Feb.01,2004)
4.1 Interim arrangement for Management Level Course applicable only until Feb.01,2004,pursuant to MTC
Resolution No.6, series of 2002
1. Cargo Handling and Care of Cargo
2. Maritime Law for Ship’s Officers
3. MARPOL 73/78 Annexes 1&2
4. Collision Avoidance/SSBT and practical assessment
5. GOC (with at least one (1) year validity)

CATEGORY B-Marine Deck Officers who are holders of ’78 Certificates In the Management level AND have NOT been
previously Issued STCW’95 certificates.

1. Advance Fire Fighting –NAC


2. Personnel Safety and Social Responsibility-NAC
3. Medical Emergency First Aid-NAC
4. Medical care-NAC
5. GOC-with at least one (1) year validity
6. RSC- Radar Navigation, Radar Plotting and use ARPA
7. ARPA- (RNRPUA)
8. Proficiency in Survival Craft and Rescue Boat-NAC
9. Proficiency in fast Rescue Boat (If applicant will board a ship equipped with fast Rescue Boat)
10. ECDIS (If applicant will board a ship equipped with ECDIS)
11. Management Level Course (Mandatory after Feb.01,2004)
11.1 Interim arrangement for Management Level Course applicable only until February 01,2004.

CATEGORY C- Marine Deck Officers who will board seagoing ship requiring special training.

1. For Tanker Regulations V/I


a. Six(6) month the approved seagoing service appropriate to their duties on the type of tanker on which they
serve (must be supported by Certificate of Service from the Company)
b. Shore-based Fire fighting
c. Specialized trading on the type of tanker on which to serve
- Oil Tanker – NAC
- Chemical Tanker – NAC
- Liquefied Gas Tanker –NAC
2. For RO-RO Passenger Ship (Reg. V/2) and Passenger Ship Other Than RO-RO (Reg. V/3)
2.1 Crowd Management, Passenger Safety and Safety Training for Personnel Providing Direct Services to Passengers
in Passenger Spaces ( IMO Model Course 1.28) NAC
2.2 Proficiency in Crisis Management and Human Behavior Training in Passenger Safety, Cargo Safety and Hull
Integrity Training (IMO Model Course 1.29)- NAC
2.3 Familiarization Training on Board RO-RO Passenger ship ( Reg. V/2) / Passenger ship other than RO-RO( Reg. V/3)
V. 2X2 picture in proper uniform with white background
VI. Long brown envelope
