hereby solemnly affirm & state as follows:I completed my Diploma Course in the Year November 2010 and appeared for the Diploma Examinations 1st Year (April 2008,Nov 2008, April2009, Nov 2010),IIIrd Sem (Nov2008),IVth Sem (April2009, Nov 2009), Vth Sem (Nov 2009), VIth (April 2010) with Register Number 87030827. My Diploma certificate is issued to me with Register Number 87030827. The same was irrecoverably lost from my custody in transists from Ernakulam to Mutholapuram after attending an interview and they are irrecoverably lost . I made all my efforts to get the above certificate. Now I am convinced that the certificate is irrecoverably lost. I need a duplicate copy of the certificate. I am swearing this affidavit for getting a duplicate of Diploma Certificate and submit that I am highly in need of a duplicate of Diploma Certificate as early as possible.

If at any time the lost Certificate is recovered it will be surrendered to the concerned authority and I shall not misuse the above Certificate for any other purpose. The above facts are true.
