Introduction:
Heaven Princeton (pseudonym) is a 31-year-old, African American, homosexual female,
who currently resides at home with her 79-year-old father. Heaven was born and raised in
Cleveland, Ohio. She currently works 2 two jobs. Ms. PrincetonHeavenHeaven is employed at
Legacy Village; she has been an Operation Associate with this company for 6 six months. She is
also the office manager at Rio Pro Brazilian Jiu-Jitsu (Martial Arts School) where she has been
employed for 16 months. In the past, the clientshe has had suffered from financial issues due to
her side hobby as an event planner; which is the reason behind her living arrangements with
her father. Ms. PrincetonHeavenShe has plans on movingto move in April to her own apartment.
Clients Her sole purpose for coming to see a social worker is because of the high stress levels
that she is experiencing due to recently finding out that her partner of 6 six years is no longer
happy in their relationship. In addition, the clientshe worries about moving from her fathers
apartment in April, due to his age and current medical conditions. These issues have been
affecting her overall performance at work and social activities. This has been an ongoing
problem for approximately 2 two years. Ms. PrincetonHeavensHeavens ethical dilemma in this
situation is whether or not to move closer to be with her partner, finding ways to cope with such
high stress stresslevels, or whether or not to just let go of her relationship and move on with her
life.
Family of Origin History:
Clients Heavens mother Rosalyn Jefferson received her high school diploma and has
some college experience. Her father, William McCollom, received high school diploma, didnt
did not attend college, but was active in the military (full-term). Clients Her mother was 27 and
her father was 48 at time of clients Heavens birth. The pregnancy was not planned. The
clientsHer parents were not married at time of her birth but were cohabitatingliving together.
Clients Her parents later married when she was 4four-years-old. Ms. PrincetonHeavenHeaven
states that home life growing up was fairly wellgood. Her family lived in an
impoverished area but was more fortunate then than others in the area.
Family trips were taken, i.e.,in places such as Disney World and Niagara Falls.
Clients Her social life was affected due to the fact that they were more
fortunate than others in their environment. Holidays were spent at home or
in Alabama at her mothers parentsmaternal grandparents house. Clients Her
parents later separated and her father moved out. Client She was 14-years-old at the time of the
separation and states that she took the break-up hard by rebelling. Reason for the separation was
infidelity and physical abuse.

Ms. PrincetonHeaven has 3 three siblings. Her younger sister, Holly is a single 22 years
old sophomore at Cleveland University. Holly suffers from Cerebral Palsy, epilepsy
epileptic seizures, vision problems, and the right side of her body was
paralyzed at birth. Ms. PrincetonHer fraternal twin sisters(?) Dominique McCollom is
age 31 years of age, healthy, and married with 3 three children. She,
received her high school diploma and is currently enrolled at a University
taking online courses. Dominique is a financial consultant at Cleveland Clinic
and has been employed there for 7 seven years. Client She has an older
brother, William Jefferson, who is 44, single, and works for Just Energy as a
Sales Rep. Williams health is fineWilliam is healthy, but his only problems
are with his vision. William received a high school diploma, but didnt
decided not to further his education. Ms. PrincetonHeavens fathers current age
is 79, h. He is has high blood pressure, and currently wears glasses to correct
his visionsee better. Her father has owned his own barbershop for over 40
years. Clients Her mothers current age is 57, and she wears glasses for
reading. Ms. PrincetonHeavens mother is relatively secretive when it comes
to her health. Ms. PrincetonShe knows that her mother suffers from arthritis, but
knows there are more health issues or concerns. I was unable to obtain
information about death or sexual abuse due to the potential cause of
distress for the clienther. Client She looked dejected when asked about the
death and sexual abuse.
Developmental History:
Ms. PrincetonHeaven was born prematurely by one month. She weighed 5 lbs.five
pounds at birth. At the time of the her birth, the clientshe suffered from asthma but later out grew
it. Ms. PrincetonHeavens mother delivered her vaginally and she was bottle-fed. Ms.
PrincetonHeaven did not have any feeding issues and was weaned off the bottle by age 2two.
Client She mentioned that she began walking around 9 nine months and began saying little
words by 11 eleven months. She was potty trained by at the age of 2two. Client She was never
told that she had developmental delays but was enrolled in a speech class as child to correct
certain speech patterns.
Health History:
Clients Heavens health at birth was good; she had no childhood illnesses or chronic
illnesses. Ms. PrincetonShe did tear her left ACL during horseplay with friends and received
surgery. As an adult, she does not have any chronic illness or health problems. She did receive
surgery on her right knee in 2010 for a torn meniscus. Client She currently has no weight issues
or family members who are ill. Ms. PrincetonShe started her menstrual cycle at the age of 12.
Client She stated that she has been in approximately 5 five car accidents but was never seriously
injured. Ms. Princeton isntShe is not currently taking any medicine. In addition, she has no
known allergies to any medications.
Relationship/Sexual History:

Client Heaven grew up in an impoverished neighborhood. She had a hard time keeping
maintaining friends due to the vastly different lifestyle that her she and her peers lived. Client
She stated that kids children would pick on her, which resulted in several fights during her
formative years. Ms. PrincetonHer home experienced frequent break-ins more than anyone else
in our neighborhood. High school was different than middle school considering the clients her
parents separated during this time. During high school, client she met her friends Gloria, Marty,
and Amber, who currently remain active in her life. Client She has strong friendships, and the
majority of her friends are from her high-school years. Ms. PrincetonShe stated, I usually keep
my friends around and they will be around a lifetime. She looked happy when talking about her
close circle of friends.
Client is a homosexual, but she did loose lose her virginity to a male at the age of 14. She
began dating seriously at age 18, where she revealed to her family that she was a lesbian. Client
She enjoys sex, and orgasms are perfectly normal with her sexual partner. Clients Her current
sexual partner is her partner of 6 six years. Client She was on birth control in the past but
currently isntis not. Ms. PrincetonHeaven has not had any sexually transmitted diseases.
Family of Creation History:
The clientShe is currently in a committed relationship, but she is not married or has never
been. Ms. PrincetonHer partner was 21 at their age of commitment. Client She and her partner
dont do not have any children. Reasons for the breakups within her past committed relationships
were due to mistrust and uncertainty. Clients Her partner received her high school diploma and
is currently a junior at Oakland University. Her partner has never been pregnant. Ms.
PrincetonHeaven and her partner have had a few physical altercations in the past, due to control
issues. Clients body language seemed as though she was interested when talking about her
partner.
Substance Use/Abuse History:
Client Heaven drinks alcohol moderately. She states that she never used any hard-core
drugs but has smoked marijuana in the past. Ms. PrincetonShe does not have any addictions; so
therefore treatment was not needed.
Mental Health History:
Ms. PrincetonHeaven received counseling during the time of her parents divorce. Client
She cannot recall the location of her counseling sessions. The whole entire process lasted six
months and was successful in helping client her adjust to her new lifestyle. No medications were
prescribed at the time. Overall, clients her mental health is wellgood. Ms. PrincetonHeaven
described a few months ago that she felt spiritually, mentally, physically, and emotionally broken
due to certain factors in her life.
Military History:
Client Heaven has had no military history and does not plan to join the military any time
soon.

Financial History:
Clients Heavens family financial history growing up was fairly well good as she stated.
Her mother and father had well- paying jobs, they drove luxurious vehicles, wore designer
clothes, and received thebought things that they not only needed but also wanted. In 2012, client
was in a financial whirlpool due to a bad investment. Ms. PrincetonShe states that it took her a
year to fully recover from the debt. Client She expressed that she is more responsible with
money due to past experiences. Client She is currently investing money properly for plans for her
future and goals.
Legal History:
Client Heaven has a history of legal trouble; all being traffic offenses. Ms. PrincetonShe
received her first DUI in 2011, in addition to driving under a suspended license. Client She was
arrested and later placed on house arrest.
Immigration Issues:
Client Heaven is a U.S. citizen and she has no cultural issues relevant to her mental health.
