The movie Shutter Island took place in 1954. The plot was about two U.

S Marshalls (Edward
Daniels and Chuck Aule) who traveled to an institution where criminal with mental health issues
were locked away. The two US Marshalls went to this institution to investigate the disappearance
of a patient Rachel Solando who was incarcerated for drowning her three children. Shortly after
the Marshalls arrived on the island there was a terrible storm that forced them to be stationed on
the island for a few days. One of the detectives Daniels finds the staff unusually confrontational,
and the lead psychiatrist Dr. Cawley refused to hand over hospital records containing staff
information. When the two Marshalls tried searching ward C, they learned that, that part of the
hospital was of off limits to be searched. They were told by the staff that ward C had already
been searched, so there was no need to search it. The airy feeling that surrounded the hospital
was causing Daniels to have migraine headaches, blurry vision, and disturbing dreams of this
dead wife, who was set on fire by a local arsonist. As the two Marshalls continued their
investigation, they find out that the staff at the hospital found Solando without any explanation.
Nothing about the investigation made sense to the Marshalls, so they decided to go off on their
own without the help of the staff or patients help. While they continued the investigation the two
Marshalls met a man named Gorge Noyce who was a patient staying in solitary confinement.
Gorge tells the Marshalls that the hospital is performing questionable experiments on the
patients. He also states that the patients which the hospital deems incurable are sent to ward C to
be lobotomized. As the Marshalls were leaving Gorge takes Daniels aside in informs him that
everyone in the hospital in playing a sick game on him, including his partner Chuck Aule. After
Daniels receives this news, he regroups with Aule, but the reunion was short lived because while
attempting to go around the cliffs they become separated. On his way back he spots a woman
hiding in a cave, the woman claims that she is the real Rachel Solondo. The woman claims that

she was once a psychiatrist at the hospital, but after she found out that the other psychiatrists
were using psychotropic medication to control the minds of the patients, she attempted to alert
the authorities, but the hospital did not want their secret revealed, so they committed her as a
patient to prevent her from leaving. Although Daniels was confused he returned to ward C where
he found Dr. Crawley waiting for him. Dr. Crawley reveals to Daniels that he was the one that
killed his wife, and he was in fact one of their most dangerous patient in the hospital. Dr Crawley
revealed to Daniels that he was a part of an experiment to see how he would react to being of his
medication, and that the whole thing was staged. After observing Daniels the doctors came to the
realization that he was incurable, and sent him off to be lobotomized.
When an individual becomes ill there are many physical effects that can be challenging to that
individual, their family and support system. There are three significant physical effects such as; a
lack of sleep, aches and pains due to the increase in stress, and having a balanced diet. I chose
these three physical effects because I believe it is very important for these effects to be well
maintained. Sleep plays an important role in ones physical health. It is significant for healing
and repairing your body. Having good sleeping habits is detrimental to a patient that is
undergoing a health challenge. A lack of sleep combined with a health problem may contribute to
faster deterioration of the patients body. For example, ongoing sleep deficiency may lead to
increased risk of heart disease, kidney disease, high blood pressure and stroke. It is also
important for the family member of the patient to maintain good sleeping habits, they will be
able to focus their attention on the patient, and provide a positive environment for the patients
healing.
Aches and pains due to stress can have a negative impact on patients success in getting better.
Pain can diminish job performance, decrease your motivation to exercise, and can disrupt

peoples daily living and routines. It is almost impossible for a person to be motivated to
physically do anything when they are in pain. It is stressful enough or a patient and their loved
ones to find out that there is a challenge on their health, and to have to worry about pain
intensifies the situation.
Having a balanced diet is very significant because it supplies your body with the nutrients it
needs to function. Also, having a balanced diet is important because it will help your body to
fight diseases and infections. This is especially vital for a patient that is trying to overcome an
illness. It is imperative that family members and the support surrounding the patient maintain a
balanced diet as well because not only will this improve their physical health, but it will help to
promote good mental function.
There are many psychosocial factors that affect the effects of health. A lot of the times patients
and their family members do not realize the strain that most psychosocial effects could have on
their lives and their environment. Some of these psychosocial effects include, coping, depression.
It is important for these individuals to find effective coping mechanisms to cope with their illness
because that will allow the individual to handle negative feelings and anxiety that they are facing
due to their illness. Coping with health challenges may involve dealing with a large amount of
stress. There are ways on how an individual may cope with the stress, for example, joining a
support group with people that are in similar situations. Joining these groups may help to
decrease the feeling of overwhelmed, sense of loneliness and isolation. Another way that might
help an individual cope with their health challenge is to receive counseling from a trained
professional. This will help the individual to express true emotion, and encourage self reflection.
A good support team is always important when an individual is encountered with an illness. It is

imperative to have people around you that love and care about you, its also important that the
individual trust their support team.
Having to face a health challenge that causes the individual to become depressed is a very
serious problem. Depression can lead these individuals to feeling sad, empty, and lonely.
Depression may cause the individual to withdraw from society and not wanting to be around
friends and family. The most significant problems that depression can cause are thoughts of death
and suicide. When and individual becomes depressed it is vital that they have a very good
support team. Having a good support team can help an individual maintain a healing connection
to people while dealing with their depression and their health challenge.
The priority effect that I will be focusing on is coping. It is extremely detrimental that the
nurse focuses on providing holistic nursing care to help the patient cope with their illness, and
their challenges of health. In the movie Shutter Island, the main character Daniels, had a
mental health issue. He did not have a solid grip on reality. He was hallucinating constantly and
could not come to terms with his wifes death. The healthcare team at the institution did not try to
help this patient cope with his illness. Instead of proving interventions to help to increase the
patients health, they tormented him by taking him off his medication, and playing games with
his mind. The patients health did not increase because there were no methods put in place by the
health care providers to help him cope. The healthcare provider should teach the patient that
there are effective coping mechanisms, and ineffective coping mechanisms. One example of
ineffective coping mechanisms is; Denial: "Everything's fine, no problem" but obvious evidence
points to real sources of conflict or concern (Building Resiliency, promoting greater quality of
life and improved physical functioning). Another ineffective coping mechanism is turning to
alcohol, mood altering drugs, over eating or isolating oneself (Building Resiliency, promoting

greater quality of life and improved physical functioning). It is the healthcare professionals job to
encourage the patient to use effective coping mechanisms such as; taking it one day at a time,
face stressors by acting directly when possible (Building Resiliency, promoting greater quality of
life and improved physical functioning). Another effective coping mechanism is, establish clear
and realistic goals for your personal and professional lives. Manage time; don't let it manage you
(Building Resiliency, promoting greater quality of life and improved physical functioning).
A nurse must have specific nursing-interventions that focus on coping; this will help to ensure
success when caring for the patient. I have proposed three nursing interventions that focus on
coping. Firstly, use verbal and non-verbal therapeutic communication approaches including
active listening, empathy and acceptance to encourage the patient to verbalize fears and
concerns, and express negative emotions. Secondly, monitor the patient for risk of harming
themselves or others, and intervene as necessary. Thirdly, offer the patient instructions regarding
alternative coping strategies, such as using physical activity as a distraction, and deep breathing
exercises.
Interprofessional collaborative care is important when providing care to a patient. It takes a
team of healthcare providers to come together and create a plan of care to ensure a successful
outcome for the patient. The use of interprofessional health care strategies to help a patient cope
fosters team problem-solving and better coordination between healthcare workers. I have
proposed three collaborative care strategies when dealing with patient coping. Firstly, the
collaboration with a social worker is important because they are trained to establish and facilitate
client and caregiver support groups. Secondly the collaboration with an occupational therapist is
important because they are trained to evaluate capabilities and determine the types of assistance
needed for coping strategies. The occupational therapist will also help to modify the environment

to help the patient to problem solve, reduce stress and increase communication. Thirdly,
collaborate with patient to identify personal strengths and coping mechanisms that were
successful in the past during times of stress.
There are a lot of resources within the community that will help individuals to use effective
coping mechanisms when dealing with health challenges. Utilizing community resources can
play an integral part in stabilizing and or maintaining the function of a patient and their family.
A good support group is essential for helping with coping. Support groups focus on reflective
practice, and self care. There a lot of resources that are offered online or over the telephone, such
as, Health Link, this website gives individual access to free non-emergency issues for an
individual, or a family member. They answer questions about mental health concerns, and you
can speak with a registered nurse about any symptoms you may be experiencing or speak with a
pharmacist about medication questions. To be able a access these resources so quickly will help
the individual to cope better with their health challenge because they can get answers and help
right away, which may decrease their stress and they will have a more positive outlook on the
progression of their health.
