I, Aaluri Yeliya S/o.Late Ramaiah, aged 45 years, Hindu, resident
of D.No.6-51, S.C. Colony, Elchuru Village, S.Magulur Mandal,
Prakasam District, do hereby solemnly affirm and sincerely state as

follows:-

I am the deponent herein and 1st petitioner in the above O.P. The 2nd
petitioner is my wife.

I am giving this affidavit on behalf of 2 nd petitioner

also. The above O.P is filed for compensation for the death of my Son, who
died in a Motor accident.

I submit that I did not file any similar application in any court of law for
the self same relief nor I did not entrust the same to anybody except the
Advocates, Sri.K.Ramesh Babu, V.Venkata Pavan Kumar, who are filing the
above O.P.
