Summary
I have been a nurse for over 20 years and have practiced in many different settings. Currently I practice in a
diabetes and heart failure (HF) clinic with a focus primarily on teaching diabetes patients self-management
skills. I am the program coordinator which ensures American Diabetes Association credentialing. My goal
is to obtain my Certified Diabetes Educator (CDE) certification within the next year. I have also worked
as a team lead in the diabetes and HF clinic which has provided me with the opportunity to assist with the
management of daily activities such as scheduling, day-to-day planning, operation and problem-solving needs
to meet the highest level of service. I have previous and extensive experience in cardiac care working on a
step down cardiac unit for over 6 years. This has provided me with experience in telemetry and advanced
cardiac life support (ACLS) with focus on assessment, diagnosis, planning, treatment and evaluation of the
preoperative and post- operative surgical cardiovascular patient with emphasis on rhythm interpretation
and hemodynamics. Previous to that, I worked as an Orthopedic Nurse and received specialized training to
care for patients with diseases and disorders of the musculoskeletal system. I have been trained in external
fixation care, neurovascular status monitoring, traction and continuous motion therapy. I have experience as a
referral manager for a major insurance company in utilization management. As a referral manager, I provided
oversight for the referral and procedural authorization process to ensure effective and efficient workflow
processes. I am dedicated to setting an example and doing what it takes to get towards the next step to a vision.
I thrive during change and chaos and believe that it will eventually bring about an unexpected but critical
solution to any problem. I believe patient-centered care should be the priority and model for the future of health
care.

The registered nurse (RN) in the diabetes program assists in implementation and facilitating the programs
goals and objectives designed to provide diabetes education to patients diagnosed with diabetes. The RN
is responsible for providing group and/or individual diabetes self-management education/training (DSMT)
to patients and family members that enable them to learn techniques to bring their disease under control
and avoid potential complications and bring about lasting improvements in quality of life. A program
coordinator, plans, implements and evaluates the diabetes education and continually reviews the program and
processes to assure that the program complies with recognition requirements.

As a RN on the cardiac unit I was trained in Advanced Cardiac Life Support (ACLS) with focus on

patients with emphasis on Cardiovascular Surgery including rhythm interpretation, hemodynamics, airway
management, pain management, medication management, safety precautions, patient/family education and

and advanced medical care for patients suffering from various types of heart problems or problems in the
cardiovascular area. Patients who are hospitalized due to life-threatening ailments such as coronary artery
