penalty of the State of Utah having been made to me by Detective MARTIN ERNEST
WEBB of Provo, this day, I am satisfied that there is probable cause to believe
THAT
On the person(s) of: Sina Belgard 01/31/1966;
In the City of Provo, County of Utah, State of Utah, there is now certain property or
evidence described as:
Hair and hair follicles and blood for drug testing
and that said property or evidence:
Was unlawfully acquired or is unlawfully possessed;
has been used or is possessed for the purpose of being used to commit or
conceal the commission of an offense; or
is evidence of illegal conduct.
Affiant believes the property and evidence described above is evidence of the crime
or crimes of Child endangerment and negligent homicide.
YOU ARE THEREFORE COMMANDED:
to make a search of the above-named or described person, vehicle, and/or premises
for the herein-above described property or evidence and if you find the same or any
part thereof, to bring it forth with before me at the FOURTH DISTRICT COURT - ALL

AFFIDAVIT FOR SEARCH WARRANT

The undersigned affiant, Detective MARTIN ERNEST WEBB of Provo, upon an oath
or written affidavit subscribed under criminal penalty, declares:
That your affiant has reason to believe:
THAT
On the person(s) of: Sina Belgard 01/31/1966;
In the City of Provo, County of Utah, State of Utah, there is now certain property or
evidence described as:
Hair and hair follicles and blood for drug testing
and that said property or evidence:
Was unlawfully acquired or is unlawfully possessed;
has been used or is possessed for the purpose of being used to commit or
conceal the commission of an offense; or
is evidence of illegal conduct.
Affiant believes the property and evidence described above is evidence of the crime
or crimes of Child endangerment and negligent homicide.
The facts to establish the grounds for issuance of a Search Warrant are:
I am a Police Officer for the Provo City Police Department. I have been
a peace officer since 2007. I attended and graduated from the Utah Police
Academy (POST). I am currently assigned as a detective in the Special Victims
Unit. I have received specialized training on crimes against persons, child

exploitation, child sex abuse, child death investigation, child forensic interviewing,
and multiple trainings from the Conference on Crimes against women, Conference
on Child and Family Maltreatment, Internet Crimes Against Children Task Force,
Utah County Sex Crimes Task Force, Federal Bureau of Investigation, and
the Childrens Justice Center. I have conducted numerous investigations as a
detective relating to special victims and crimes against persons.
On 12/01/15, David and Sina Belgard who live at 509 W 1800 N in Provo allowed
Casey Cormani, Cassandra Leydsman/Richards and their daughter 1 year old
Penny Cormani to stay in their home.
On 12/02/15 1 year old Penny Cormani woke up at 730 and played in a downstairs
living room of 509 W 1800 N. Cassandra, the mother of Penny, reported feeding
Penny and doing laundry as Penny played in the front room. Cassandra put Penny
down for a nap around 11am with a bottle of approximately 3 oz of vitamin D milk.
Cassandra went to check on Penny at noon and found that she had purple or blue
lips and was not responding. Cassandra took Penny downstairs and attempted
CPR. Cassandra called 911. Provo paramedics responded and transported Penny
to the UVRMC where Penny was later declared deceased.
Residents David and Sina consented to a search of their home. I found numerous
items of drug paraphernalia including burnt foil with residue and straws with burnt
ends and residue. Burnt foil with residue and burnt straws were found inside of
the room that Cassandra and Casey stayed in both on the ground and among
Cassandra's personal belongings. Burnt straws were also found on the carpet
and on a coffee table in the living room which is a common area downstairs. Also
found downstairs in the common area was a razor blade on an end table that had
a white residue on one side. All of the identified paraphernalia was visible in plain
sight. Burnt foil was found in the trashcan of the common and only bathroom in
the apartment. David disclosed that he had observed both Cassandra and Casey
using straws to inhale their prescriptions. Cassandra and Casey both deny any
illicit drug use and state that they consume their medications orally as prescribed.
Both Casey and Cassandra reported that the paraphernalia belonged to either
David or Sina.
Cassandra reported that only she and Casey have taken care of Penny for the last
two weeks and that only Casey, Cassandra, David and Sina were in the house
with Penny.
Penny was transported to the Office of the Medical examiner where an autopsy
was performed. Tests returned from the Autopsy that indicated that Penny had a
lethal amount of Heroine and that Codine was also present.

A criminal history check of Sina Belgard indicates that Sina has been arrested
and/or convicted for charges including DUI, Possession of drug paraphernalia,
possession of heroin and forged prescriptions.
WHEREFORE, your affiant prays that a Search Warrant be issued for the seizure of
said items.
I declare under criminal penalty of the State of Utah that the foregoing is true
and correct.

The personal property listed below or set out on the inventory attached hereto was
taken from the person of Sina Belgard 01/31/1966, by virtue of a search warrant dated
the 15th day of December, 2015, and issued by Magistrate JAMES BRADY of the
FOURTH DISTRICT COURT - ALL DEPARTMENT:
blood and hair with hair follicles.
I, Detective MARTIN ERNEST WEBB of Provo, by whom this warrant was executed,
do swear that the above listed or below attached inventory contains a true and
detailed account of all the property taken by me under the warrant, on the 15th day
of December, 2015.
All of the property taken by virtue of said warrant will be retained in my custody subject
to the order of this Court or of any other court in which the offense in respect to which
the property, or things taken, is triable.

I declare under criminal penalty of the State of Utah that the foregoing is true
and correct.
