Affidavit of Discrepancy
I, ILDEFONSO CLETO LARRAZABAL CORTES, JR, of legal age,
married, Filipino citizen and a resident at #8 Tamparong Apartments, LunaVelez Streets, Cagayan de Or0 City, after having been duly sworn to in
accordance with law, hereby depose and say:
1.That I am habitually and continuously using the name ILDEFONSO
CLETO LARRAZABAL CORTES, JR in all my records with particular
reference to my Firearm License with Control No. I-F072288 and my
Philippine Passport with No. EC1853362 that will expire on August 10,
2019 ;
2. That in my Certificate of Live Birth, my name appears as ILDDIFONSO
CLETO LARRAZABAL CORTES, JR, and in my Baptismal Certificate,
my name appears as ILDEFONSO LARRAZABAL CORTES JR;
3.That the discrepancy is due to inadvertence and with no intention to
evade any violation of the law;
4.That under the penalty of perjury, I hereby manifest that the names
ILDEFONSO CLETO LARRAZABAL CORTES, JR, ILDDIFONSO
CLETO LARRAZABAL CORTES, JR and ILDEFONSO LARRAZABAL
CORTES JR refers to one and the same person;
5.That I execute this affidavit freely and voluntarily to attest to the truth of
the foregoing facts and for whatever purpose this may serve best.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 14 th
