The cost is $10.00 each week per bowler. Each player is put on a team of 3; or the child may
pick their own teammates (See below). This is a team sport and a 15 week commitment.
Each team member relies on their team members to make every effort possible to show.
The last week is when the children will have their roll off to see who WINS! They will then
have a small banquet with pizza and prizes, and a dance to celebrate how great they all had
done (more on this as we get closer to the end).
The kids will take the bus to Norwich Bowling Entertainment Center. They will leave the
school cafeteria at approximately 3:45 and arrive to the bowling center at approximately
4:15. They will return back to Sterling Community School at approximately 6:30. Parents
please arrive to pick up your child on-time. Parents you may also opt to pick-up your child
at bowling where a sign-out sheet must be signed. (Please take note we will be checking
licenses until we get to know everyone).

For Insurance Purpose:

In consideration for the privilege of participating in the 2016 Sterling Bowling League, the Sterling Recreation Authority
program I, the undersigned hereby agrees that
1. I understand that there are inherent risks involved in the 2016 Sterling Bowling League, including the risk of
serious physical injury or death and
2. I FULLY ASSUME ALL RISKS ASSOCIATED WITH THE 2016 Sterling Bowling League, EVEN IF DUE TO THE
NEGLIGENCE OF STERLING RECREATION AND THEIR AGENTS, SERVANTS OR EMPLOYEES, including but not
limited to collisions while transporting, contact from patrons and spectators; equipment failure; lack of safety
devices or inadequate safety devices; lack of warning or inadequate warnings; lack of instruction or inadequate
instruction; and the like.
I, for myself and for my heirs, assigns, successor, executors, administrators, and legal representatives, AGREE TO
DEFEND, INDEMNIFY AND HOLD HARMLESS THE TOWN OF STERLING AND THEIR AGENTS, SERVANTS OR EMPLOYEES
FROM any and all claims, suits or demands by anyone arising from my use of the TOWN OF STERLING facilities and
equipment including CLAIMS OF NEGLIGENCE ON THE PART OF THE TOWN OF STERLING AND THEIR AGENT,
SERVANTS OR EMPLOYEES.

I, for myself and for my heirs, assigns, successors, executors, administrators and legal representatives, HEREBY
RELEASE, AND AGREE THAT I WILL NOT SUE THE TOWN OF STERLING AND THEIR AGENT, SERVANTS, OR
EMPLOYEES for money, damages for personal injury sustained by me while using the TOWN OF STERLING
facilities and equipment EVEN IF DUE TO THE NEGLIGENCE OF THE TOWN OF STERLING AND THEIR AGENTS,
SERVANTS OR EMPLOYEES.

I HAVE READ THIS WAIVER, HOLD HARMLESS AGREEMENT AND RELEASE OF LIABILITY ANF FULLY UNDERSTAND ITS
TERMS. I FURTHER UNDERSTAND THAT BY SIGNING THIS AGREEMENT THAT I AM GIVING UP SUBSTANTIAL LEGAL
RIGHTS. I HAVE NOT BEEN INDUCED TO SIGN THIS AGREEMENT BY ANY PROMISE OR REPRESENTATION, AND I SIGN
IT VOLUNTARILY AND OF MY OWN FREE WILL.
