This letter is to inform you that I have received an incident report documenting your involvement in
an alleged violation of the following ---policy (it maybe residence life, housing, SSC, etc.)--:
List ALL possible violations. Example:
1.2 Self-Leadership: All students must conduct themselves in a manner that
promotes and supports the well-being of the community, its integrity, and
Christian values. For example, students should refrain from actively participating
in or being present during violations of community standards.
2.0 Breaking the Law: Students who break the law, either on or off campus,
will be subject to investigation and the disciplinary process. When a student is
charged by a federal, state, or local authority, the university will not request
special consideration for that individual because of his/her student status. The
university will cooperate fully with law enforcement agencies in the enforcement
of the law.
12.0 Hosting Disruptive Gatherings: No student living on or off campus can
host any disruptive party, gathering, or event which disturbs or impacts the
peace of another. This includes but is not limited to: disturbing the peace of
others; excessive noise; violent, offensive, disorderly behavior, or quarrelsome
conduct; traffic obstruction of public or private streets by crowds or vehicles;
litter; etc. Any host or student who participates in such an activity will be subject
to disciplinary process.
12.1 Hosting Gatherings Involving Alcohol or Drugs: Persons who host or
in any way assist or promote a gathering (on or off campus) that includes alcohol
given to underage persons, underage consumption of alcohol, illegal drug usage,
or any drunkenness will be subject to probable suspension or expulsion from the
university. Those living at the location where the party is held may be held
responsible as hosts regardless of who provides the alcohol.

In light of these concerns I would very much like to speak with you regarding your involvement. In
referring to your academic schedule, I noticed that you do not have any schedule courses on
date-- at time--. Therefore, please plan on meeting with me in the ---name of the
office-- for your hearing. If you cannot meet at this time, you must contact me by email or at
phone number-- at least 24 hours prior to this meeting to reschedule. Please be prepared to supply
several alternative meetings times when you call.
If you fail to comply with this request, a hearing will be conducted none the less, as it will be
assumed that you chose to waive your opportunity to hear and present evidence related to the
disciplinary process as discussed in the Student Standards of Conduct (add the website link). If
you are found responsible, sanctions will be placed without your input and further discipline action
may be taken, including suspension from the university.
