You may cancel this contract, without penalty or obligation, within Five (5) business days after the date your
enrollment payment is received.
If you cancel, any payment made by you under the contract will be returned within Five (5) business days
after the date of receipt by the seller of your cancellation notice.
To cancel this contract, mail, fax, or deliver a signed dated copy of this cancellation notice or other written
notice, to (Palm Beach Credit Professionals) at above address, not later than midnight of the 5th business
day after your enrollment payment is received.

DISCLOSURE STATEMENT
1. (Palm Beach Credit Professionals) shall,
upon the start of agreement, provide guidance
to Customer with regards to obtaining the
Customers credit reports from all three creditreporting agencies Equifax (CSC for Texas
Residents), Experian, and Trans Union via a
Consumer Credit Monitoring service that
updates consumer credit file data every 30
days.
2. (Palm Beach Credit Professionals) shall
also, within the course of 3 to 7 business days
of receipt of Agreement signed by Customer,
setup the Customers online file access that
will allow the Customer to follow the progress
of their services purchased. A unique login and
password will be provided to the Customer to
access their file online. The Customer will be
responsible for re-setting and securing their
own password.
3. (Palm Beach Credit Professionals) shall
provide guidance when the Customer is
determining the actions to take with each
account in regards to that Customers file.
4. (Palm Beach Credit Professionals) shall
prepare challenges for items appearing on the
Customer's credit report, which the Customer
indicates are inaccurate, incomplete, obsolete,
or may be in need of verification.

5. Each consecutive month while the Customer


pays the monthly service fee (See Exhibit A
for Explanation) (Palm Beach Credit
Professionals) shall prepare all follow-up
challenges, as per the Fair Credit Reporting
Act, Fair Debt Collection Practices Act, Fair and
Accurate Transactions Act and Fair Credit
Billing Act and transmittal of the same.

6. Every month the customer is responsible to
provide (Palm Beach Credit Professionals)
with all the necessary information for any
credit monitoring service in order for (Palm
Beach Credit Professionals) to track the
changes that have occurred regarding the
Customers credit file. The Customer from time
to time may receive responses from the 3
credit bureaus, Equifax (CSC Credit Services
for TX residents), Experian, & Trans Union, in
response to the dispute process and shall keep
these for their records, but does not need to
send them back to (Palm Beach Credit
Professionals) to continue the process.

acknowledging that all my questions have been answered. I also acknowledge and agree to the total
costs of these services.

Disclaimer: All registration completed online via www.pbcreditprofessionals.com/join authorizes a onetime
registration fee of $99, which covers the shipping and mailing cost for one year. After which 30 days from the
day of registration, client will be automatically billed $100 (per item each month) until per item removed
service is paid for and client has paid for each item removed.
REFERRALS
Palm Beach Credit Professionals has elected to offer its clients a wonderful referral service. Our
referral service is designed to reduce monthly payments and balance. Each client has the option
to receive $50 per referral for an unlimited amount of referrals. This $50 may also be applied to
the clients balance. Palm Beach Credit Professionals has elected as a courtesy to implement a
cap system for removed items. The client is not required to pay for more than 24 negative items

thats been removed. One successful referral will reduce the cap to 18 items, two successful
referrals will reduce the cap to 12 items, and three successful referrals will reduce the cap to 6
items, which means the client will not be required to pay for more than 6 items (after the third
referral, client will receive $50 credit per referral). The client must contact Palm Beach Credit
Professionals to be enrolled in this program before receiving referral benefits. (24 is the
maximum of items we will charge for, any additional items removed will be at no charge)
SETTLEMENTS
In an effort to ensure that were providing affordable credit repair service for all of our clients.
Palm Beach Credit Professionals have the right to offer its clients a settlement offer at any time
for high balances. This shall be negotiated between Palm Beach Credit Professionals and the
client. Both parties are required to sign a settlement agreement. Clients can also agree choose
to pay a flat rate of $600 for 6 months of service or $900 for 1 year of service before the 2 nd
round begins. This will save the client a great deal of money, because they will not be charged
per negative item removed. Client must contact Palm Beach Credit Professionals after round 1
has begun and before round two begins to take advantage of this offer. Payments are accepted in
the form of a money order or cashiers check.

EXHIBIT D - Limited Power of Attorney


This form is a limited power of attorney and will be used for the sole purpose of helping with the
restoration of your credit. This does not give us full power of attorney over any of your other affairs.

and authority, in and on my behalf, to act as my attorney-in-fact in order to undertake and perform the
following matters only as set forth herein: obtaining information, including but not limited to credit reports

bureaus, creditors, or collection agencies.
If negotiation of a debt is necessary I give (Palm Beach Credit Professionals), its officers, employees
and agents the right to discuss information to help resolve a debt. I hereby authorize (Palm Beach
Credit Professionals), its officers, employees and agents this release or copy thereof within (12)
months of this date to obtain any information in my credit report that may involve medical records and/or
credit records.

presentation of this Limited Power of Attorney. I hereby release the bearer of this authorization as well as
the recipient, including but not limited to, the Custodian of such records, Repository of the Court Records,
Credit Bureaus (TransUnion, Equifax, Experian, Innovis, Chexsystems), consumer reporting agencies, retail
business establishments, lending institutions, student loan agencies (public and/or private), from liability
including whatever kind which may at any time result to me, my heirs, family or associates, because of

Consumer Credit File Rights Under State and Federal Law

You have a right to dispute inaccurate information in your credit report by contacting the credit bureau directly. However, neither you nor any
''credit repair'' company or credit repair organization has the right to have accurate, current, and verifiable information removed from your
credit report. The credit bureau must remove accurate, negative information from your report only if it is over 7 years old. Bankruptcy
information can be reported for 10 years.
You have a right to obtain a copy of your credit report from a credit bureau. You may be charged a reasonable fee. There is no fee, however, if
you have been turned down for credit, employment, insurance, receive a free copy of your credit report if you are unemployed and intend to
apply for employment in the next 60 days, if you are a recipient of public welfare assistance, or if you have reason to believe that there is
inaccurate information in your credit report due to fraud.
You have a right to sue a credit repair organization that violates the Credit Repair Organization Act. This law prohibits deceptive practices by
credit repair organizations.
You have the right to cancel your contract with any credit repair organization for any reason within 3 business days from the date you signed
it.
Credit bureaus are required to follow reasonable procedures to ensure that the information they report is accurate. However, mistakes may
occur.

You may, on your own, notify a credit bureau in writing that you dispute the accuracy of information in your credit file. The credit bureau
must then reinvestigate and modify or remove inaccurate or incomplete information. The credit bureau may not charge any fee for this service.
Any pertinent information and copies of all documents you have concerning an error should be given to the credit bureau.
If the credit bureau's reinvestigation does not resolve the dispute to your satisfaction, you may send a brief statement to the credit bureau, to be
kept in your file, explaining why you think the record is inaccurate. The credit bureau must include a summary of your statement about
disputed information with any report it issues about you.

(b) Separate Statement Requirement.--The written statement required under this section shall be provided as a document which is separate
from any written contract or other agreement between the credit repair organization and the consumer or any other written material provided
to the consumer.
(c) Retention of Compliance Records.-(1) In general.--The credit repair organization shall maintain a copy of the statement signed by the consumer acknowledging receipt of the
statement.
(2) Maintenance for 2 years.--The copy of any consumer's statement shall be maintained in the organization's files for 2 years after the date on
which the statement is signed by the consumer.
