EMPLOYEE’S SELF-ASSESSMENT
INSTRUCTIONS
o Think about how you fulfilled the job's requirements. Think about your
achievements of both corporate and professional development objectives.
Print your completed Self-Assessment and provide this to your manager. Your
manager should then set up a time with you to discuss the Performance
Evaluation.
EMPLOYEE’S SELF-ASSESSMENT FORM
1. ASSESS YOUR PERFORMANCE SINCE YOUR LAST REVIEW (OR DATE OF HIRE):
I am always up for the challenge of learning new skills and receiving additional training.
I would like to develop new skills in different areas of the job so that I can be a versatile employee. An area
which needs improvement would be for me to be able to express my opinion when I am asked, instead of going
with the flow so often.
4. OTHER COMMENTS:
The LS9 team has been very helpful, supportive, encouraging, and attentive in incorporating me into this
company. All of my co workers have been very helpful in providing positive and constructive criticism which I
take with me and try to incorporate it into my everyday job.
