EMPLOYEE’S SELF-ASSESSMENT FORM

1. ASSESS YOUR PERFORMANCE SINCE YOUR LAST REVIEW (OR DATE OF HIRE):

Overall rating: Exceeds expectations

I have gone above and beyond to make sure that the laboratory is stocked with plenty of supplies before
there is a shortage of anything needed. A positive accomplishment that also contributes to this rating is that I
was able to learn the job here at LS9 rather quickly.

2. AREAS OF STRENGTH AND OPPORTUNITIES TO UTILIZE CAPABILITIES:

I am always up for the challenge of learning new skills and receiving additional training.

3. AREAS FOR IMPROVEMENT AND SPECIFIC ACTIONS TO IMPROVE:

I would like to develop new skills in different areas of the job so that I can be a versatile employee. An area
which needs improvement would be for me to be able to express my opinion when I am asked, instead of going
with the flow so often.

4. OTHER COMMENTS:

The LS9 team has been very helpful, supportive, encouraging, and attentive in incorporating me into this
company. All of my co workers have been very helpful in providing positive and constructive criticism which I
take with me and try to incorporate it into my everyday job.
