Preface by Dr. Heinz Maurer

Skin care is a basic human need: it starts with cleanliness. Besides its hygienic function, cleanliness also has an integrative social role in modern society. Therefore it has become a seemingly self-evident necessity in everyday life. However, this fact also creates considerable dermatological problems for a large group of people. My primary motivation for the development of sebamed was my experience in the Dermatological Department of the University of Bonn in the 1950ies. At this time a soap prohibition existed for patients with skin diseases. This hardly tolerable stress drove my search for a solution. The first successes with soap-free tensidic products encouraged me to carry on with the development of a preparation for the cleansing of problem skin.

Preface of the Scientific Advisory Board

In 2006, the average per capita spending on skin care in Western Europe (EC-countries, Switzerland and Norway) was 147 Euro, amounting to a total 4.2 Billion Euro spend on cosmetic products in Western Europe. This fact alone indicates the significance of skin care in modern society. Reflecting on this statistical data and the fact that skin care can have positive, as well as, negative effects on the skin, research activities, especially in the fields of Dermatology, Chemistry, and Pharmacy are necessary to elucidate the interactions between the organ skin, the environment, and skin care products; and to contribute to optimizing the efficacy and safety of skin care products. The value and rewards of such efforts are demonstrated by the essential benefits, that have been achieved for such population groups as the 20%, who suffer from a genetic disposition to dry skin, which is further provoked by the daily cleansing with soaps or syndets and deteriorated by this irritation of the skin; or the 80% of adolescents with predominantly moderate forms of acne. The deterioration of the latter skin condition caused by the regular use of soap, can be avoided by using an acidic syndet. In skin diseases, adequate skin care can provide substantial support of the therapy and healing process. Its not insignificant to mention the role that skin care plays, in contributing to the maintenance of the skin's health, and in the prevention of skin damage and diseases. Medicinal skin care requires intensive efforts in dermatological research . Dermatological research has made significant advances in recent years. This incremental increase in the understanding of the physiology and pathology of the skin has lead to establishing fundamental requirements for medicinal skin care and the development of objective methods to evaluate the efficacy and safety of products. State-of-the-art dermatological research calls for a scientific benefit-risk-analysis similar to that for drugs. This brochure documents the medical/scientific evaluation of the in-use benefit- risk analysis of sebamed products, which has played a prominent role from the very beginning. The presented studies also demonstrate the progress, which has been achieved in the evolution of the methods utilized for product testing and evaluation in the course of the last decades. Since 1992, the Scientific Advisory Board of Sebapharma has provided Sebapharma with its vital connection to the scientific community devoted to dermopharmaceutical research. The establishment of this interdisciplinary co-operation has proven to be of special value, which is expressed in the research results presented on the following pages.

The result was the market introduction of sebamed Cleansing Bar forty years ago in 1967. This created a new category: Medicinal Skin Care, in which sebamed is still the market leader after more than 30 years. While initially only persons with skin problems benefited from this innovation, which had rendered the soap prohibition obsolete, an altered view on skin care was slowly developing within the whole population. There was an increasing focus on skin care as a means of keeping the skin healthy and preventing skin diseases. Additionally, a growing number of people were encountering skin problems as an undesirable side effect of the regular use of increasing amounts of products, which impair or even damage the physiological functions of the skin. It is my aim to continually optimize the efficacy and skin tolerance of sebamed products through the perpetual adaptation of their formulae to reflect state-of-the-art dermatological research acquired through close cooperation with notable, international universities, dermatologists and pharmacists during the process of product development and elaboration. To this end sebamed bestows biannually the Heinz Maurer Award for dermatological research on the skins surface and its environment. The companys principle QUALITY THROUGH RESEARCH, which has been the foundation of the brand sebamed since the very beginning, also finds its expression in the large number of scientific publications, which are summarized in this brochure to give you, the reader, an objective impression of the product performance of sebamed medicinal skin care.

The acid mantle of the skin

The pH value of the skin is slightly acidic
The skin surface has a slightly acidic pH. This observation made by Marchionini (1929) was confirmed by later investigations (Korting 1992). Within the Stratum corneum and the hydrolipid film the average pH value lies between 4.5 and 6.0. In the deeper layers with living cells, in the epidermis up to the Stratum granulosum, the pH reaches the neutral range. The acidic pH is not homogenously present throughout the whole Stratum corneum but localized in clearly defined domains within and in between individual corneocytes with a non-linear gradient toward the Stratum granulosum, as shown by the advanced technology of Fluorescence Lifetime Imaging (FLIM; Behne et al. 2002) The skins acid mantle average pH of 5,5 present in the Stratum corneum and the hydrolipid film, predominately in the extracellular domains with a gradient towards the viable epidermis
(Fotos were kindly provided by M.J.Behne, Department of Dermatology and Venerology University Hospital Hamburg-Eppendorf, Hamburg, Germany)

The acid mantle stabilizes the ecological balance of the skins micro-flora
The acid mantle of the skin fulfills a variety of protective functions. As early as 1938, Marchionini found a relation between the skins surface pH and its colonization with microorganisms. The commensalic resident flora of the skin: Staphylococci, Micrococcae, Propionibacteria and coryneform bacteria (Dott 1992) lives predominantly from sebum, sweat, and corneocytes. It is more or less well adapted to the slightly acidic pH. The growth optima, however, vary markedly between the different species (Lucacz 1990).

(Dott, 1992)

This acid mantle was formerly thought to be created solely by sweat, sebum and metabolic products of the skin surface bacteria. New investigations also demonstrated the presence of a Na+/H+-anti-porter system in the membranes of keratinocytes and in the Stratum corneum which is expressed in the dependence of the cell differentiation and produces an acidic milieu by the exchange of H+ from the cells into the intercellular space (Hachem et al. 2005). Additionally, enzymatic processes like the liberation of free fatty acids from phospholipids by acid phospholipase A2 contribute to the establishment of the acidic pH (Fluhr et al. 2004) As a third mechanism of acidification the formation of urocanic acid derived from histidine as a breakdown product of filaggrin has been identified (Krien and Kermici, 20001). Free fatty acids produced by hydrolysis of sebum lipids by lipases from species of the skin resident flora are also acidifiers of the Stratum corneum (Bibel et al, 19892) The skin shows considerable resistance against pH changes, due to the redundant sources of hydrogen ions in the Stratum corneum described above, together with the buffer capacity of fatty acids produced and the long-known lactic acid/lactate system. An impairment of the alkali neutralization capacity and even elevated Stratum corneum pH values are present in various skin diseases and physiologically in newborns whose skin surface pH drops from near neutral to slightly acidic within a few days to weeks.

Between the various organisms there exists in part antagonistic interactions like competition for room and nutrition, ammensalism by excretion of substances inhibiting the growth of other species or impairing them directly (Dott 1992). Changes in the skins surface pH lead to quantitative and qualitative alterations in the skins micro-flora (Braun-Falco, Korting 1995).

The proliferation and differentiation of the skin cells is pHdependent

In cell culture, keratinocytes require a slightly acidic milieu for optimal proliferation at pH 5.0 and 7.0, respectively, the cell growth is markedly inhibited as compared with pH 6.0. The culture medium of the keratinocytes shows an acidification with time (Soehnchen 1990). The exchange of extracellular sodium cations against intracellular hydrogen ions found by Behne et al. (1999) may explain this observation.

The acid mantle supports the synthesis of barrier forming lipids within the Stratum corneum
Extracellular lipids undergo various enzymatic processes after their secretion from the Odland bodies at the transition between the Stratum granulosum and the Stratum corneum, which produce hydrophobic lipids from more hydrophilic precursors, for instance, ceramides from sphingolipids. For one of the enzymes involved, -glucocerebrosidase, Mauro et al. (1998) demonstrated a higher activity at pH 5.5 as compared with pH 7.4. Recovery of the barrier function after experimental impairment by lipid extraction from the skin with aceton in vivo as measured by an initial rise in transepidermal water loss (TEWL) and subsequent gradual decrease back to baseline values, was markedly delayed under exposure of the skin to a buffer with pH 7.4 as compared with a buffer with pH 5.5. The formation of free fatty acids by secretory phospholipase A2 as constituents of the barrier lipids from phospholipids secreted from the lamellar bodies is also a mechanism for the maintenance of the homeostasis of the acid pH value within the Stratum corneum. The specific inhibition of this enzyme leads to both an increase in the skin pH and a disturbance of the barrier function as well as decreased cohesion of the corneocytes resulting in a decreased integrity of the Stratum corneum (Fluhr et al., 2001) Recent studies have revealed a further role of the acid mantle in maintaining Stratum corneum cohesion and integrity and, on the other hand, the desquamation process through attenuation of the activity of serin proteases and probably also other enzymes involved in the break-down of corneodesmosomes. Increase in Stratum corneum pH inhibits some of the enzymes involved while acitvating others, leading to irregular desquamation and decreased Stratum corneum integrity and cohesion (Hachem et al., 2005). Besides the bilammellary lipid sheets, this feature may play a major role for the skins barrier function. The role of the pH for the skin barrier function has recently been repeatedly reviewed (Elias 2005; Lee, Jeung, Ahn, 2006; Schmid-Wendtner, Korting 2006; Korting, Schmid-Wendtner 2007)

The acid mantle stabilizes the barrier structures of the Stratum corneum lipids
The bilamellary structure of the intercellular lipids in the Stratum corneum is crucial for the skin's barrier function. Double lamellae form only at a pH range between 4.5 and 6.0, according to the findings of Osborne and Friberg (1987). At lower pH values the lipids are freely dissolved without any ordered structure. At pH values over 6.0 globular micellae form. Outside the pH range mentioned above, the barrier function of the intercellular lipids of the Stratum corneum is lost, resulting in an increased water loss from the skin and an enhanced penetration of exogenous substances to which the skin is exposed.

The influence of skin care on the acid mantle

Washing with soap impairs the acid mantle
Soaps: alkali salts of long-chain fatty acids by chemical definition, always form lyes in aqueous solution with pH values between 8 and 12. Acidic pH values cannot be established without loosing the surfactant activity. Even in healthy skin the alkali neutralization capacity is exhausted by a single wash process resulting in a prolonged increase in the skin surface pH (Kober 1990). This is not the case when the acidic syndet sebamed is used for washing. Repetitive washing with soap leads to a permanent though slight increase of the skin surface pH (Korting 1990). Even pH neutral skin cleansing influences the skin surface pH

Soap-free cleansing preparations contain synthetic detergents in short syndets as cleansing agents. Their surfactant activity, which they share with soaps, enables them to dissolve fatty soiling and thus cleanse the skin thoroughly. They have some advantages as compared with soap:

The chemical variability of the syndets is high as compared with soap. This makes it possible to adjust the skin tolerance and cleansing efficacy to the individual requirements. Syndets, unlike soaps, form no insoluble lime soaps in harsh, i.e. magnesium and calcium rich water. Thus they do not loose their foaming and cleansing efficacy and can be used economically, independent of water harshness. Probably this makes skin cleansing with syndets more skin friendly than soap in areas with harsh water (McNally et al., 1998) Syndets do not form lyes. The pH of syndet cleansers can be adjusted freely within a broad range. Usually, the pH is adjusted to a neutral or to the physiological, slightly acidic value of 5.5. When an alkaline syndet (pH 8.5, same composition of sebamed Liquid Face & Body Wash, except for pH) is used for skin cleansing, the acid mantle is disturbed in a similar way as by an alkaline soap (Zienicke 1992).

It falls back to the initial level much more slowly than after washing with the original sebamed Liquid Face & Body Wash with its pH 5.5 (Korting 1991).

Influence of the pH value on the dehydration of the skin

After washing the skin hydration first increases by an up-take of water resulting in skin swelling. The degree of this increase depends on various factors like the duration of the water exposure, the temperature, the pH value, and the quality and quantity of the cleansing agents applied. A comparison of cleansing preparations of different pH showed a minimal swelling at a slightly acidic pH. This is paralleled by structural damage to the epidermis with loss of nuclei of the keratinocytes at pH values above 9 (Gehring 1992). Skin swelling has to be considered as an adverse effect of skin cleansing, even though it causes a momentary pleasantly smooth skin feeling. Skin swelling is always followed by a loss of hydration of the Stratum corneum below the initial level. The more skin lipids and moisturizing factors are removed together with soiling, the stronger is the dehydration. The loosening of the Stratum corneum barrier structures by the previous swelling processes further amplifies the dehydration, just like pH-related conformation changes of the barrier lipids. In standardized washing procedures, Gehring and Gloor (1990) found a shorter lasting and less pronounced dehydration by soap-free syndet preparations adjusted to a slightly acidic pH (4.5 and the original value of sebamed Liquid Face & Body Wash, 5.5), than at pH 7.0 or 7.5. The incubation of human skin in vitro with Isopropylpalmitate and Cetiol HE caused a structural damage of the Stratum corneum, which impairs its barrier function. This leads to a swelling, which mediates a smooth skin sensation immediately after washing but gives way to an increased dehydration later on. Influence of the pH value of cleansing preparations on the skins micro-flora

The bacterial count of Propionibacterium acnes, a species with a narrow growth optimum between pH 6.0 and 6.5, increased under repeated washings with soap, but decreased when sebamed Liquid Face & Body Wash with pH 5.5 was used, as shown in a cross-over study (Korting 1992). The crucial factor for the decrease in bacterial count is the pH value, as in further tests the syndet preparation produced an increase in bacterial counts if its pH was adjusted to 7.0 or 8.5 (Zienicke 1990, Korting et al, 1990).

Influence of the application of skin care products on the pH value of the skin
While the influence of the pH value of cleansing preparations on the acid mantle, the skin hydration, its barrier function, and the resident skin flora is well known, the effect of the pH value of creams and lotions has scarcely been studied yet. Skin cleansing preparations are removed almost completely from the skin surface by rinsing after a short exposure period. Skin care preparations, by contrast, are left on the skin until the next washing, which usually is performed only after several hours. All skin care products which contain water have a definite pH value in their aqueous phase. In a study by Jacobi (1954) an alkaline cream (pH 8.3) caused an increase in skin surface pH which could be observed over 75 minutes. Gehring (2000) could show that even 3 hours after the application of an alkaline cream an increased pH value was still measurable on the skin surface, whereas the same formulation adjusted to pH 5.5 did not chance the skin pH.

Older syndet preparations often had dehydrating or even irritative effects, due to the content of cleansing agents like Sodium Lauryl Sulfate, which cause damaging lipid removal and disturb the barrier. Therefore, such syndets were predominantly used for skin diseases associated with seborrhoea. By contrast, many dermatologists still recommend soaps for dry skin. Modern syndet preparations with milder cleansing agents like sebamed Dry Skin Wash can be adjusted to the requirements of dry skin, leading to less dehydration and lipid loss than soaps.

Dermatological tests provided evidence that additives for re-lipidization in cleansing preparations cannot reduce dehydration or lipid loss. By contrast, standardized washing procedures with cleansing preparations which were diluted to give equal cleansing effects, resulted in more pronounced loss of water and squalen from the skin when two frequently used re-lipidizing additives were added than with the original preparation.

Baby sebamed
The best protection from the very first day
Baby skin is extremely sensitive and dehydrates easily due to its loose and soft structure. The protective acid mantle develops during the first weeks of life, but remains less stable and resistant until adolescence. Microbial colonization of the skin by apathogenic species is controlled by the acid mantle, which thus prevents skin infections. Therefore, baby requires cleansing and skin care formulated with a pH value of 5.5, highly efficient moisturizers for emollients, and maximum skin tolerance.

Baby Bubble Bath

The extra mild soap and alkali free tenside complex combines with emollients, moisturizers, and soothing chamomile to provide gentle cleaning for sensitive baby skin and protect it from dryness and irritation. To be added to the baby bath after the navel has healed. Dermatologically tested. Package size: 250 ml, 500 ml

Ingredients: Aqua, Cocamidopropyl Betaine, Sodium Laureth-6 Carboxylate, Disodium Cocoamphodiacetate, Sodium C14-16 Olefin Sulfonate, Chamomilla recutita extract, Sodium Lactate, Glyceryl Laurate, Propylene Glycol, PEG-55 Propylene Glycol Oleate, PEG-18 Glyceryl Oleate/Cocoate, Alcohol, Parfum, Phenoxyethanol, Sodium Benzoate.

The incidence of dry skin in the panel decreased in favor of normal skin during the use of the bubble bath, as did clinical scores for skin symptoms: eyrthema 57%, tension 90 %, dryness 57%, flaking 67%, stinging 81%, papules/pustules 54%. Skin surface lipids increased markedly while pH remained constant, and hydration decreased slightly over the observation period.

sebamed sun care

Triple protection for the skin
In the sun the skin absorbs vitamin D and reacts to UV exposure positively through the thickening of the Stratum corneum, which strengthens its resistance to environmental influences and promotes melanin synthesis. However the powerful UV rays of the sun can also cause irreparable injury to the skin through damage of the DNA, lipids and proteins in the cells and tissue; producing sunburn, sun allergies and increasing the risk of skin cancer. The skin absorbs UV-A (long-wave ultraviolet radiation) and UV-B rays (short-wave ultraviolet radiation) and this is where the highly effective sun filters of sebameds waterproof sun care products offer the best possible protection. All sun care products are readily absorbed by the skin and formulated with the pH value of 5.5 to protect the health of the skins acid mantle. Dry skin is a common skin problem with symptoms of tightness, roughness, scaling and which irritation, require special skin care. Independent of the cause, like genetic disposition for sebostasis, age hormones, skin diseases like atopic eczema or psoriasis, systemic diseases like diabetes, or medications affecting the skin like chemotherapy, sebamed Anti Dry offers a compelte program for uniquely mild cleansing, facial care and body care with phytosterols from rapesseed oil, which combine emollience with anti-inflammatory efficacy. All products are clinically and dermatologically tested. Suitable for children.

sebamed Anti-Dry
Regenerates dry skin, recommended therapy supportive skin care for atopic dry skin

Highly effective sun filters absorb the harmful UVA and UVB rays Vitamin E neutralizes free radicals to protect against premature aging and sunburn Highly effective moisturizing complex protects the skin, preventing dehydration Water resistant sun screens: All sebamed sun care products are water-proof. Sweating and bathing removes filter substances and pigments from the skin, decreasing the SPF. For this reason sebamed Sun Care has a special water-proof formula, which builds a protective film. However by prolonged water contact the sun protection is ultimately reduced. For this reason, it is important to reapply the sun care product. Rules for using sun screens: Sun lotions or creams should be applied 15 - 20 minutes prior to UV exposure. Reapplication after 150 minutes, or directly after extensive bathing and towel drying is necessary to maintain the labelled SPF. The adequate SPF should be chosen according to the individual skin type and exposure time. A special note: Children under 1 year of age should not be exposed to the sun. For older children, sebamed Sun Cream SPF 28 is recommended.

Protection and moisture for the sensitive facial skin
Facial skin is thinner and more permeable than the skin on other parts of the body. Furthermore, the face is much more exposed to noxious environmental factors like UV radiation, pollutants, and low air humidity. Additionally, skin alterations in the face are clearly visible, and therefore of higher concern to an affected person. Thus, it requires special care, protection and skin tolerance.
