The result was the market introduction of sebamed Cleansing Bar forty years ago in 1967. This created a new category: Medicinal Skin Care, in which sebamed is still the market leader after more than 30 years. While initially only persons with skin problems benefited from this innovation, which had rendered the soap prohibition obsolete, an altered view on skin care was slowly developing within the whole population. There was an increasing focus on skin care as a means of keeping the skin healthy and preventing skin diseases. Additionally, a growing number of people were encountering skin problems as an undesirable side effect of the regular use of increasing amounts of products, which impair or even damage the physiological functions of the skin. It is my aim to continually optimize the efficacy and skin tolerance of sebamed products through the perpetual adaptation of their formulae to reflect state-of-the-art dermatological research acquired through close cooperation with notable, international universities, dermatologists and pharmacists during the process of product development and elaboration. To this end sebamed bestows biannually the Heinz Maurer Award for dermatological research on the skins surface and its environment. The companys principle QUALITY THROUGH RESEARCH, which has been the foundation of the brand sebamed since the very beginning, also finds its expression in the large number of scientific publications, which are summarized in this brochure to give you, the reader, an objective impression of the product performance of sebamed medicinal skin care.
The acid mantle stabilizes the ecological balance of the skins micro-flora
The acid mantle of the skin fulfills a variety of protective functions. As early as 1938, Marchionini found a relation between the skins surface pH and its colonization with microorganisms. The commensalic resident flora of the skin: Staphylococci, Micrococcae, Propionibacteria and coryneform bacteria (Dott 1992) lives predominantly from sebum, sweat, and corneocytes. It is more or less well adapted to the slightly acidic pH. The growth optima, however, vary markedly between the different species (Lucacz 1990).

This acid mantle was formerly thought to be created solely by sweat, sebum and metabolic products of the skin surface bacteria. New investigations also demonstrated the presence of a Na+/H+-anti-porter system in the membranes of keratinocytes and in the Stratum corneum which is expressed in the dependence of the cell differentiation and produces an acidic milieu by the exchange of H+ from the cells into the intercellular space (Hachem et al. 2005). Additionally, enzymatic processes like the liberation of free fatty acids from phospholipids by acid phospholipase A2 contribute to the establishment of the acidic pH (Fluhr et al. 2004) As a third mechanism of acidification the formation of urocanic acid derived from histidine as a breakdown product of filaggrin has been identified (Krien and Kermici, 20001). Free fatty acids produced by hydrolysis of sebum lipids by lipases from species of the skin resident flora are also acidifiers of the Stratum corneum (Bibel et al, 19892) The skin shows considerable resistance against pH changes, due to the redundant sources of hydrogen ions in the Stratum corneum described above, together with the buffer capacity of fatty acids produced and the long-known lactic acid/lactate system. An impairment of the alkali neutralization capacity and even elevated Stratum corneum pH values are present in various skin diseases and physiologically in newborns whose skin surface pH drops from near neutral to slightly acidic within a few days to weeks.

Between the various organisms there exists in part antagonistic interactions like competition for room and nutrition, ammensalism by excretion of substances inhibiting the growth of other species or impairing them directly (Dott 1992). Changes in the skins surface pH lead to quantitative and qualitative alterations in the skins micro-flora (Braun-Falco, Korting 1995).

The acid mantle supports the synthesis of barrier forming lipids within the Stratum corneum
Extracellular lipids undergo various enzymatic processes after their secretion from the Odland bodies at the transition between the Stratum granulosum and the Stratum corneum, which produce hydrophobic lipids from more hydrophilic precursors, for instance, ceramides from sphingolipids. For one of the enzymes involved, -glucocerebrosidase, Mauro et al. (1998) demonstrated a higher activity at pH 5.5 as compared with pH 7.4. Recovery of the barrier function after experimental impairment by lipid extraction from the skin with aceton in vivo as measured by an initial rise in transepidermal water loss (TEWL) and subsequent gradual decrease back to baseline values, was markedly delayed under exposure of the skin to a buffer with pH 7.4 as compared with a buffer with pH 5.5. The formation of free fatty acids by secretory phospholipase A2 as constituents of the barrier lipids from phospholipids secreted from the lamellar bodies is also a mechanism for the maintenance of the homeostasis of the acid pH value within the Stratum corneum. The specific inhibition of this enzyme leads to both an increase in the skin pH and a disturbance of the barrier function as well as decreased cohesion of the corneocytes resulting in a decreased integrity of the Stratum corneum (Fluhr et al., 2001) Recent studies have revealed a further role of the acid mantle in maintaining Stratum corneum cohesion and integrity and, on the other hand, the desquamation process through attenuation of the activity of serin proteases and probably also other enzymes involved in the break-down of corneodesmosomes. Increase in Stratum corneum pH inhibits some of the enzymes involved while acitvating others, leading to irregular desquamation and decreased Stratum corneum integrity and cohesion (Hachem et al., 2005). Besides the bilammellary lipid sheets, this feature may play a major role for the skins barrier function. The role of the pH for the skin barrier function has recently been repeatedly reviewed (Elias 2005; Lee, Jeung, Ahn, 2006; Schmid-Wendtner, Korting 2006; Korting, Schmid-Wendtner 2007)

The bilamellary structure of the intercellular lipids in the Stratum corneum is crucial for the skin's barrier function. Double lamellae form only at a pH range between 4.5 and 6.0, according to the findings of Osborne and Friberg (1987). At lower pH values the lipids are freely dissolved without any ordered structure. At pH values over 6.0 globular micellae form. Outside the pH range mentioned above, the barrier function of the intercellular lipids of the Stratum corneum is lost, resulting in an increased water loss from the skin and an enhanced penetration of exogenous substances to which the skin is exposed.

The chemical variability of the syndets is high as compared with soap. This makes it possible to adjust the skin tolerance and cleansing efficacy to the individual requirements. Syndets, unlike soaps, form no insoluble lime soaps in harsh, i.e. magnesium and calcium rich water. Thus they do not loose their foaming and cleansing efficacy and can be used economically, independent of water harshness. Probably this makes skin cleansing with syndets more skin friendly than soap in areas with harsh water (McNally et al., 1998) Syndets do not form lyes. The pH of syndet cleansers can be adjusted freely within a broad range. Usually, the pH is adjusted to a neutral or to the physiological, slightly acidic value of 5.5. When an alkaline syndet (pH 8.5, same composition of sebamed Liquid Face & Body Wash, except for pH) is used for skin cleansing, the acid mantle is disturbed in a similar way as by an alkaline soap (Zienicke 1992).

The crucial factor for the decrease in bacterial count is the pH value, as in further tests the syndet preparation produced an increase in bacterial counts if its pH was adjusted to 7.0 or 8.5 (Zienicke 1990, Korting et al, 1990).

Influence of the application of skin care products on the pH value of the skin
While the influence of the pH value of cleansing preparations on the acid mantle, the skin hydration, its barrier function, and the resident skin flora is well known, the effect of the pH value of creams and lotions has scarcely been studied yet. Skin cleansing preparations are removed almost completely from the skin surface by rinsing after a short exposure period. Skin care preparations, by contrast, are left on the skin until the next washing, which usually is performed only after several hours. All skin care products which contain water have a definite pH value in their aqueous phase. In a study by Jacobi (1954) an alkaline cream (pH 8.3) caused an increase in skin surface pH which could be observed over 75 minutes. Gehring (2000) could show that even 3 hours after the application of an alkaline cream an increased pH value was still measurable on the skin surface, whereas the same formulation adjusted to pH 5.5 did not chance the skin pH.

Older syndet preparations often had dehydrating or even irritative effects, due to the content of cleansing agents like Sodium Lauryl Sulfate, which cause damaging lipid removal and disturb the barrier. Therefore, such syndets were predominantly used for skin diseases associated with seborrhoea. By contrast, many dermatologists still recommend soaps for dry skin. Modern syndet preparations with milder cleansing agents like sebamed Dry Skin Wash can be adjusted to the requirements of dry skin, leading to less dehydration and lipid loss than soaps.

Dermatological tests provided evidence that additives for re-lipidization in cleansing preparations cannot reduce dehydration or lipid loss. By contrast, standardized washing procedures with cleansing preparations which were diluted to give equal cleansing effects, resulted in more pronounced loss of water and squalen from the skin when two frequently used re-lipidizing additives were added than with the original preparation.

The well-established influence of the pH of skin cleansing preparations on the skin physiology supports the demand formulated by Yosipovitch and Maibach (1996) for slightly acidic skin care preparations with a pH of 5.5.

Results of an in use test with sebamed Liquid Face & Body Wash
sebamed Liquid Face & Body wash was used daily by 30 persons for 3 weeks. Bioengineering methods were applied to measure skin function parameters, comparing skin areas washed with water only to those washed with the test product.
