I understand that Turning the Page (TTP) is a non-profit corporation that provides Garfield Elementary
School with support, programs and resources (TTP program).

As part of Garfield’s participation in the TTP Program, I will participate in an arts, photography and
writing program called Community Visions. As part of Community Visions I will take photographs
and produce accompanying writing.

I will be loaned a camera from TTP to take photographs for the Community Visions program. I will be
loaned the camera, retail price $17.99, and I will be responsible for returning the camera to my teacher
at the conclusion of the program.

I hereby grant TTP the irrevocable, non-exclusive right to use, and to grant others the right to use, my
photographs and writing in connection with the TTP Program, and the publishing, advertising,
promoting and publicizing, thereof, in any and all languages, formats and media, without any
compensation to me whatsoever.

I hereby release TTP from all claims and liability of any kind whatsoever, arising out of or related to,
the exercise of any of the rights granted to TTP above, including, without limitation, for any violation
of any rights of privacy, publicity, defamation or any other personal or property right. I hereby
indemnify TTP against all claims, actions, losses, damages, judgments and liabilities resulting from
any breach or alleged breach by me of this release. In no event shall I have any right to seek or obtain
injunctive or other equitable relief with respect to TTP or its use of my photographs and writing.

This is the complete and binding agreement between us. This agreement cannot be changed except by
a written agreement, signed by all parties, and it shall be exclusively governed by the laws of the
District of Columbia.

I hereby warrant that I am the parent of and/or legal guardian for the person who signed above, that I
have read and understood the foregoing, that I have caused said person to sign above, that I will
indemnify TTP against all claims, liability and expense respecting the foregoing in the event of any
breach, and that, knowing of your reliance, I agree to cause said person to adhere to all of the terms
above.
