Rights of Patient

1. The patient has the right to considerate and respectful care irrespective of socio-economic status. 2. The patient has the right to obtain from his physician complete current information concerning his diagnosis, treatment and prognosis in terms the patient can reasonably be expected to understand. When it is not medically advisable to give such information to the patient, the information should be made available to an appropriate person in his behalf. H has the right to know by name or in person, the medical team responsible in coordinating his care. 3. The patient has the right to receive from his physician information necessary to give informed consent prior to the start of any procedure and/or treatment. Except in emergencies, such information for informed consent should include but not necessarily limited to the specific procedure and or treatment, the medically significant risks involved, and the probable duration of incapacitation. When medically significant alternatives for care or treatment exist, or when the patient requests information concerning medical alternatives, the patient has the right to such information. The patient has also the right to know the name of the person responsible for the procedure and/or treatment. 4. The patient has the right to refuse treatment/life giving measures, to the extent permitted by law, and to be informed of the medical consequences of his action. 5. The patient has the right to every consideration of his privacy concerning his own medical care program. Case discussion, consultation, examination and treatment are confidential and should be conducted discreetly. Those not directly involved in his care must have the permission of the patient to be present. 6. The patient has the right to expect that all communications and records pertaining to his care should be treated as confidential. 7. The patient has the right that within its capacity, a hospital must make reasonable response to the request of patient for services. The hospital must provide evaluation, service and/or referral as indicated by the urgency of care. When medically permissible a patient may be transferred to another facility only after he has received complete information concerning the needs and alternatives to such transfer. The institution to which the patient is to be transferred must first have accepted the patient for transfer. 8. The patient has the right to obtain information as to any relationship of the hospital to other health care and educational institutions in so far as his care is concerned. The patient has the right to obtain as to the existence of any professional relationship among individuals, by name who are treating him. 9. The patient has the right to be advised if the hospital proposes to engage in or perform human experimentation affecting his care or treatment. The patient has the right to refuse or participate in such research project. 10. The patient has the right to expect reasonable continuity of care; he has the right to know in advance what appointment times the physicians are available and where. The patient has the right to expect that the hospital will provide a mechanism whereby he is informed by his physician or a delegate of the physician of the patients continuing health carerequirements following discharge. 11. The patient has the right to examine and receive an explanation of his bill regardless of source of payment. 12. Magna Carta of Patients Rights and Obligations of 2008 PATIENTS RIGHT AN ACT OF PROCLAIMING THE RIGHTS AND OBLIGATIONS OF PATIENTS, PROVIDING A GRIEVANCE MECHANISM THEREOF AND FOR OTHER PURPOSES. The following individual rights of Patients shall be respected by all those involved in the delivery of Health Care Services: 1. Right to Good Quality Health Care and Humane Treatment Every person has a right to continuity of good quality Health Care without discrimination and within the limits of the resources, manpower and competence available for health and medical care. 2. Right to Dignity The Patients dignity, culture and value shall be respected at all times in medical care and teaching. 3. Right to be informed of His Rights and Obligations as a Patient Every person has the right to be informed of his rights and obligations as a Patient. 4. Right to Choose His Physician/Health Institution The Patient is free to choose the services of a physician or health institution of his choice except when he chooses to be confined in a charity ward. 5. Right to Informed Consent The patient has a right to self-determination and to make free decisions regarding him/her. However the attending physician shall inform the Patient of the consequences of his/her decisions. 6. Right to refuse Diagnostic and Medical Treatment The Patient has the right to refuse diagnostic and medical treatment procedures, provided that the following conditions are satisfied: o The Patient is of legal age and is mentally competent; o The Patient is informed of the medical consequences of his/her refusal; o The Patient releases those involved in his care from the obligation relative to the consequences of his/her decision; o The Patients refusal will not jeopardize public health and safety 7. Right to Refuse Participation in Medical Research The Patient has the right to be advised of plans to involve him/her in medical research that may affect the care or treatment of his/her condition. 8. Right to Religious Belief and Assistance The Patient has the right to receive spiritual and moral comfort, including the help of a priest or minister of his/her chosen religion. 9. Right to Privacy and Confidentiality The Patient has the right to privacy has the right to privacy and protection from unwarranted publicity.

10. Right to Disclosure of and Access to Information In the course of the patients treatment and hospital care, the Patient or his/her legal guardian has the right to be informed of the result of the evaluation of the nature and extent of his/her disease. 11. Right to Correspondence and to Receive Visitors The Patient has the right to communicate with his/her relatives and other persons and to receive visitors subject to reasonable limits prescribed by the rules and regulations of the Health Care Institution. 12. Right to Medical Records The Health Care Institution and the Physician shall ensure and safeguard the integrity and authenticity of the medical records. The Patient, upon his/her request, is entitled to a medical certificate and clinical abstract. 13. Right to Health Education Every person has the right to health education that will assist him in making informed choices about personal health and about available health services. 14. Right to Leave Against Medical Advise The Patient has the right to leave a hospital or any other Health Care Institution regardless of his physical condition, Provided, That: o He/she is informed of the medical consequences of his/her decision; o He/she releases those involved in his/her care from any obligation relative to the consequences of his/her decision; and o His/her decision will not prejudice public health and safety. 15. Right to Express Grievances Every Patient has the right to express valid complaints and grievances about the care and services received and to know the dispositions of such complaints.
