Honeywell HomMed Introduces LifeStream Services To Streamline Telehealth Delivery

New Managed Service Offering Provides Customizable Telehealth Support for Healthcare Providers
ORLANDO Oct. 22, 2012 Honeywell (NYSE: HON) HomMed, a global leader in telehealth and remote patient care, recently announced a new managed service offering LifeStream Services to streamline the telehealth delivery process between healthcare providers and patients. LifeStream Services will provide customizable support to address any infrastructure gaps for healthcare providers such as hospitals and health plans who want to provide a robust telehealth offering for their patients. The new managed services solution provides a complete end-to-end delivery system of telehealth products and services by leveraging Honeywells range of telehealth products and services, in addition to collaborating with accredited industryleading companies, such as Carenet Healthcare Services. "Working with companies such as Carenet, who will provide a unified communications platform between care providers and patients, gives Honeywell HomMed the opportunity to offer healthcare providers all the efficiencies associated with a full telehealth program, said Terry Duesterhoeft, president of Honeywell HomMed. We look forward to teaming up with other top companies in the future as we expand our LifeStream Services solution. LifeStream Services provides customized support to provide supplemental aspects of telehealth delivery, including flexible device acquisition options (available for rent, lease or purchase). For example, if a rural hospital wants to provide its Type 1 Diabetes patients with remote personal health devices to track patients biometric signs, but lacks the resources or staff to manage the devices or monitor the data, LifeStream Services can provide the solution. The LifeStream Services offering includes the following capabilities: patient device acquisition and maintenance; patient fulfillment; patient installation; and triage and monitoring services through its collaboration with Carenet. Patient device acquisition and maintenance: Patient device acquisition provides offsite storage and maintenance (such as repair and cleaning) for personal health devices, ensuring there is always a working supply of inventory available. Patient fulfillment: LifeStream Services will coordinate the delivery of the device to the patient, as well as the pick-up of the device upon completion of its use, if the patient is able to receive and set-up their own monitoring system with no outside assistance or minimal virtual assistance. Patient installation: If the patient requires more personalized assistance with the installation of their personal health device, whether that is in-home installation, monitor set-up or troubleshooting for monitoring device communications with the patients healthcare provider, LifeStream Services can provide the staff resources to accommodate the request.

It includes: - Continuity-of-care via dedicated care providers - Effective new-patient communication strategy - Monitoring the physical condition of at-home patients - Providing clinical follow up on abnormal results - Identifying patient problems early to improve patient self-management - Providing patients with a clinical resource for their healthcare questions or concerns
