USER GUIDE

Set out below is a guide on how to complete the prescribed forms located on our website in relation to the Football NSW Grievance and Disciplinary Regulations (Regulations). All the prescribed forms can be downloaded from our website which can be completed and signed electronically without the need to print. Each form must be sent to a designated email address, as set out below. Prescribed Form 02 Match Official Send Off Report This form must be used by all Match Officials in relation to send-offs. Match Officials are to send this form to matchreports@footballnsw.com.au within 2 days of the completion of a Match. Prescribed Form 03 Match Official Incident Report This form must be used by all Match Officials in relation to incidents which occur before, during or immediately after a match. Match Officials are to send this form to matchreports@footballnsw.com.au within 2 days of the completion of a Match. Prescribed Form 04 - Notification Form for Mistaken Identity in a Match Official Send-Off Report This form is to be completed by either a Participant (or by the Club if the Participant agrees) if he or she believes there has been a case of mistaken identity as a result of a send-off. The form is to be sent to dc@footballnsw.com.au within 24 hours of the completion of the Match. Prescribed Form 05 - Written Statement by Participant - Mistaken Identity in a Match Official Send-Off Report This form is to be completed by the Participant who has lodged Prescribed Form 04. The Participant is to name the person who actually committed the Offence. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of lodging Prescribed Form 04. Prescribed Form 06 - Written Statement by Actual Participant - Mistaken Identity in a Match Official Send-Off Report This form is to be completed by the Participant who actually committed the Offence and agrees for the Suspension to be transferred across. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of lodging Prescribed Form 04. Prescribed Form 07 - Written Statement by Club - Mistaken Identity in a Match Official Send-Off Report This form is to be completed by a club representative (as an alternative to the actual Participant lodging of Prescribed Form 06) who must state that the Participant who lodged Prescribed Form 04 was not the person who committed the Offence and must also state the Participant who actually committed the offence. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of lodging Prescribed Form 04. Prescribed Form 08 - Notice of Appeal of a Disciplinary Committee This form is to be completed by a Participant who wishes to appeal a decision of the Disciplinary Committee. The form is to be sent to tribunal@footballnsw.com.au within 5 working days of the decision being made by the Disciplinary Committee, together with the application fee. Prescribed Form 09 - Notification Form for Mistaken Identity in a Match Official Incident Report This form is to be completed by either a Participant (or by the Club if the Participant agrees) if he or she believes there has been a case of mistaken identity as a result of an incident. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of receiving a Match Official Incident Report.

Prescribed Form 10 - Written Statement by Participant - Mistaken Identity in a Match Official Incident Report This form is to be completed by the Participant who has lodged Prescribed Form 09. The Participant is to name the person who actually committed the Offence. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of sending Prescribed Form 09. Prescribed Form 11 - Written Statement by Actual Participant - Mistaken Identity in a Match Official Incident Report This form is to be completed by the Participant who actually committed the Offence and agrees to be issued with a Notice of Suspension. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of lodging Prescribed Form 09. Prescribed Form 12 - Written Statement by Club - Mistaken Identity in a Match Official Incident Report This form is to be completed by a club representative (as an alternative to the actual Participant lodging of Prescribed Form 11) who must state that the Participant who lodged Prescribed Form 09 was not the person who committed the Offence and must also state the Participant who actually committed the offence. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of lodging Prescribed Form 09. Prescribed Form 13 - Notification Form for Exceptional Circumstances This form is to be completed by either a Participant (or by the Club if the Participant agrees) if he or she believes there were exceptional circumstances that warrant the Suspension to be reviewed by the nd Disciplinary Committee. The form is to be sent to dc@footballnsw.com.au by 4pm on the 2 working day of the Club receiving the Notice of Suspension. Prescribed Form 14 - Written Statement by Participant - Exceptional Circumstances This form is to be completed by the Participant who has lodged Prescribed Form 13. The Participant is to set out the grounds that support his or her challenge. The form is to be sent to dc@footballnsw.com.au by 4pm the next working day of sending Prescribed Form 13. Prescribed Form 15 - Notice of Response This form is to be completed by a Member who has been issued with a Notice of Charge. The form provides an opportunity for a Member to plead guilty/not guilty, to submit any written statements and whether they wish to have representation at the tribunal. This form is to be sent to tribunal@footballnsw.com.au within the prescribed time set out in the Notice of Charge. Prescribed Form 16 - Notice of Appeal of a GPT This form is to be completed by a Member who wishes to appeal a decision of the General Purposes Tribunal. The form is to be sent to tribunal@footballnsw.com.au within 5 working days of the decision being made by the General Purposes Tribunal, together with the application fee. Prescribed Form 17 - Grievance Form This form is to be completed by a Member who wishes to raise a Grievance against another Member and has undertaken the prescribed process under the Regulations. This form is to be sent to tribunal@footballnsw.com.au within 14 days after the dispatch of the initial notice sent to a Member under section 8.3(c) of the Regulations, together with the application fee. Prescribed Form 18 - Notice of Appeal of a decision of an Association Appeals Committee This form is to be completed by a Member who wishes to appeal a decision of the Association Appeals Committee or body. The form is to be sent to tribunal@footballnsw.com.au within 5 working days of the decision being made by the Association Appeals Committee, together with the application fee. Prescribed Form 19 - Notification of Suspension Served This form is to be completed by Clubs when a Member has fully served his or Suspension.
