PROCTER AND GAMBLE, IBADAN PLANT SAFETY PLAN FOR DIVERSION OF PVC PIPE BACKGROUND The Project MAMBILLA Contractor (Vita Construction Limited) need to divert the PVC pipe at making area 2 WORK SUPERVISION AND DURATION: 1 The work is proposed to be done on 2nd February, 2013 2 Supervision will be done by Mambilla Project civil Engineer

WORK DESCRIPTION Barricade of the approved area with caution tape Set SGB scaffold to for access round the roof area Fix a bend to divert the pipe to the top of the roof and connect the pipe to the existing PVC pipe with a Tconnector Dismantle the scaffold Clean up the area WORK SUPERVISION AND DURATION The contractor (Vita Construction Limited) is familiar with the scope of the work Any change will be communicated within 24 hours ahead of implementation. Supervision will be done by the site Construction civil Engineer when the job is going on. There will be a dedicated P & G Safety Officer on site. There will be restrictive access for passage while performing the tasks except those that are working on the project. P & G first aider shall be notified of the operation

Logistics shall be provided by contractor in case of emergency to retainer clinic

ADDITIONAL CONSIDERATION 1. There will be a housekeeping programme to clear the site before and ongoing and after work is completed 2. The following basic PPEs must be used by the contractor while doing the job; safety shoes, helmet, hand gloves safety goggles etc 3. Valid fire extinguisher must be on ground 4. The area of work will be properly barricaded with caution tape 5. P & G approved disciplinary action/ matrix will be implemented on any personnel/contractor found violating safety rules 6. POSSIBLE CAUSES Failure of the employees to attend toolbox talk or understand the working order

MITIGATION MEASURES Permit (work order) must be secured, RA, Method Statement must be discussed and understood with/by all employees involved in the task. Questions will be asked to ensure that employee understood the method of task All personnel involve in the task must be well kitted with safety shoes, hard hat, reflective jacket and hand gloves

All the off cuts must be properly kept in skips at approved place 1. Deploy this risk analysis to workers before starting the job.
