Make a Video to Save a Childs Life Acute Child Global Initiative Video Contest $2,500 Award Www.facebook.

We are Acute Child Global Initiative (ACGI) and we are currently seeking videos that convey the global severity of the number two killer of children under the age of 5 - acute watery diarrhea (AWD). Annually, there are over 1.7 billion cases and over 760,000 deaths due to AWD, which account for 11% of child deaths globally. Our non-profit organization desires to raise public awareness on this issue and inspire key audiences (including individuals and communities) to give, so that we can effectively deal with the precipitating causes of AWD and save the lives of thousands, if not millions, of children. Check out the fact sheet at the end of this form to find information that may be included in your video. Getting Started Basic Information Follow these four steps to enter the ACGI Contest: 1: Get the Details A. Submitted videos must be 30 or 60 seconds long and suitable for TV (must be exactly 30 or exactly 60 seconds)

B. Videos will be judged by an expert panel on the basis of: 1. Creativity and Originality 2. Quality of Audience Appeal/Response 3. Technical Accuracy and Content 4. Video Leads to Epiphany or Wow Factor C. Keep it clean. No violence, profanity, sex, or direct attacks on individuals or organizations D. Your video must not infringe on any third party rights E. Videos previously produced for compensation or already created about AWD are not eligible F. By submitting a video to this contest, you grant ACGI an exclusive royaltyfree ownership including but not limited to copying, distributing, modifying, displaying and performing publicly and otherwise use, and any other use as determined by ACGI, and/or its affiliates, and associates that may authorize others to use the video for any educational and/or informational purpose in any media. Also please initial next at the bottom of each page to show your understanding. G. ACGI will allow all participants included in release forms to use the video in portfolios, resumes, and other use. Refer to 2D to understand the legal rights ACGI holds to the video. 2: Make your Video Your video should meet the following requirements: A. Your video must be submitted as a QuickTime/MOV, WMV, MPEG, AVI, and must be uploaded to the YouTube ACGI Acute Watery Diarrhea Contest group page B. The video file should not exceed 50MB C. Your video must be either 30 or 60 seconds D. You must have a video or photograph release form signed by each person appearing in the video, and his/her guardian if s/he is a minor. If your video is chosen, ACGI will need to have copies of these releases. ACGI will provide the winner a sample release form. E. The video should end with text that directs the viewers to our Website for a minimum of three seconds: www.AcuteCGI.org F. The Fact sheet at the bottom of this sheet are all taken from UNICEF, and they should receive credit at the end of the video, along with any other sources you may choose to use (preferably a 1 second credit that mentions your sources) G. Contestants must provide copies of the original video files and release forms to ACGI to receive the $2,500 award Videos should focus on the contest theme: individual, family, and community health, and the economic impact that result from effectively dealing with AWD, which show

steps that can be taken to raise awareness on the severity of the Global problem. Log into YouTube. If you dont have an account already, its easy and only takes a few minutes to set one up. The use of YouTube.com to accept contest entries does not imply an endorsement of the site or its parent company by the Acute Child Global Initiative. Users wishing to submit their entries directly may do so. B. Once you have set-up the account then post the video and make sure to change the settings so that the video is unlisted. After that, all you have to do is copy the link and email it to AcuteCGI@gmail.com. Make sure to mention your names and title in the email. C. Post your video by the June 21st deadline. 4: Make it Official Complete the Entry Form Your entry is not complete until you fill out the contest entry form. Your video will not be included in judging if you have not completed the entry form. The contest winner will be notified in July via e-mail and certified mail. All entry and release forms should be emailed in. Contest Rules A. You must be a U.S. citizen or legal resident to enter and win. B. Entrants must complete the contest and entry form and provide valid contact information. C. J. ACGI reserves the right to make your video available to the general public from its Web site and to distribute it to groups and any other organizations interested in showing it for educational and informational purposes, including, but not limited to, on Internet sites, at conferences and events, on television, electronic platforms and/or public/private group meetings or events, and any other media outlets. K. You must have a contest entry form filled out and signed by each person appearing in the video, and his/her guardian if s/he is a minor. If your video is chosen, ACGI will need to have copies of these releases. L. Entries must be received or postmarked by June 21, 2013. The winner will be notified via e-mail and announced on the contest Web site in July. M. Entrants must agree to these terms and conditions, or the agreement of a legal guardian must be obtained if the entrant is a minor. Privacy Policy Personal information collected from video submitters to the ACGI Video Contest will never be sold nor given to profit or non-profit corporations without your written consent. The information collected for this contest will only be used to contact video submitters in direct relation to the contest. The contest winners name will be announced publicly, after consultation with the winner, as a part of the ACGI Video Contest award recognition process. All personal information collected through the Contest Web site will be destroyed within 6 months of the conclusion of the ACGI Video Contest. Criteria Creativity & Originality Considerations Video concept and approach is unique. Entrant employs innovative methods to convey the message. Audio and visuals enhance, rather than distract from, the underlining message. Audio quality any dialogue can be easily understood, music and/or sound effects of adequate fidelity. There are 1.7 billion infectious diarrhea cases worldwide annually among children under the age of 5 Over 760,000 children die each year from diarrhea complications* (which is greater than the death toll for children with AIDS, malaria, and measles combined) o India alone has about 380 million cases annually (killing nearly a 220,600 children annually) Diarrhea affects nearly 20% of the pediatric population at any given time, which makes it a major public health problem Sources to be aware of as causes: travel history, sources of water, recent food intake, history of diarrheal episodes, dehydration, fever, hematochezia, nausea, vomiting, and abdominal pain. (Also blood in stool is very telling factor) Africa and Asia account for over half the cases of childhood diarrhea o East Asia & Pacific: 371 million o South Asia: 519 million o Africa: 805 million o Rest of the world: 278 million 11% of child deaths are caused by diarrhea, second only to Pneumonia 18% (Malaria, Measles, and Aids account for 10% combined) More than 80% of child deaths are due to diarrhea in Africa and South Asia Nearly three quarters of child deaths due to diarrhea occur in just 15 countries (India, Nigeria, Congo, Afghanistan, Ethiopia, Pakistan, Bangladesh, China, Uganda, Kenya, Niger, Burkina Faso, Tanzania, Mali, and Angola) Diarrhea: having loose or liquid stools at least three times per day, or more frequently than normal for an individual (World Health Organization) Acute cases can lead to significant fluid loss and dehydration, which may result in death or other severe consequences if fluids are not replaced at the first sign What causes diarrhea? o Gastrointestinal infections caused by wide range of pathogens, including bacteria, viruses and protozoa (most specifically Cryptospordium with children) What causes Acute cases? o Rotavirus, E.
