BEFORE ME, the undersigned Notary Public, duly commissioned and qualified in this State and Parish, personally came and appeared JANE DOE, petitioner in the above captioned petition, who, after being duly sworn did attest and testify as to the truth of all of the factual allegations contained in the petition, specifically as follows: Petitioner, JANE DOE, is domiciled in East Baton Rouge Parish. Defendant, JOHN DOE., is domiciled in the East Baton Rouge Parish. The parties are husband and wife, having married on January 14TH, 1993, in East Baton Rouge Parish, Louisiana. Their last matrimonial domicile was in Baton Rouge, State of Louisiana. The parties have not contracted a covenant marriage. No children were born out of the marriage of the parties and none were adopted. Petitioner and Defendant voluntarily have lived separate and apart continuously since April 4TH, 2006. The Parties have lived separate and apart continuously since the filing of the petition for divorce of August 16TH, 2011 a period of more than six (6) months to the date the petition for divorce was filed. There has been no reconciliation between Petitioner and the Defendant. Petitioner requests that a judgment of divorce be granted. Further Affiant Sayeth Naught.
