Joint Commission International Library of Measures

The Joint Commission International Library of Measures, a required resource for selecting measures to meet the JCI accreditation requirements, has been announced to the field. The Joint Commission International Accreditation Standards for Hospitals, 4th Edition (effective 1 January 2011) requires organizations to select at least five of the 11 required clinical measures from the Library of Measures. This requirement is intended to begin the process of standardizing a set of measures across all JCI accredited organizations. In 2012, or possibly later, the measures will be reported to JCI for creation of a comparative database to enhance learning and benchmarking. In addition, JCIs Clinical Care Program Certification requirements include the need for programs to choose at least two of their four reported measures from the Library of Measures. Measure Description

Acute Myocardial Infarction (AMI) I-AMI-1 Aspirin received within 24 hours of arrival to the hospital for patients having an acute myocardial infarction (AMI). Aspirin prescribed at discharge for patients who had an acute myocardial infarction. ACEI (angiotensin converting enzyme inhibitor) or ARB (angiotensin receptor blocker) for patients who have LVSD (Left Ventricular Systolic Dysfunction) after having an acute myocardial infarction.

Adult smoking cessation advice/counseling given to patients who had an acute myocardial infarction. Beta-blocker prescribed at discharge for patients who had an acute myocardial infarction.

Heart Failure (HF) Heart failure patients with documentation in the hospital record that left ventricular systolic (LVS) function was evaluated before arrival, during hospitalization, or is planned for after discharge ACEI (angiotensin converting enzyme inhibitor) or ARB (angiotensin receptor blocker) for heart failure patients who have LVSD (Left Ventricular Systolic Dysfunction) Adult smoking cessation advice/counseling given to heart failure patients

Hospital-Based Inpatient Psychiatric Service (HBIPS) I-HBIPS-2 Psychiatric patients who were placed in physical restraints during their inpatient hospitalization. This measure will determine the total number of hours that patients

were maintained in physical restraints for those admitted to a hospital-based inpatient psychiatric setting Psychiatric patients who were placed in seclusion during their inpatient hospitalization. This measure will determine the total number of hours that all patients were maintained in seclusion for those admitted to a hospital-based inpatient psychiatric setting.

Nursing-Sensitive Care (NSC) Patients that have hospital-acquired (nosocomial) pressure ulcer(s) (category/stage II) on the day of the prevalence study. Note: Please see Appendix E for details on how to collect this measure. All documented falls with or without injury, experienced by patients in a calendar month. All documented falls by a patient with an injury level of minor (2) or greater.
