27, 2013 Event Location: Sunset Park, 2601 E Sunset Rd, Las Vegas, NV 89120 Participants Name: _______________________________________________ Participants Address: _____________________________________________ Participants DOB: ___/____/______ Contact #: _______________________ This agreement is between the participant (noted above) and the LAS VEGAS ZOMBIE RUN (herein referred to as the EVENT) in addition to the event producer, Generation Vegas. (herein referred to as GV) and The Sunset Park and its affiliated entities (herein referred to as SP). 1. I understand and agree that I am at least 18 years of age prior or on the day of the EVENT. I understand that I must present a valid government issued photo ID at the time I check in at registration for the EVENT. If I am under the age of 18 on the day of the EVENT, I understand that I must have the signature of a parent or legal guardian in order to be permitted to participate in the EVENT. There will be no exceptions to this. 2. I understand the nature of the EVENT and I am aware that my participation in the EVENT will be of a physical nature wherein I will be running, jumping, crawling, climbing and engaging in other physical activities as part of the EVENT. I agree that I am in good physical and mental health with no pre-existing medical conditions that would prevent me from participating in the EVENT and all of the aspects therein. 3. I understand that as a Zombie Participant, I will be chasing Runner participants throughout the course of the EVENT. I understand that I am only permitted to attempt to take the flags from the Runner participants as part of the entertainment and strategy of the EVENT (this is a similar game strategy as flag football). I understand that the Runner participants will abide by a strict set of guidelines and rules that ensure my safety during participation in the EVENT. 4. I understand and agree that I will not touch, hit, strike, or engage in any physical conduct with other Runner participant or zombie performer during my participation in the EVENT. I understand that any violation of the rules for the

EVENT will be considered a breach of this agreement and can result in my disqualification and potential ejection from the EVENT, with NO REFUND. 5. I understand and agree to follow all of the posted and noted rules for participation in the EVENT. 6. I agree to follow any instructions given to me by EVENT staff and personnel. 7. I understand that the EVENT or GV or SP will not cover any medical expenses resulting in injury incurred as part of my participation in the EVENT. I understand that there is no medical insurance coverage as part of my ticket purchase for the EVENT. 8. I agree that I will conduct myself in a careful and respectful manner at all times during the EVENT and will not injure or damage any other person or property. 9. I understand and agree that if I arrive at the EVENT under the influence of alcohol or any kind of drugs or narcotics, that I will not be admitted into the EVENT and will receive no refund as a result. 10. I understand that if I am injured or have any other type of loss or damage (including theft of personal property), I will not make any claim against the GV or SP. I will also indemnify (reimburse) GV and/or SP if any other person makes a claim against it for anything I did or am responsible for.

11. I hereby agree and authorize GV to record my voice, photograph and record (on film, tape, photographs or otherwise), my participation in the EVENT. I understand and agree that GV has without limitation, the right to use my likeness, voice and name in all forms of media, worldwide, in perpetuity with regards to the marketing, promotion, merchandise and advertising of the EVENT. I understand that there will be no compensation for said use. I further acknowledge that GV owns all rights, in perpetuity, to the results and proceeds of my services rendered in connection herewith. 12. I understand that as a Zombie Performer I will be reporting to the EVENT, where I will check in with GV management staff no less than 90 minutes prior to my shift start time. In the 90 minute process prior to my shift I will be instructed in regards to what is expected of me for my participation in the EVENT as well as getting any makeup and wardrobe work completed on me before the start of my shift.

13. I understand and agree that as a Zombie Participant I will be placed in a designated area on the EVENT course by GV staff. While in my designated area, I agree to follow all directions, instructions and guidelines given to me by GV staff. Furthermore, I acknowledge that any deviation from these directions, on my part, may result in the termination of my being a Zombie Participant. 14. I understand and agree that my being permitted to participate in the EVENT is adequate consideration for the releases, waivers, and indemnities given herein.

15. I, the parent or legal guardian of the above named participant (MINOR), give my approval for MINORs participation in the EVENT. I assume all risks and hazards incidental to the MINORs participation in the EVENT and I release, waive discharge, absolve, indemnify, agree to hold harmless and covenant not to sue the Releases for any claim arising out of an injury to the MINOR and from any and all claims, causes of action, obligations, lawsuits, charges, complaints, controversies, damages, costs or expenses of any kind, nature, or description, whether direct or indirect, in law or in equity, in contract or in tort, or otherwise, whether known or unknown, arising out of or connected with MINORs attendance at and participation in the EVENT, whether or not caused by the Releases negligence or gross negligence. I consent to the foregoing, grant permission for MINOR to participate in the EVENT and acknowledge that I have carefully analyzed, accepted and agreed to the terms of this Agreement and Release Waiver, and that I know and understand its contents and sign below on my own free act and deed. 16. I hereby on behalf of myself, my heirs, executors and administrators waive and release any and all rights and claims against the following entities and persons, and agree to indemnify and hold harmless the following entities and persons: Generation Vegas, and any other entity involved with the EVENT, their officers, directors, agents, employees and event volunteers, with respect to any and all liability of any nature in any way arising out of the EVENT and my participation therein, including not by means of limitation, the condition of the premises in or around the EVENT course. The foregoing release of liability includes all liability with respect to my travel to or from the EVENT.
