Col Adm OTA Gaya (Through proper channel) APPLICATION FOR PREMATURE RETIREMENT ON EXTREME COMPASSIONATE GROUNDS Sir, 1 2 3 With due respect and humble submission, I beg to state the following few lines for your kind consideration and favourable action please. I, the above named soldier was enrolled in the Army Medical Corps on 31 Aug 1995 and completed 16 yrs and 10 months of service. Presently I am serving with this est since 07 Sep 2011. My family consist of My father (74 yrs), mother (65 yrs), wife, and daughter. My father Shri Dattaram S Sawant is suffering from PIVD (Prolapsed inter vertable disc). He is unable to stand/walk, his right leg is restricted, and he is totally bed ridden patient (Xerox copy of MRI report attached). I am the only son of my elderly parents and my wife is the only person looking after my parents at home. Due to stress and strain she has been diagnosed as Primary Hypertension and Type II Diabetes Mellitus (medical documents attached). Now she is unable to take care of my parents. My wife has to shoulder all the domestic responsibilities inspite of her ill health. In this scenario my only child is being neglected too. There is miserable condition at my home. Thus, how can I perform my duties, leaving my family in miserable condition? In view of the above at this juncture only my retirement from service can help me to look after my family. Hence, it is my humble request to you, to sympathetically consider my application for premature retirement from service on extreme compassionate ground. I shall remain grateful to you for this act of kindness Thanking you in anticipation
