2.1 It is the policy of this facility to plan for, support and coordinate, a systematic approach to patient and family education throughout the facility. It is our belief that educating the patient and his/her family improves healthcare outcomes by enabling the patient or family to: 2.1.1 Increase understanding of the patient's health status; 2.1.2 Increase compliance with the healthcare plan; 2.1.3 More actively participate in the decision making process concerning healthcare options; 2.1.4 Increase the family care skills and coping mechanisms along the continuum of care; 2.1.5 Promote an overall healthy patient lifestyle; 2.1.6 Increase understanding of financial implications for treatment and other healthcare choices. 3. SCOPE 3.1 Planning on how to educate the patient at Gulf Diagnostic Center Hospital. 4. RESPONSIBILITY 4.1 Physician 4.2 Registered nurse 5. PROCEDURE 5.1 All patients shall be assessed for learning needs at time of admission to the facility and reassessed throughout their stay with subsequent follow-up as appropriate. Each discipline providing care to the patient shall participate in the education of the patient. 5.2 Interdisciplinary Patient/Family Educational Record shall be used by all disciplines a s a means to document teaching provided to the patient and family. The Educational Record shall be completed as follows: 5.2 1 Date: The date that teaching occurred. 5.2.2 Learning Factors (culture, religion, readiness, barriers): These factors should ` be assessed and considered at the time of teaching. Documentation is required if one of these factors will interfere with the learning process, or if it is felt to enhance in terms of learning preference, the educational experience. The term

"barriers" includes, but is not limited to: emotional barriers, desire and motivation to learn, physical or cognitive limitations, financial implications for healthcare choices, family/patient beliefs and values, literacy and language and/or communication barriers. 5.2.3 Identified Learning Need: The learning need shall be reassessed prior to each teaching/educational session. If a learning need is not met at the initial teaching session, it shall be identified and further teaching provided. 5.2.4 Patient/Family Involved in Learning: Teaching should include the family or significant other whenever possible. The terms family and significant other shall be considered synonymous and defined as anyone who plays a significant role in the patient's life. 5.2.5 Evaluation of Learning: All learning shall be evaluated for effectiveness of teaching. Those involved in the learning process shall either demonstrate learning or verbalize understanding. If they are unable to show that learning took place, then future teaching is required. 5.2.6 Comment: Include any additional information that the individual providing the teaching may find beneficial in this area. Resource materials provided can be included here as well. 5.2.7 Initials: The person providing the teaching should place his/her initials in this section, the initials should correspond with the name and initials located at the bottom of the record. 5.2.8 Department: The discipline providing the education should complete this section of the record stating the name of the department represented. More than one discipline may provide teaching on any one subject. 5.3 Evaluation of organizational patient education programs 5.3.1 Periodic evaluations of the facilitys patient education programs shall be performed to determine the effectiveness of the facilitys efforts toward educating the patient population. Evaluations are felt to be most beneficial when they are performed with an interdisciplinary focus to allow for a comprehensive assessment of how well the facility as a whole educates patients. The goal of the evaluations is to determine if the patient and/or family benefits from the education. Evaluations are performed using the following methods: 5.3.1.1 Review of multidisciplinary patient satisfaction survey which includes educational component;

5.3.1.2 Review of diagnosis-specific organizational education with emphasis on unplanned readmissions to the facility with the same or related diagnosis; 5.3.1.3 Review of patient education plans to determine if the majority of goals have been attained prior to discharge. 5.3.2 Evaluations are performed utilizing patient satisfaction survey results on a quarterly basis, or more frequently if requested by any member of the facility, who demonstrates appropriate reason. 5.3.3 Diagnosis-specific patient education evaluations are performed as requested basis (medical staff or facility Patient Education Team request), but not less than semiannually. 5.3.4 Random review of patient education plans will be performed on a quarterly basis by the facility Patient Education Team or their designee.
