PSYCHOSOCIAL REHABILITATION CLINICAL REFLECTIVE JOURNAL

Monday Aug 5th 2013 Stat. Holiday Tuesday Aug 6th 2013 Today was a little quieter I sat in on the glucose checks again this morning and ensured that the inmates were administering their correct medication and dosage after their glucose levels were checked. I feel like I am starting to develop a routine around the unit right now in that I am getting to know my bearings a little better. For example I am more comfortable handling basic requests from inmates such as handling compliant/request forms and answering questions pertaining to clinic schedules (1.05). While these may seem insignificant I recognize them as advancements towards being fully independent as a student nurse for my perceptorship (1.03). Throughout the course of the morning a minor altercation had taken place between two inmates over laundry which was being monitored by the corrections officers. Later in the morning one of the inmates with the history of self harm approached me at the console to tell me that he was planning to self harm himself and he wished to speak with either his RPN or his psychologist. I informed the client that I would contact his RPN and in the mean time I attempted to talk to the client along with my preceptor (2.06), (6.06), (7.07). However, the client informed us both that he did not wish to speak with anyone other than his psychologist or RPN. When his RPN arrived he talked with the client and helped him to understand that his thoughts were coming as a result of a silly confrontation (8.08). The client was administered 100mg of Serequel PRN and monitored for the rest of the day. In the afternoon I booked an appointment to meet with the

PSYCHOSOCIAL REHABILITATION CLINICAL REFLECTIVE JOURNAL Social Worker regarding a client which I wish to use for my clinical assignment (2.01), (2.06), (5.11). She agreed to meet with me Thursday August 8th, 2013 at 10 am. Wednesday Aug 7th 2013 I spent most of today sitting in treatment team meetings (2.04), (2.05), where the progress notes of 6 inmates were reviewed against their rehabilitation goals. Each meeting is attended by a multifunctional team consisting of a RPN, Parole Officer, Psychologist and Social Worker (2.01), (2.02). I found it interesting to note the differences between the behaviors of each of the

clients (1.03), (1.04). Some were very distant within the meeting with no clear desire to be there even though they appreciated the importance of the meeting. One client in particular was very interested in getting parole which was due to come up in 2014 yet he saw no value in participating in programs such as AA or Anger Management which are preconditions to his parole acceptance. I noted at the time that the clients affect was blunted throughout and was very deflective when discussing responsibility for his crime (1.03), (1.04). He preferred instead to blame his victim rather than his anger. His use of language was also impersonal referring to his victim as that person (1.03), (1.04). In the afternoon, I continued to participate in treatment team meetings focusing on clients with cognitive impairments. What I am really enjoying about this clinical rotation is the way that I am made to feel a part of the team. During todays treatment team meetings for example I was asked for my opinion on several occasions, which helped me to ask questions and verbalize my own observations which I really appreciated (2.06), (4.01), (4.04). PSYCHOSOCIAL REHABILITATION CLINICAL REFLECTIVE JOURNAL I had a pretty quiet day today. I spent the morning supervising glucometer checks and I am pretty happy that I am competent at that skill. Earlier in the week, I had booked some time with Diana, the Social Worker for the client I wish to use for my case study (2.06) (5.11). Diana was very helpful and answered any questions that I had regarding the placement of a client being released on parole. We discussed the various different placement options available within the Lower Mainland and the Fraser Valley. We also discussed the various types of services offered

at each facility (4.04), (1.02). In the afternoon I helped the clients with some basic requests such as the correct completion of documentation, physicians schedules and chart client time tables. Overall today was a very quiet day with not a lot of activity, which I did not enjoy as much as other days as I prefer to be busy.
