Alzheimer Idea Principal El Alzheimer es una enfermedad neurodegenerativa que se manifiesta como deterioro cognitivo y trastornos conductuales.

Se caracteriza en su forma tpica por una prdida de la memoria inmediata y de otras capacidades mentales, a medida que las clulas nerviosas (neuronas) mueren y diferentes zonas del cerebro se atrofian. La enfermedad suele tener una duracin media aproximada despus del diagnstico de 10 aos, aunque esto puede variar en proporcin directa con la severidad de la enfermedad al momento del diagnstico. La enfermedad de Alzheimer es la forma ms comn de demencia, es incurable y terminal, que aparece con mayor frecuencia en personas mayores de 65 aos de edad. La causa de esta enfermedad es por el momento desconocida. Existen tres grandes grupos de teoras etiolgicas: la gentica, la infecciosa y la txica, y por otra parte, una serie de teoras patognicas que explican la involucin cerebral observada en la enfermedad de Alzheimer. Hiptesis gentica. Es la ms estudiada en la actualidad, basada en el hecho de que se han constatado varios casos de transmisin en el seno de un cierto nmero de familias. Se han hecho estudios que han demostrado que los enfermos con Sndrome de Down que logran llegar a una cierta edad adulta pueden desarrollar la enfermedad de Alzheimer. Como el Sndrome de Down est asociado a una anomala gentica localizada en el cromosoma 21, se sugiri que dicho cromosoma podra ser el asiento de los genes anormales capaces de originar las alteraciones neuronales observadas en la enfermedad de Alzheimer. Las investigaciones ms recientes han encontrado otras alteraciones familiares en los cromosomas 1, 4 y 19, as como en el DNA mitocondrial. En la actualidad se piensa que la persona ser sensible a la enfermedad por una predisposicin gentica, pero sern necesarios otros factores para desencadenarla. Hiptesis infecciosa Se basa en el descubrimiento de enfermedades vrales, producidos por virus lentos que se incuban, a veces, durante gran parte de la vida, o bien, a la existencia de unas protenas especiales (priones) que tienen la capacidad de replicarse y que son txicas para el sistema nervioso, provocando su destruccin. Esto nos hace pensar que estos enfermos presenten alguna deficiencia inmunolgica o algn otro factor que les haga sensibles a un virus lento, pero esto an no se ha podido demostrar. Hiptesis txica En las lesiones cerebrales de la enfermedad de Alzheimer se ha encontrado ciertas cantidades de aluminio y de silicio, pero no se sabe si estas lesiones son producidas por el aluminio y el silicio, o si las lesiones muestran afinidad por estas sustancias.

Se han hecho y se siguen realizando estudios para demostrar la influencia real de la ingestin de aluminio (por uso de utensilios de cocina, de ciertas aguas de bebida, de lluvias cidas...), pero an no se ha podido justificar estas acumulaciones de aluminio en el cerebro. Esta enfermedad tiene una duracin variable, de dos a diez aos. En este tiempo, el enfermo va a pasar por diferentes estadios: Estadio I (Se compara con la edad mental de un nio de 8 a 12 aos). Memoria: olvida sus citas, el nombre de las personas (familia, amigos,...), objetos familiares, le cuesta seguir una conversacin, se equivoca en sus cuentas. Comportamiento: bruscos cambios de humor (monta en clera cuando se da cuenta que pierde el control de la situacin, que olvida los objetos (llaves,...). Sale menos y no quiere ver a sus amigos. Lenguaje: contina comunicndose pero tiene problemas para encontrar palabras precisas, usa frases ms cortas, mezcla ideas que no tienen relacin directa entre s. Coordinacin de gestos y movimientos: en esta fase an est bien, no se pierde, puede conducir, se viste solo, come bien, etc... Actividades diarias: sigue trabajando, ya que todava no est afectado ms que por prdida de memoria. Estadio II (Se comporta con la edad mental de un nio de 4 a 7 aos). Memoria: olvida los sucesos recientes (lo que acaba de comer hace 5 minutos) Le cuesta comprender hechos recientes (bodas, muerte de un familiar). Persiste, sin embargo, el recuerdo de hechos lejanos, aunque los recuerda mal en el tiempo (pide informacin sobre su madre muerta, pregunta por personas que hace aos que no ve)... Comportamiento: reacciones violentas y desproporcionadas respecto al motivo que las ha encadenado (le acusa de haber roto cosas que l ha perdido, se vuelve agresivo si se insiste en que se bae). Tiene miedos injustificados: cualquier ruido le asusta, una cortina que se mueve, una luz, etc... Camina de un lado para otro. Lenguaje: habla menos, su vocabulario se empobrece, repite las mismas frases durante horas. Responde lentamente, no acaba las frases. Coordinacin de gestos: pierde el equilibrio, se golpea fcilmente, las cadas son frecuentes. Enlentece los movimientos, necesita ayuda para ir al bao o a la habitacin. Pueden aparecer temblores, convulsiones, contracturas musculares, etc... Actividades: le es difcil hacer actividades cotidianas como elegir su ropa; se visten sin importarle cmo, sin pensar en la moda o en la estacin del ao. Le cuesta saber qu elementos usan en la mesa (tenedor, cuchillo, ...), qu orden seguir en la ducha o en el bao (si secarse primero, o enjabonarse, o

desvestirse). Pierde su autonoma, ya no puede viajar slo, ni conducir. Se pierde incluso en trayectos conocidos. Debe ser vigilado las 24 horas porque puede hacer actividades peligrosas para l y para los dems, como abrir la llave del gas sin encenderlo, olvidar un cigarrillo encendido, etc... Estadio III (Se comporta con la edad mental de un nio de 3 aos) Memoria: no reconoce a nadie, olvida hechos recientes y pasados. Lo nico que conserva es la memoria emocional, conoce a la persona que le cuida, le ayuda y le quiere, y esto es algo que tiene que estar siempre presente en la mente de quien se ocupa de l. Comportamiento: llora, grita, se agita. No reacciona coherentemente ante una situacin, no comprende una explicacin. Lenguaje: repite palabras sin pies ni cabeza, solo usa correctamente algunas palabras correctas. No entiende lo que se le dice. Coordinacin de gestos: no controla sus gestos, no sabe levantarse, sentarse o andar, le cuesta tragar, no controla esfnteres y aparece incontinencia. Actividades cotidianas: han desaparecido totalmente, suele permanecer en cama, con posibles riesgos de lceras por presin, infecciones respiratorias, etc. Existen, segn el paciente, diferentes evoluciones, unas ms rpidas y otras ms lentas, pudindose mezclar los signos de los diferentes estadios. Especificar la incidencia social, familiar y personal del paciente de Alzheimer Palabras claves Alzheimer, demencia, neurodegeneracion, deterioro cognitivo Justificacin Basando en las estadsticas actuales sobre demencias, podemos afirmar que el Alzheimer, englobada en la demencia degenerativa primaria, tiene una repercusin del 50%, es decir, la mitad de todos los casos de demencias existentes actualmente. No pudiendo, actualmente, los tratamientos mdicos combatir eficazmente este problema, toman un papel muy importante los cuidados, los cuales son realizados por el personal de enfermera, utilizando todos los medios a su alcance y ensendolos a la familia y cuidadores para que los realicen, poniendo en prctica todos los conocimientos sobre la enfermedad, para facilitar la vida al paciente y a los que lo rodean (familiares y/o cuidadores). Una vez expuesto el importante papel del personal de enfermera en esta patologa, lo fundamentamos en el espectacular aumento de esta enfermedad, que de seguir aumentando al ritmo observado en la documentacin estudiada, para el ao 2000 en Espaa, esta enfermedad ser padecida por una poblacin de ancianos de 500.000 personas, y en Europa por 8.000.000 de personas.

No existe un tratamiento curativo para esta enfermedad. El tratamiento con medicamentos esta orientado a tratar algunos sntomas del paciente (ansiedad, agitacin, insomnio, depresin) con el fin de reducir las molestias provocadas por la enfermedad.
