Room
Your room assignment at CMC-NorthEast is based on your admitting diagnosis and the bed availability on the day of your admission. Rooms are private (single bed). If your room temperature is not comfortable, please notify your nurse.

During the Night


Please stay in bed after you have been prepared for the night. Unfamiliar surroundings and sleeping medications may create a hazard if you get out of bed. For assistance during the night, use your call button.

Hospital Bed
Hospital beds are electrically operated; your nurse will show you how to work your bed properly. Bedside rails are for your protection. They may be raised at night or during the day if youre resting, recovering from surgery or taking certain medications.

Leaving the Unit


If you want to leave the unit, please check first at the nurses station to make sure your doctor has given approval and to let the staff know where you can be reached. It is important to stay in your room until your doctor has made rounds and treatments have been completed.

Calling Your Nurse


A button to call your nurse is located at your bedside. When you press the button, the nurses station is alerted that you need assistance, and a staff member will respond to your signal as soon as possible.

Television
Color television sets are provided (except in Intensive Care) free of charge in each room. To watch your television, press the ON button located on your bedside railing and use the CHANNEL button to change channels. For FM radio, press the ON button again and use CHANNEL to search for stations. Use the VOLUME dial to adjust loudness. Press the OFF button to turn off the power to your television. If you need help with your television or find that it isnt working, call your nurse. Please be considerate of other patients by playing TV sets softly and by turning off your set at bedtime. For complete programming information, please refer to TV Guide Channel 10. Patient Education is Channel 15 and the Newborn Channels are 19 and 20 (English and Spanish, respectively).

To make a long-distance call, you must use a calling card, place the call collect or bill the call to a third party. To reach a patients room from outside the Medical Center, call Patient Information at 704-403-1595. If you have problems placing a call, please contact the Medical Center operator at 0 for assistance. Public telephones are located in the waiting area on each floor and in the Emergency and Outpatient areas of the Medical Center. Phones are also located in the lobbies of the Mariam Cannon Hayes Family Center and the Surgery Center. Cell phone use is restricted in some areas. Check with your nurse or a hospital staff member regarding non-restricted areas for cell phone use.

Televisin
En cada habitacin hay televisores a colores (excepto enTerapia intensiva), los cuales no tienen cargo adicional. Para ver televisin, oprima el botn de encendido ("ON") ubicado en la baranda lateral de la cama. Para cambiar de canal, utilice el botn "CHANNEL". Para escuchar la radio FM, oprima nuevamente el botn de encendido "ON" y utilice el botn "CHANNEL" para sintonizar las estaciones. El dial de "VOLUME" permite ajustar el volumen. Oprima el botn de apagado ("OFF") para apagar el televisor. Si necesita ayuda con el televisor o si no funciona, llame al personal de enfermera. Le rogamos tener consideracin por los dems pacientes y escuchar la televisin a bajo volumen y apagarla a la hora de acostarse. ood nutrition plays an important part in your general wellbeing and recovery. Our goal is to provide you with quality foods, expertly prepared and attractively served. Menu options are available through the nutritional care representative who delivers your meal tray. Meal choices will be prepared to your specifications. If you have any food allergies or food preferences, please notify your nurse or nutritional care representative immediately.

Food is available for take-out or cafeteria dining. Caf 920 Express, located on the fourth floor of the Clinical Services Building, is open from 6 a.m. to 6 p.m., Monday Friday. Take-out food is not allowed in the Cardiac or Intensive Care units. For an additional charge, visitors may arrange to have a guest tray delivered with the patient meal. Requests for guest trays should be made through the nursing staff.

Cafeteria
Caf 920 is located on the first floor of the Medical Arts Building. Meals are served each day as follows: Breakfast 610 a.m. (Grab & Go items available from 1011 a.m.) Lunch 11 a.m.2 p.m. (All stations except Saut and Homestyle, available from 24 p.m.) Dinner 47 p.m. Late Night 7 p.m.2:30 a.m. Coffee Bar 6:30 a.m.5:30 p.m., Monday Friday

Vending Machines
Snack and beverage vending machines are located near the volunteer station at the gold elevators and on the first floor of the Mariam Cannon Hayes Family Center.

FOOD AND NUTRITION SERVICES

The Medical Staff


The physician who admits you or the hospitalist on staff is responsible for directing your care while you are in the hospital. Your physician or hospitalist, as the coordinator for your treatment program, should be consulted if you have questions about your illness.

Dietitians
The hospital maintains a staff of registered dietitians to meet your dietary needs during your stay. If you have questions about your meals or diet, ask your nurse to have a dietitian visit you.

The Nursing Staff


Twenty-four hour nursing care is provided by a team of professional registered nurses, licensed practical nurses and care partners. A nurse manager is responsible for directing and coordinating nursing care on each unit. Please feel free to contact your nurse or the nurse manager if you have questions or concerns.

Social Workers/Case Management


The Case Management department consists of registered nurses and social workers trained to assist patients and families through the continuum of care including wellness, prevention, acute inpatient and post-admission or discharge. Our staff is knowledgeable about community agencies and services that may assist you after discharge. These services include home health agencies, durable medical equipment, extended care facilities, rehabilitation centers, counseling services, mental health and substance abuse clinics. We also communicate with your insurance companies, Medicare and Medicaid regarding treatment plans and certification of days. You may contact the Case Management department at x31597 from 8 a.m. to 4:30 p.m., Monday through Friday. After hours or on weekends, the Medical Center operator can page the on-call staff.

Pastoral Services
CMC-NorthEast believes that total patient care demands attention to the spiritual, as well as the physical and emotional needs of patients, families and staff. Members of the pastoral services staff are available 24-hours a day, seven days a week to assist you, whatever your religious tradition. Your priest, minister or rabbi is always welcome to visit you while you are here. You may contact one of the chaplains by dialing x31443 between 8 a.m. and 4:30 p.m., Monday through Friday. After office hours, chaplains are available for emergency situations by calling your nurse or the Medical Center operator.

Environmental Services
Your room is cleaned daily by a member of the housekeeping staff. If there is a housekeeping problem in your room, please tell your nurse.

Volunteers
Volunteers contribute many hours of service by supplementing the services of the hospital staff. They can be identified by their teal uniforms and khaki pants or skirts. If you are interested in becoming a volunteer, you may obtain an application by calling x31376.

Other Personnel
During your stay, you may be visited by many other healthcare professionals, including personnel from the laboratory, radiology and physical or occupational therapists. In addition, the CMC-NorthEast family includes many behind-the-scenes workers, such as secretaries, food service workers and others who contribute greatly toward your well-being. Personal mdico
El mdico que se encargue de admitirlo o el hospitalista de guardia es el responsable de dirigir su atencin mdica mientras se encuentre en el hospital. Dietistas

Personal de enfermera
El personal de enfermera, integrado por enfermeros registrados profesionales, enfermeros prcticos matriculados y asistentes de atencin son quienes brindan atencin de enfermera las 24 horas. El gerente de enfermera es responsable de dirigir y coordinar la atencin de enfermera de cada unidad. Si tiene preguntas o inquietudes, comunquese con el personal o el gerente de enfermera.

Trabajadores sociales/Administracin de casos


El departamento de Administracin de Casos est integrado por enfermeros registrados y trabajadores sociales capacitados para ayudar a los pacientes y sus familias brindndoles asistencia continua en materia de bienestar, prevencin, hospitalizacin de pacientes agudos e incluso despus de la admisin o el alta hospitalaria. Nuestro personal tiene informacin sobre las agencias comunitarias y los servicios que pueden ayudarlo despus del alta hospitalaria. Estos servicios incluyen las agencias de salud hogarea, equipos mdicos permanentes, instalaciones de atencin prolongada, centros de rehabilitacin, servicios de asesoramiento, clnicas de salud mental y para el tratamiento de la dependencia de sustancias. Tambin estamos en contacto con su compaa de seguro, con Medicare y Medicaid, para resolver los planes de tratamiento y la certificacin de das. El plantel de servicios pastorales est disponible las 24 horas del da, los siete das de la semana, para ayudarlo, cualquiera sea su tradicin religiosa. Su sacerdote, su ministro o su rabino siempre son bienvenidos si desean visitarlo mientras se encuentra aqu. s a natural outgrowth of our organizational values and mission, the Board of Directors, medical staff and employees of CMC-NorthEast jointly affirm and recognize the following rights and responsibilities of patients. You or your representative should be given this information at the earliest possible time. You have the right to good quality care and high professional standards that are continually maintained and reviewed. You have the right to expect emergency procedures to be implemented without unnecessary delay. The Medical Center will care for you to the best of our ability. If you require treatment that we cannot deliver, we will help you find another place that can provide the special care you need. When medically permissible, you may be transferred to another facility only after you or your next of kin or other legally responsible representative has received complete information and an explanation concerning the need for, and alternatives to, such transfer. We will not transfer you until another medical facility and a physician agree to accept you. If you are discharged from the Medical Center and need follow-up care, you will be given information to assist in making a decision about that care. You have the right to assistance in obtaining consultation with another physician at your request and expense. You cannot be denied the right of access to an individual or agency that is authorized to act on your behalf to assert or protect the rights as listed within this document. You have the right to designate visitors who shall receive the same visitation privileges as your immediate family members, regardless of whether the visitors are legally related to you by blood or marriage.

Access

You have the right to medical and nursing services and to be treated with respect and receive competent care no matter your race, color, sex, religion, age, sexual preference, beliefs, country of birth, source of payment or illness. You have the right not to be awakened by hospital staff unless it is medically necessary. You have the right to be free from needless duplication of medical and nursing procedures. You have the right to medical and nursing treatment that avoids unnecessary physical and mental discomfort. You have the right to be free from seclusion and restraints of any form that are not medically necessary. You have the right to make any reasonable request.

Communication
You have the right to full information, in laymens terms, concerning your diagnosis, treatment and prognosis, including information about benefits, alternative treatments, risks and possible complications. When it is not possible or medically advisable to give such information to you, the information shall be provided on your behalf to your designee. If you have a hearing or special problem, or speak another language, we will try to help you understand your care. If you do not speak English, you shall have access, when possible, to an interpreter. Except for emergencies, the physician must obtain the necessary informed consent prior to the start of any procedure or treatment, or both. You have the right to refuse, and a physician shall inform you of your right to refuse, any drugs, treatment or procedure within legal limits. If you refuse, we will inform you of the possible medical risks of the refusal. You have the right to have a family member or representative and your own physician notified promptly of your admission to the Medical Center.

Si est mdicamente permitido, usted puede ser transferido a otra institucin slo una vez que usted, un familiar u otro representante legalmente responsable hayan recibido informacin completa y una explicacin sobre la necesidad de dicho traslado y las alternativas. No lo trasladaremos hasta que otra institucin mdica y un mdico se comprometan a aceptarlo. Comunicacin
Tiene derecho a recibir informacin completa, en trminos sencillos, acerca de su diagnstico, tratamiento y pronstico, como tambin informacin sobre beneficios, tratamientos alternativos, riesgos y posibles complicaciones. Si no fuera posible o mdicamente recomendable brindarle dicha informacin, la informacin se le dar a la persona que usted designe. Si tiene problemas auditivos o especiales, o si habla otro idioma, trataremos de ayudarlo para que comprenda su atencin. Si no habla ingls, en la medida de lo posible tendr acceso a un intrprete. Excepto en situaciones de urgencia, el mdico debe obtener su consentimiento informado necesario antes de comenzar cualquier procedimiento, tratamiento o ambos. You have the right to ask questions regarding information contained in your medical records and receive a response in a timely manner. This facility shall provide you or your designee, upon request and receipt of a legally valid authorization, access to all information contained in your medical records. Your attending physician may restrict your access to medical records. If your physician restricts your access to information in your medical records, he or she shall record the reasons in your medical records. Access shall be restricted only for a sound medical reason. You have the right to be given information about your medical problem at the earliest possible time in the course of your hospitalization. You should be told how the problem will be treated, if treatment will hurt and how long it will take. If requested, you should also be told the approximate cost of your treatment. You have the right to examine and receive a detailed explanation of your bill. You have the right to full information and counseling on the availability of known financial resources for your healthcare. You have the right to know what facility rules and regulations apply to your conduct as a patient.

Resolution of Complaints
If you have a problem or conflict regarding your care, you have the right to speak to the person(s) in charge or with the Ethics Committee of the Medical Center. Privacy, Confidentiality and Security


You have the right to receive care in a safe setting, free of all forms of abuse or harassment. It is your right to every consideration of privacy concerning your own medical care program. Case discussion, consultation, examination and treatment are considered confidential and should be conducted discreetly. It is your right to expect that we will do all we can to provide for your security and protect your personal privacy. Your healthcare will be discussed only with those involved in your care. You have the right to expect that all information about your care will be kept private unless you give permission, except as required by law. You have the right to request protective services. We will provide information at your request regarding agencies and services to meet your needs. You have the right to know the name of any physician, nurse or other person who provides your care, and the purpose of that care. You have the right to keep your personal clothing and belongings as long as space and safety permit. The Medical Center is not required to replace or pay for lost or stolen items. Choice
You have the right to participate in developing and implementing your plan of care, including inpatient, outpatient, discharge and pain management treatment. You have the right to make decisions about your care and to be given clear information to determine those decisions. You have the right to an advanced directive (i.e., a living will, healthcare power of attorney or advanced instruction for mental health treatment) and to expect the Medical Center to honor your wishes as the law permits. Responsibilities
Ask questions. You and your family are responsible for asking questions when you do not understand your care or what you are expected to do. Let your doctor or nurse know about any pain you may experience. Cooperate with staff giving care. Be considerate of other patients. Respect other patients needs and desires for privacy. Do not have too many visitors in your room at one time. Remind visitors to respect other patients needs for quiet and that smoking is not allowed. Use television, radio, telephone and lights in ways that will not disturb others. Give complete details about past illnesses, stays in the hospital, allergies, medicines, present complaints, perceived risks to your care and other matters relating to your health. Tell the doctor or nurse about any changes in your health that you may experience. Let the doctor or nurse know if you think you cannot or will not follow your treatment plan. Be prepared to accept the consequences of your actions. Do not misuse Medical Center supplies and equipment. You are financially responsible for any loss or damage that you cause to Medical Center property. Keep appointments. If you cannot keep your appointment, you should notify the doctor, the Medical Center or department concerned as soon as possible. Pay bills promptly to keep healthcare costs down. Secure your valuables either by sending them home or by placing them in the Medical Center safe until discharge.

What is Pain?
Pain is an unpleasant feeling in your body. It can be caused by a physical condition. Your pain may be related to past experiences. Your thoughts and emotions may affect your pain. It is common for different people to have the same pain, but to use different words to describe it.

You Have the Right To:


Expect us to listen when you say you are hurting. A prompt response when you say you are hurting. Staff who are concerned about your pain. The best pain relief treatments we have available. Information about pain and pain relief. Receive answers to your questions about pain. Be treated with dignity.

You Have the Responsibility To:


Ask for pain relief as soon as the pain starts. Help doctors and nurses find out how bad your pain is. Tell your doctor or nurse about any pain that will not go away. Ask your doctor or nurse about your pain management plan. Discuss choices you need to make about pain relief with your doctor or nurse. Work with your doctor or nurse to develop a pain relief plan.

El mejor tratamiento para el dolor del que dispongamos. Informacin acerca del dolor y su tratamiento. Recibir respuestas a sus preguntas acerca del dolor. Que se lo trate con dignidad.

Tiene la responsabilidad de:


Pedir alivio para el dolor tan pronto como lo sienta. Ayudar a los doctores y al personal de enfermera a descubrir cun grave es su dolor. Informar a su mdico o al personal de enfermera acercar de cualquier dolor que no se alivie. Preguntarle a su doctor o al personal de enfermera acerca del plan de tratamiento del dolor. Dialogar acerca de las elecciones que debe hacer acerca del alivio del dolor con su mdico o el personal de enfermera. Trabajar con su doctor o el personal de enfermera para desarrollar un plan de tratamiento del dolor.

MC-NorthEasts facilities and parking areas are monitored by our security staff and closed-circuit television cameras. If you need to reach Security, you may call x66595 or contact the Medical Center operator. Emergency telephones are located in the Medical Center parking areas. We require our employees, volunteers and physicians to wear identification badges. If someone you dont know enters your room and cannot show you his or her badge, please call your nurse.

Valuable or Lost Items

Fire Drills
For your protection, the hospital conducts fire and disaster drills regularly. If a drill occurs while you are here, please remain in your room and do not become alarmed. The hospital is a fire-resistant building, and the staff is trained in fire protection.

We ask that you leave money, jewelry, credit cards and any other valuables at home, or give them to a family member or other responsible person to keep for you. You may also ask your nurse to have Security deposit your valuables in the Medical Center safe until your discharge. If you will be out of your room for diagnostic tests or surgery, or if you will be in other parts of the Medical Center, please do not leave valuable property unattended in your room. CMC-NorthEast Security operates a Lost and Found service. You may contact Security at x66595 for Lost and Found items. CMC-NorthEast is not responsible for valuables or personal property.

Specific Ways To Protect Your Personal Belongings While You Are A Patient Include:

Medications
Your healthcare team needs to be made aware of any medications you are currently taking. After being admitted, return any drugs you have brought from home to your family members, unless otherwise instructed by your doctor. All medications you take while in the hospital are prescribed by your doctor, dispensed by the hospital pharmacy and administered by a nurse. Patients are not permitted to administer their own drugs or keep personal medications at their bedsides. If your doctor gives you a prescription, it can be filled in the CMC Rx Pavilion Pharmacy located on the first floor of the Pavilion Building.

Oxygen
Special regulations are in effect in areas where patients are receiving oxygen. Electrically operated equipment and aerosol products are not permitted in these areas. Absolutely no smoking is permitted in any room where oxygen is in use or on standby.

Clothing Keep extra clothing in the closet, your suitcase or in the drawer. Do not place soiled clothing in trash bags. A hospital gown will be provided upon request. Dentures When your dentures are not in use, please keep them in a denture cup and place them in the drawer of your bedside stand. We ask that you do not wrap your dentures in tissues or paper towels, or leave them on your food tray, overbed table or on top of the bedside stand. Do not place your dentures in a paper cup or place them under your pillow or inside the pillowcase. Glasses When you are not using your glasses, keep them in their case and place in a drawer or bedside stand. Do not leave your glasses on the overbed table or on top of the bedside stand. We also request that you do not store your glasses under your pillow or inside the pillowcase. Ring the call bell when you need help getting in or out of bed or to the restroom. If you need additional instructions on how to use the call bell system or bed controls, ask your nurse. Have the bed in the lowest position when getting in and out. Avoid using side tables for support when getting in and out of bed. In Pediatrics, crib rails should be fully raised at all times for children who are able to sit up. Illegal drugs, alcohol and weapons are not permitted on Medical Center property. If fire or any emergency situation arises, remain calm and follow the instructions given by staff members. Be aware of the nearest stairwell, and in case of fire, use the stairs, not the elevator. Speak up. Participate in your healthcare, and ask questions. For Your Safety

Smoking
CMC-NorthEast and its affiliates are tobacco-free. To ensure a healthy environment for everyone, we ask all employees, volunteers, patients and visitors to refrain from using tobacco products while on our property. If you need a tobacco substitute to help you get through the day, our employees and volunteers can direct you to places on campus where substitute products are available.

Wheelchairs
Wheelchairs are available on all nursing units, but getting in and out of them without assistance may be unsafe. Please ask for help from a member of the hospital staff.

Prcticas de evacuacin en caso de incendio


Para su proteccin, el hospital realiza prcticas de evacuacin para casos de incendio y catstrofes en forma regular. Si se realiza una prctica mientras est en el hospital, permanezca en su habitacin y no se alarme. El hospital es un edificio resistente al fuego y el personal est capacitado en procedimientos de proteccin contra el fuego.

Le pedimos que deje el dinero, las joyas, las tarjetas de crdito y otros valores en su hogar, o que se los de a un familiar u otra persona responsable para que los guarde. Tambin puede pedirle al personal de enfermera que Seguridad deposite sus valores en la caja de seguridad del Centro Mdico hasta su alta. Si debe ausentarse de su habitacin para pruebas de diagnstico o una ciruga, o si va a estar en otra parte del Centro Mdico, no deje sus valores solos en la habitacin. El departamento de Seguridad de CMC-NorthEast tiene servicio de "Objetos perdidos". Puede ponerse en contacto con Seguridad a la extensin 66595 para averiguar por "Objetos perdidos". Dentadura postiza: cuando no est usando la dentadura postiza, colquela en un recipiente y gurdelo en el cajn de su mesa de noche. Le pedimos que no envuelva su dentadura con papel tis o toallas de papel, ni las deje en la bandeja de la comida, en la mesa que se encuentra sobre la cama o sobre la mesa de noche. No coloque la dentaria en vasos de papel ni dentro de la funda de la almohada o debajo de sta. Lentes: cuando no est usando los lentes, gurdelos en su estuche y colquelos en un cajn de la mesa de noche. No los deje en la mesa que se encuentra sobre la cama ni sobre la mesa de noche. Tambin le rogamos que no deje sus lentes debajo de la almohada o dentro de la funda.

Oxgeno
Existen reglamentos especiales para las reas en las que los pacientes reciben oxgeno. En estas reas no se permite el uso de equipos elctricos ni productos en aerosol. Est absolutamente prohibido fumar en cualquier habitacin en la que se est usando oxgeno, aun si el equipo est desactivado.

CMC-NorthEast y sus filiales son edificios libres de tabaco. At CMC-NorthEast, we believe that family and friends are important to the healing process, and have established an open visitation policy that will allow your loved ones to visit at any time. We also feel a quiet and restful atmosphere is an important factor in helping patients to recovery quickly. Please urge your visitors to observe these rules, which are designed for the safety and well-being of all our patients. You may have a family member or friend stay with you overnight. A sleep chair is available upon request. Due to limited lobby space, please ask a nurse for assistance with accommodations if a family member requests to stay overnight in a patient care area. Children under 12 years of age may visit in the patient room if they are healthy and accompanied by a parent or guardian. Visitors, as well as personnel, should remember that hand washing is the most important way to prevent the spread of infection. Persons who have recently been exposed to contagious diseases, including the mumps, measles or chicken pox, or persons with a fever, cold or rash should not visit hospitalized persons. Children should not visit in the Medical Center if their immunizations are not current or if their resistance to infection is low. The primary nurse or charge nurse may temporarily alter or limit visiting hours based on changes in the patients condition.

Medical-Surgical Intensive Care Unit


Two visitors are allowed in a patients room at any time except during the hours of 68 a.m. and 68 p.m.

Pediatrics Unit
Parents and grandparents may visit at any time. Visitors under 12 years of age must be accompanied by an adult and may only visit immediate family members. Visitors are limited to three people at any one time. One person may spend the night. Any person taking chemotherapy or susceptible to infectious diseases should not visit the Pediatrics Unit without first checking with the nurse in charge.

Pediatric Intensive Care Unit (PICU)


Two visitors may visit at any one time, and must be at least 13 years old. Visiting hours are from 8 a.m.6:30 p.m. and 810:30 p.m.

Newborn Intensive Care Unit (NICU)


Parents and siblings over the age of five may visit anytime, with up to four visitors at the bedside. Siblings under the age of five may visit with their parents one time each week. Child Life Specialists are available to provide guidance during sibling visits as needed. Our multidisciplinary team meets every Thursday from 9:30 10:30 a.m. to discuss each babys progress and plan of care. Parents are encouraged to actively participate in these rounds.

Acute Coronary Care Unit (ACCU)


One designated family member may stay at the patients bedside except during the hours of 68 a.m. and 68 p.m. Two visitors are allowed in a patients room from 89 a.m., Noon2 p.m., 56 p.m. and 89 p.m.

Labor and Delivery Rooms


Fathers and significant others are welcome anytime. You may have up to three adult visitors at any given time before delivery. When accompanied by an adult, the parents and the newborns siblings are allowed in the labor/delivery area for short visits. However, extended stays are not permitted. Visitors may briefly interact with you and your baby after routine delivery. Following cesarean delivery, the father or significant other may visit in the recovery room.

Cardio-Vascular Recovery Unit


Two visitors at a time are allowed at the following times: 88:15 a.m., 1010:15 a.m., 11:15 p.m., 55:15 p.m. and 88:15 p.m. No visitors are allowed from 68 a.m. and 68 p.m. Only six immediate family members may enter for the first visit after surgery.

Mother/Baby Rooms
Children are not allowed in the room without adult supervision. No visitors are allowed during quiet time from 24 a.m. and 24 p.m.

Dialysis Unit
Visitation during dialysis treatment is limited to one visitor at a time in the treatment area, with no more than one five-minute visit per hour. No visitors are permitted during or removing access.

High-Risk Obstetrics
Visitation may be restricted based on the clinical state of the patient.

Reglamento y horario de visitas


En CMC-NorthEast creemos que la familia y los amigos son importantes durante el proceso de recuperacin y tenemos una poltica de visitas abierta que permite que sus seres queridos lo visiten en cualquier momento. Unidad de terapia intensiva mdica y quirrgica


Se permiten dos visitas por habitacin en todo momento excepto de 6 a 8 a.m. y de 6 a 8 p.m.

Unidad peditrica
Los padres y abuelos pueden visitar a los pacientes en todo momento. Los visitantes menores de 12 aos de edad deben estar acompaados por un adulto y slo podrn visitar a familiares directos. Las visitas se limitan a tres personas al mismo tiempo. Slo una persona podr quedarse a pasar la noche. Las personas que se tratan con quimioterapia o que son susceptibles a las enfermedades infecciosas no deben visitar la unidad peditrica sin consultar primero al miembro del personal de enfermera que se encuentre a cargo.

Unidad de terapia intensiva peditrica (UTIP)

Unidad de terapia intensiva neonatal (UTIN)

Chapel
The chapel is located inside the main entrance of the Clinical Services Building, across from the gift shop. Visitors of all faiths are welcome to visit the chapel for worship services, prayer and meditation. Chapel is held at 11 a.m. on Wednesdays, and is an interdenominational service. No food or drink is permitted in the chapel.

Gift Shop
The Pink Pinafore gift shop, located inside the main entrance of the Clinical Services Building, offers cards, gifts, stamps, magazines, balloons and a variety of items. We accept Visa, MasterCard, Discover, American Express, cash and personal checks with appropriate identification. The gift shop business hours are 8:30 a.m.7 p.m., Monday through Thursday; 8:30 a.m.5:30 p.m., Friday; 10 a.m. 3 p.m., Saturday; and 12:30 p.m.4:30 p.m., Sunday.

Wishing Well
Wishing Well, our cancer care specialty shop, located in the front entrance of the Medical Arts Building, offers a variety of specialty items for cancer patients including sleepwear, clothing, hats, personal hygiene products and inspirational gifts. We also provide professional mastectomy bra and prosthetic services in a private, comfortable setting. We accept Visa, MasterCard, cash and personal checks with appropriate identification. Interpreters
Interpretation services are offered in more than 140 languages thru the AT&T language line. To request this service, please contact your nurse.

Advance Directives
All patients receive advance directives information shortly after admission. This information includes living will and healthcare power of attorney documents. Advance instructions for mental health treatment information are also available. A living will allows patients to specify their wishes to receive or refuse medical treatment should they become unable to speak for themselves. A healthcare power of attorney allows patients to name a person to make healthcare decisions for them should they become incapacitated. If you have not received advance directives information, let your nurse know as soon as possible. If you wish to execute an advance directive, the Medical Center will provide a notary and two witnesses. Neither Medical Center employees nor family members can serve as witnesses.

Mail
Letters, cards and packages for patients are delivered weekdays by volunteers. Mail arriving after you have been discharged will be forwarded to your home. You will be contacted to pick-up if parcels arrive after you are discharged. Stamps and stationery may be purchased in the gift shop. Outgoing mail may be left at the nurses station. Bank Machines
Three 24-hour automated banking machines are located in the Medical Center. They are located near the main entrance in the second floor connector hallway, outside the cafeteria in the Medical Arts Building and at the employee entrance near the Surgery Center.

Newspapers
Local newspapers are available at newsstands near the main entrances, in the second floor connector hallway, outside the cafeteria in the Medical Arts Building, in the lobby of the Mariam Cannon Hayes Family Center and in the Emergency Department.

Ethics Committee
Patients, families, doctors and other healthcare professionals who are faced with difficult medical decisions may choose to contact CMC-NorthEasts Ethics Committee. This committee was created with the realization that modern medicine, which has made great advances in its ability to prolong life, sometimes presents us with situations in which there is little hope of recovery or of regaining an acceptable quality of life. The Ethics Committee assists individuals involved in such situations in considering the available options and reaching a joint decision. The committee includes clergy, nurses, physicians, social workers, Medical Center administrators and community members. The committee does not make decisions for you, but rather helps you evaluate your individual circumstances and alternative courses of action. If you or your loved one need(s) such help, you are encouraged to contact the Ethics Committee. You may do so by calling the chaplains office at x31443, or asking your doctor or nurse to contact the committee for you.

Notary Public
The services of a notary are available for all medically-related documents. To request this service, please contact your nurse.

Patient Education
Channel 15 is the hospitals closed-circuit TV station, which features a number of health-related programs. The Newborn Channels are 19 and 20 (English and Spanish, respectively). If you do not have a program schedule in your room, please ask your nurse for one.

Pharmacy
CMC Rx Pavilion Pharmacy is located on the first floor of the Pavilion Building. The pharmacy offers prescription and non-prescription medications, and accepts most third-party prescription cards and workers compensation prescriptions. Prescriptions are filled 8 a.m. until 5:30 p.m., Monday through Friday.

Special Needs
CMC-NorthEast offers the following services for patients with special needs: Closed-captioned television Telephones equipped with volume amplifiers Talking books/tape recorders on loan from the North Carolina Library for the Blind and Physically Handicapped Letter writing and book reading services Sign language interpreters TDD Telecommunication device Language translation services Community resource referrals for individuals with specialized post-hospital needs If you have a special request, please inform your nurse and every effort will be made to assist you.

Flowers
Flowers addressed to you will be delivered to your room. Both fresh and silk flowers are available in the gift shop, and fresh flower vending machines are located in the second floor connector hallway near the main entrance and in the Mariam Cannon Hayes Family Center lobby. For the protection of our patients, flowers are not permitted in Acute Coronary Care, Intensive Care or Labor/Delivery rooms. Please keep in mind that large plants and flowers are difficult to deliver and remove upon discharge.

Hearing Aids
CMC-NorthEast dispenses and services hearing aids for patients. If you wear a hearing aid and need service, repair or batteries while at the Medical Center, call x31472 between 9 a.m. and 5 p.m., Monday through Friday. If you are interested in having your hearing tested, or in purchasing a hearing aid, you may call 704-403-1472 to schedule an appointment.

Comit de tica
Los pacientes, familiares, mdicos y otros profesionales de la salud que enfrentan decisiones mdicas difciles pueden recurrir al Comit de tica del CMC-NorthEast. Este comit se cre debido a que la medicina moderna, que tanto ha avanzado en su capacidad de prolongar la vida, a veces nos presenta situaciones en las cuales hay poca esperanza de recuperacin o de recuperar una calidad de vida aceptable. El Comit de tica ayuda a las personas involucradas en esas situaciones a considerar las opciones disponibles y tomar una decisin conjunta. El comit incluye miembros del clero, personal de enfermera, mdicos, trabajadores sociales, administradores del Centro Mdico y miembros de la comunidad. El comit no toma las decisiones por usted, sino que lo ayuda a evaluar sus circunstancias individuales y las alternativas que tiene. Si usted o un ser querido necesitan de esa ayuda, le recomendamos que se ponga en contacto con el Comit de tica. Educacin para pacientes


El canal 15 es una estacin de circuito cerrado propia del hospital que muestra varios programas relacionados con temas de salud. Los canales sobre bebs recin nacidos son el 19 y 20 (en ingls y espaol, respectivamente). Si no tiene la programacin en su habitacin, pdasela al personal de enfermera.

Farmacia
La farmacia Rx Pavilion de CMC est ubicada en el primer piso del Pavilion Building. La farmacia tiene medicamentos de venta bajo receta y de venta libre y acepta la mayor parte de las tarjetas de descuento en los medicamentos recetados de otros proveedores de servicios de salud y las recetas de la cobertura por accidentes de trabajo. El horario de atencin es de 8 a.m. a 5:30 p.m. de lunes a viernes.

Flores
Las flores dirigidas a usted se enviarn a su cuarto. En la tienda de regalos podr comprar flores frescas y artificiales y tambin encontrar mquinas expendedoras de flores frescas en el pasillo del segundo piso, cerca de la entrada principal, y en la recepcin del Centro de Familia Mariam Cannon Hayes. Para proteger a nuestros pacientes, no se permiten las flores en la Unidad de terapia intensiva coronaria, Terapia intensiva o Salas de parto. Por favor, tenga en cuenta que las plantas y arreglos florales grandes son difciles de entregar y de llevar al momento del alta.

Patient Satisfaction Surveys


At CMC-NorthEast, it is important to us that you receive excellent care. To obtain your valued opinion, you may receive a phone call from Professional Research Consultants and be asked to participate in a telephone survey. This survey helps us understand your level of satisfaction while you were here. Professional Research Consultants also monitors satisfaction in home care and other outpatient areas including the Emergency Department. You may be asked to rate us on all aspects of your stay. No confidential medical information regarding your stay at CMC-NorthEast will be released to this organization. Additionally, ASK FIRST, a health information resource of CMC-NorthEast, conducts follow-up telephone surveys with patients who have had outpatient surgical procedures. Patients are asked questions regarding any postsurgical care problems, pain management and general staff/facility satisfaction. Please take a moment to give us your opinion. Whatever the nature of your comments, we value your input and will use your ideas to continuously improve the patient services we offer.

Discharge Instructions
Your doctor and nurse will give you instructions about post-hospital care. Your discharge medications can be filled at CMC Rx Pavilion, if you are interested. If you have questions about your diet, activities or other matters, please be sure to ask.

Escort Service
When you are well enough to go home, a patient escort will accompany you from your room to the patient departure area. Although most patients choose to ride in a wheelchair, you are welcome to walk. For your safety, however, you must be accompanied by a volunteer or staff member.

Patient Account Services/Billing, Insurance and Payment of Your Hospital Account


A representative from Patient Account Services can answer questions about your hospital bill and help you interpret financial policies and billing procedures at the hospital. You may contact Patient Account Services by calling 704-403-7973, from 8:30 a.m.4:30 p.m., Monday through Friday. To make your registration as easy as possible, you may be contacted by our pre-registration staff to obtain insurance and billing information prior to your arrival. You may also call 704-403-1285 to pre-register by phone. Please bring your insurance information, Medicare/Medicaid card and/or identification cards with you at the time of your admission. As a courtesy, the Medical Center will submit a claim to your insurance company for you; however, you are still responsible for prompt payment of your bill. In addition, you will be asked to make any co-payments or deductible amounts at the time of your admission or service. Your hospital bill will not include charges for private physicians or for Medical Center-based physicians such as emergency care physicians, anesthesiologists, pathologists and radiologists. These physicians will mail you a separate bill. Patient Account Services is located at 270 Copperfield Blvd., Suite 11, Concord, NC 28025, and is open from 9 a.m.4:30 p.m., Monday through Friday. Payments can be made by cash, check, money order or credit card. Accepted credit cards include MasterCard, Visa, Discover and American Express.

Hospice
Hospice & Palliative Care of Cabarrus County cares for a community of patients living with a life-limiting illness and their families. We can help transition patients from the hospital and follow them into their home, nursing home, assisted living facility or at Hospice House with our team of certified nursing assistants and volunteer support. Bereavement counselors follow the family for 13 months after the death of their loved one. This care and support is available to all patients and their families regardless of age, illness, race, gender or ability to pay. A physician referral is needed for admission to hospice. Hospice & Palliative Care of Cabarrus County offers a Community Greif Counseling Program to individuals (children, teens and adults) or groups of both Hospice and non-Hospice families. Lifeline
Lifeline is a personal emergency response system that allows many people to maintain their independence and live safely in their homes. In the event of an emergency, the Lifeline subscriber pushes a button that dials the Lifeline Response Center. In about a minute the Lifeline monitor will respond for more specific information. Para conocer su valiosa opinin, es posible que reciba una llamada de los consultores de investigaciones profesionales y que se le pida que participe de una encuesta telefnica. Esta encuesta nos permite comprender su nivel de satisfaccin con respecto a su estada.
