I would like to invite you to Mission: Possible a local mission trip hosted by the Oakland County Deanery. It is open to 8th grade through college age students. This summer at Mission: Possible we will be working with amazing organizations in Pontiac like: Grace Centers of Hope, Baldwin Center and CCRT. These organizations will provide opportunities for youth to do things like: paint rooms, knock down walls, take out/replace porches, tile bathrooms, work in a food pantry, clean up walkways, deliver furniture to needy families and more. We will be using shovels, paint brushes, brooms, and sledge hammers! Dont worry parents, most of the leadership and some of the youth are mission trip veterans, safety is our first concern! We also use work gloves, eye protection, hard hats and dust masks, if necessary.

COST: $275 - I have prayer and support letters to help youth do fundraising for Mission: Possible. I also have Oakland Deanery scholarships if the extra cost is a burden in any way.

When we arent working in the community we will explore the unknown and find how God makes all things possible! I hope you can join us as we make a difference in the world, one week at a time!

Here we will make sure all your paperwork is in order, go over fundraising kits, talk about what to expect during Mission: Possible and give out a devotion for the weeks leading up to Mission: Possible. PARENTAL CONSENT: I give full permission for my child to attend and participate in Mission: Possible Pontiac 2014, a mission trip sponsored by the Oakland county deanery. DURABLE POWER OF ATTORNEY FOR EMERGENCY MEDICAL/SURGICAL CARE: I represent and warrant that to the best of my knowledge my child has no illness, congenital defect, or other health condition that makes my childs participation in this event unsafe for my child or other participants eve n with reasonable accommodation of any disability. I appoint Andrew Wilke, Chris Gannon or any mission trip appointed adult leadership as my agent pursuant to MCLA 700.5103 for the limited purpose of consenting to any emergency medical or surgical care for my child that may be recommended by a physician regarding any injury or illness that may arise while my child is participating in this mission trip. Andrew Wilke, Chris Gannon or Mission trip adult leadership shall use their best efforts to contact me in advance of exercising this delegated power so that I may direct my childs care. If I am unavailable after reasonable attempts to contact me, or if my childs condition makes any delay medica lly unadvisable, Andrew Wilke, Chris Gannon or mission trip adult leadership may exercise the delegated power without communicating with me first. This durable power is not affected by the incapacity or disability of the principal and shall continue in full force and effect during such disability or incapacity. I agree to pay all health care providers for any services rendered to my child pursuant to this delegated power, whether through health insurance or private payment. TRANSPORTATION RELEASE: I give full permission for my child/children to be transported to activities in conjunction with this event, including being able to attend and participate in activities off site of main program campus. WAIVER OF LIABILITY:

I release Andrew Wilke, Chris Gannon, Advent Episcopal Church, All Saints Episcopal Church, St. James Episcopal Church, the Oakland County Deanery, its agents, employees, officers, directors and volunteers, from any liability of any kind or nature that may arise in any way from my childs participation in this event. I understand that it is the adult leaderships choice to send any child home during Mission: Possible for discipline reasons or other reasons and that it is my responsibility to provide a ride home for my child(ren). MEDIA RELEASE:
I give permission for photographs or video of my child or myself to be taken during my childs or my participation in this ev ent, to be used by Andrew Wilke, Chris Gannon, Advent Episcopal Church, St James church, All Saints church, EDOMI or the Oakland Deanery for promotional purposes.

Mission: Possible Pontiac 2014


It is our policy during Mission: Possible Pontiac 2014 to have the following procedures and practices: 1. Driving policy: Written permission for young people (under the age of 18) to be transported to and from youth events by someone other than parent/guardian. 2. Sign-in/Sign-out procedure: To have parent/guardian identify which adult(s) is authorized to sign-out young people upon conclusion of the mission trip. 3. Youth driving policy: The understanding that if a young person drives themselves to the mission trip that no other youth are allowed in their car or allowed to ride with them. In accordance with this policy, we ask that parents/guardians please complete and include this form with all application and/or registration forms if someone other than the legal guardian is dropping off or picking up your youth for Mission: Possible 2014.
