Important Announcement for FLVS Advanced Placement (AP) Students and Guardians: Dear FLVS Full Time Students

and/or Guardian, Please read this message in its entirety, as it is important to the completion of your AP course(s) through Florida Virtual School Full Time. To complete this process, you are required to take the AP exam(s) in May. Exams for AP courses are only offered once a year and as an FLVS Full Time student, you will need to request that your local zoned high school orders the test(s) for you in advance. In order to do so, you must take the AP exam registration form to your zoned high school no later than February 7, 2014. NOTE: The exams must be ordered by the local high school AP Testing Coordinator or School Counselor and cannot be ordered by individual students or parents. IEP and 504 Plan Students: Please provide a copy of your active IEP or 504 Plan to the school testing coordinator. Accommodations may be provided at the discretion of The College Board. All FLVS AP students are required to participate in the AP exam(s) in order to receive any course credit. Districts have been provided with information regarding how to order AP exams for FLVS students. To make sure that you have your exam(s) ready for you in May, you will need to make arrangements to test at your local participating school by February 7, 2014. Any late registration fees are the responsibility of the FLVS student. A copy of the AP Exam Order Request Form is included. Please fill out and take this form to your school counselor or testing coordinator at your zoned high school by February 7, 2014, so they can order your AP exam. You do not have to return this form to FLVS; it is strictly for the use of the school to order your exam. It is recommended that you keep a copy of the order form for your records, the date you submitted the form to the school, and the school staff members name. AP Exam Order Form 2013-2014


The College Board requires that AP Exams for FLVS students (in both Part-Time and Full-Time programs) be ordered by the zoned school in the district where the student lives in which the student is enrolled. We greatly appreciate the help of our district contacts and school counselors in providing this testing opportunity to FLVS AP Students. The purpose of this form is to request that the school counselor or testing coordinator order the AP exam(s) for the student shown below. FLVS will reimburse the district for the cost of the exam. Invoices will be available to the district and school counselors in February, 2014 and can be submitted after the AP exam window in May. Invoices must be returned to FLVS. This AP Exam Order Form does not need to be returned to FLVS. IMPORTANT REMINDERS: Participation in AP exams is REQUIRED for FLVS course credit and issuance of a final grade. Florida Virtual Schools Online Provider Code is 006 and must be entered on part G of the AP Exam answer sheet. Students must include this number on their answer sheet. If a student does not enter this code on their answer document, then FLVS will not receive their AP test scores. Students must also record their name on the AP exam exactly as they have registered with FLVS, i.e., if you registered as John Smith with FLVS, then John Smith must be the name you use on the AP exam (Not Johnny Smith).

Students: Please take this form to your zoned school counselor or testing coordinator prior to February 7, 2014 to ensure that they have ample time to order the exam. If you have an active IEP or 504 Plan, please attach a copy with this order form to discuss the accommodation options with the testing coordinator. IMPORTANT INFORMATION FOR TEST DAY (PAGE 1 OF 2)

On your student pack on the day of the test, you must print your legal name. Please be sure that your registered name with Florida Virtual School is your legal name. If not, please make changes to your name with Florida Virtual School well before your test date. The school code of your school of enrollment should be the school code entered on the student pack, which for FLVS Full Time is 102414. Your testing coordinator will NOT provide this information for you. You must remember this information.
