Company History: WellPoint, Inc. was formed when WellPoint Health Networks Inc. and Anthem, Inc.
merged in 2004. Anthem was the outgrowth of two Indianapolis-based corporations formed in 1944 and 1946
as mutual insurance companies, commonly known as Blue Cross of Indiana and Blue Shield of Indiana.
WellPoint Health Networks Inc. was formed in 1996 through the merger of WellPoint and Blue Cross of
California into a single stockholder-owned company.

WellPoint’s health benefits operations include Anthem Blue Cross and Blue Shield plans serving members in
Colorado, Connecticut, Indiana, Kentucky, Maine, Missouri (excluding 30 counties in the Kansas City area),
Nevada, New Hampshire, Ohio, Virginia (serving Virginia excluding the city of Fairfax, the town of Vienna
and the area east of State Route 123) and Wisconsin; Anthem Blue Cross and Anthem Blue Cross Life and
Health Insurance Company serving members in California; Blue Cross and Blue Shield plans serving
members in Georgia; and Empire Blue Cross Blue Shield (or Empire Blue Cross in select upstate NY
counties) in portions of New York.

UniCare
WellPoint’s full-service health plan offered in several different regions throughout the country, including those
areas where the company does not have a licensed Blue plan. Offering a comprehensive array of health care
plan and specialty products, UniCare preserves member choice at competitive prices.

Specialty Companies
WellPoint has a number of specialty companies that provide a full range of benefits and services to our
health plan customers, including pharmacy benefit management, life and disability insurance benefits and
dental. WellPoint also provides vision and behavioral health benefits.

National Government Services


An administrator of government health benefits programs, primarily Medicare. National Government Services
serves more than 24.5 million people with Medicare in 25 states and 5 U.S. Territories.
