R&R is Teaming with Philip Hawkins Architect, Inc. + Associates (PHA), and sub-contracting with select organizations/Major Trade Subcontractors to bring to the Department of Agriculture Forestry Service a Team (Team R&R) that has extensive past performance and experience working with the Federal Government on similar contracts that include Airfield environments. Team R&R Members have developed good reputations for consistent quality and all are very familiar with the three (3) Phases of Contractor Quality Control Reviews for Construction (Initial, Preparatory and Follow-Up) as well as the other Management Requirements and Controls required for the U.S. Air Force (USAF), Naval Facilities Engineering Command (NAVFA), and U.S. Army Corps of Engineer (USACE) Projects. Team R&R brings over 70 years Corporate Experience and Key Personnel with over 100 years of combined experience along with a staff of; Architects/Design Managers, Engineers, Certified USACE Quality Control Manager(s), Superintendants, Safety Managers, and Project Managers with extensive knowledge in Design-Build Projects, and offers the following list of completed Projects which reflect elements of similar complexity for consideration of past Performance and Experience.

R&R Construction Design-Build Experience


Roosevelt Center; Design-Build Services, on a historic sight for the design and construction of 22,000 SF 3-Story Mixed-use building consisting of retail, residential, food service, administrative and storage spaces. Construction requirements included accomodation of complex code requirements for 3 diverse occupancies (fire separation, access, egress, sound and odor) R&R provided the entire scope of work for the site and buildings. Past Performance: The project was located in a tight corner lot in downtown North San Diego County with zero lot lines on two sides and heavy traffic streets on both sides. Unforseen conditions included hazardous waste from early 1900s oil burning reservoir that was built unnderground. The entire project was completed within budget and on-time. Contract Completion Date:3-2009 Contract Value: $5,800,000 POC:GeneLeisher/Project Owner (T) 760-433-7767 (E) leisher@cox.net Aircraft Hangar Construction Experience Palomar Airport Center; Construction of an aircraft hangar and 3-story administrative building totaling 220,000 SF with site improvements on 15 acres. Scope of work included reconstruction of runway and construction of new taxiways. The building was structural steel with B-Deck and smooth ridged insulated decorative panels and curtin walls. The project included a Mega door system which allows 110 free span openings for aircraft and support of structural beams that are dimensionally over 6 deep and mounted within 1/4 tolerance for the total free span. Past Performance: The project was built over an old landfill, and challenges for this project included the need for a 700 building that utilized a ramped center cooridor and stepped North/South and East/West to accommodate grades of entry for large aircraft as well. The project completed on time and within budget. Design-Bid-Build Construction Experience The Quad Phase I; 150,000SF The project is a Multiple Phased project that consisted of three 4-story mixed use buildings used for retail and residential dormatories. The next phase of the development will add two five-story buildings, providing an additional 309 beds, along with ground-floor spaces housing 8,000 feet of retail and a suite of student facility. The buildings are concrete post tension deck with 5 floors of wood framing and combine energy-efficient construction with amenities that include two classrooms, a fitness space and gaming room, along with an indoor/outdoor lounge, pool and outdoor barbeques. Exterior is mixed stucco, glass trespa, aluminum exterior panels, and hardibord siding. Past Performance: R&R provided the entire scope of work for the site and buildings. All completed phases for this project have been on time and within budget. PHA INC Design-Build Experience as Prime Contractor USACE Hamilton Nursery Remodel/Conversion; DesignBuild Services for design and construction/conversionremodel of a former water treatment facility to plant nursery which supports the largest restoration project by the USACE in the US to date. The project required full AE design-build services, civil design and construction, sitework, underground utility reconnections and exterior improvements. Trades for the project included light carpentry, finish work, painting, flooring, electrical, HVAC, plumbing, excavation, landscaping, demolition and grubbing. PHA provided all architectural design, consultant and utility coordination in-house. Water sewer and electrical utilities were designed and installed underground to existing City utility services. A 2,000 SF steel open-air 50% shade structure was also designed and built as an addition to the existing building for nursery use. Past Performance: The project was modified to accommodate unforseen issues with the local permitting agencies, and was completed on time and within budget. PHA maintained solid working relationship(s) with the Contracting Officer and Contracting Officer Representative(s) throughout the duration of this project, to resolveunforeseen challenges working with the local permitting agencies to reconnect the underground utilities. As the design-build contractor, PHA was able to utilize our architect-led capabilities to push the project through the onerous obstacles from the local agencies. Contract Completion Date: 8-2012 Contract Value: $1,168,000 Contract Number: W912P7-10-C-0034 POC: Gerry Prado (T) 415-884-0640 (E) gerardo.c.prado@usace.army.mil A-E Experience/Pre-Engineered Steel Buildings PHA was the lead architect and provided full Architect and Engineering Services, as part of a Design-Build team, to the USAF for the Global Hawk Administrative multi-function facility that serves as the primary receiving area for visitors to the Beale AFB Global Hawk Operations. The Scope of work included adding 2,200 SF to an existing pre-engineered steel building along with remodeling and converting the existing 4,000 SF structure. Uses for this facility include maintenance, storage, conference, reception and administrative office spaces. The scope of work included all A-E Services including: site/building survey, utilization studies, design project management, as built drawings, construction administration and interior design, flooring and finishes. Past Performance: The design of this project was completed within the allocated required 90 day design time frame. The initial design construction cost estimate, and actual construction cost were the same. PHA was the lead architect and provided full Architect and Engineering Services, as part of a Design-Build team, for the Aero Club Maintenance Building/Hangar Facility. Uses for this building include: small aircraft maintenance and repair including storage and maintenance spaces. This project is a new ground up steel frame building on concrete slab. Scope of work included full A-E services for site planning and building design of a 4,000 SF pre-engineered building and included all electrical and utilities, runway connection, water and sewage systems, communications, alarms and security systems, plumbing, and HVAC. This project was designed according to UFC4-010-01 (DoD Minimum Antiterrorism Standards for buildings). Past Performance:The design of this project was completed within the allocated required 90 day design time frame. The initial design construction cost estimate, and actual construction cost were the same. There have been no change orders. The CBP chose a Sprung stretched skin building with automatic clamshell door to serve as the hangar for this project. The project also included a new electric service, new wet utilities with waste storage tanks, 40,000 cfm ventilation fans, light diming capabilities, Siemens LED Heliport lighting and red solar lighting all within a secured fenced compound per US Dept. of Homeland Security specifications. Technical Approach
Team R&R understands the need of the USDA Forest Service to construct a fully functional Helibase with Supporting Buildings and Utilities on approximately 9 acres of unimproved land. Based on defined scope of work and the findings of our visit to the site, Team R&R is confident of our ability to provide required design-build services for fully functional Helibase, including, but not limited to: Construction staking, site grading, erosion protection, topsoil stockpile and placement; Excavation, compaction and finish grading; Design and construction of a ~ 2,473 square foot administrative office building with patio; Design and construction of a ~1680 square foot 3-bay garage and a ~12,000 square foot hangar; Furnish and install two (2) complete septic systems; Construction of two (2) landing pads and aprons; taxiways; service roads; walkways and parking areas, fuel containment area, all constructed of Portland Cement Concrete; Asphalt paving of driveway; Design, furnish, and install all site utilities (water, electrical, mechanical); Furnish and install two (2) 20,000 gallon water tanks and all appurtenant water lines, valves andenclosures, pressure system, and hydrants; Furnish and install approximately 2840 lineal feet of 8 foot high chain link fence with barbedwire top, electric gate with keypad, and a three foot personnel gate; Furnish power and communications to the site (underground) from near Interstate 8, approximately 1.0 miles, in coordination with SDG&E and AT&T design requirements; Installation of all stormwater Best Management Practices (BMPs), detention pond, drainage features and site improvements; Drilling of new vertical well, well development, and yield and draw down testing, including allwell head improvements, pump and pump control system components. Team R&R will provide fully Functional Rigid Global Pre-Engineered Steel Buildings for UnInsulated Hangar, Office in accordance with Title 24 with metal lockers, and Garage, Helibase with Lighting, and Lightening Protection, Industry Standard Commercial Grade Appliances, Fully Functional Voice Data System including all Termination, Cable Trays, PA System/Speakers, Finishes and Millwork as noted on Kitchen Creek Plans and Specs (telephones are excluded) . Upon review of the plans for the Happy Camp Helibase support buildings, we would like to point to a concern regarding the cementitious shiplap panels called out in the specifications. We feel they may pose durability issues, and would like to propose an alternate Faux (Rigid) Rock wainscoat metal panel in lieu of the cementitious shiplap, which is asthetically pleasing, factory supplied and thus fully integral to the pre-engineered building system, but provides a more lasting and durable finish. We have included this as a value engineering alternate in our bid proposal and a schematic is attached. Our team noted the site contains minimal vegetation and is cut to fill expecting no need for substantial import or export of material. USFS noted the possiblity of lead contamination from spent amunition containing lead may be encountered. Should spent or unspent ammunition be encountered, Team R&R will immediately cease all work, and perform a hazard analysis. USFS will be notified of our findings and a determination made for the proper course of action. Team R&R will address the personal safety and environmental issues reltated to the possibity of lead contamination with an activity hazard analysis and training of personnel to prevent lead exposure beyond permissible limits. R&R is committed to adressing the issue in all aspects which include preventing migration of any lead contamination off of the site, ensuring all lead contamination is fully encapsulated.

Our team also noted that only a Narrow two lane county road provides access to helibase. All work performed on or near the right-of-way will be completed with an approved Traffic Control Plan. Team R&R recommends that USFS might want to consider installing a permanent sign indicating a turn-off for the Helibase, as it appears to be a somewhat high speed travel area. Resources No part of Team R&R, or of its Major Subcontracting Partners are currently operating at or near capacity, so we anticipate zero difficulties with the provision of all required personnel and equipment to the Project. Team R&Rs construction personnel depth and depth of support personnel will allow us to provideuninterrupted construction services, exploring contingencies when necessary, without delay impacts. A full listing of our Resources and Current Contractual Obligations are outlined in Section: Management Plan. Site-specific Safety Plan: Team R&R will utilize a comprehensive, vertically integrated safety program that follows USACE EM385-1-1 guidance (See Full Safety Program Table of Contents below). The Full Safety Plan will be supplemented by aProject Specific Plan for the Helibase that is enforced by Team R&R Safety Manager and Site Superintendent. All personnel entering the construction zone will be required to comply with the Site Specific Safety Plan, including Major Teaming Subcontractors and will be briefed prior to their entrance onto the construction site. The Site Specific Safety Plan has a project specific activity analysis for each definable feature of work, including but not limited to: excavation/grading/trenching and backfill for utilities, identified lead contaminates, traffic control, reinforced concrete installation, building erection/, electrical and lighting work,and finish trades. Our safety policies and procedures were designed to promote Zero Incidents and safe working environments for all staff, subcontractors, and project owners in full compliance with the U.S. Army Corps of Engineers Safety and Health Requirements Manual (EM385-1-1, Revised November 2003). It is the policy of Team R&R that accident prevention shall be considered of primary importance in all phases of operation and administration. Team R&R Safety Manager will bedesignated to oversee the effective execution of our Safety Program. The Safety Manager will have the authority on this project to stop work performed in an unsafe manner or without appropriate safety precautions until proper corrective action has been taken. The Safety Manager will also be responsible for certifying that all workers are aware of proper safety procedures, and will conduct weekly safety training sessions to ensure that everyone understands the particular dangers and hazards of the work in progress and that all site workers are familiar with safe operation and handling of equipment and materials. The Safety Manager will physically conduct a weekly safety administrative and safety equipment review. The USFS representatives are encouraged to attend any of these meetings. Along with the Quality Control Manager, the Safety Manager will physically conduct an on-site safety and quality control audit each week. The review will examine a checklist of critical items of safety and quality control concerns. The construction work site will be reviewed and evaluated utilizing our scoring system,according to Team R&Rs safety and quality control standards. Prevention: The prevention of accidents is an objective affecting all levels of the organization and its activities. Employees and subcontractors are expected to assist management in accident prevention activities. Unsafe conditions must be reported on the same workday they have occurred. The Safety Plan outlines methods to detect, correct, and prevent unsafe practices and hazardous conditions, tailored to this projects specific circumstances. The plan also provides information in supplying and properly using protective equipment, safety mechanisms, and establishes a regular schedule for tool and equipment maintenance. All employees and subcontractors will also be shown the location of all emergency equipment and Material Safety Data Sheets (MSDS), telephones, and evacuation routes. Standards in the areas of housekeeping, protective safety equipment, weekly safety meetings, fire protection equipment, and treating, reporting, and investigating injuries will also be defined in our complete Safety Plan. Accident Reporting: An injury that occurs on-site or off-site, even a slight cut or strain, willbe reported as soon as possible. In no circumstance, except an emergency, should an employee leave a shift without reporting an injury that occurred. The Safety Manager will coordinate all efforts and oversee the enforcement of all company safety rules and policies. Project Site Visitors: All persons visiting the project site shall sign in at the project site office, be accompanied by an authorized escort while on-site, wear a hard hat and safety glasses, comply with all project safety rules, and sign out upon leaving the project facility. This policy promotes the safety and security of all visitors. Mandatory PPE: When on the project site, all workers and site personnel are required to wear the following Mandatory Personal Protective Equipment (PPE); hard hats, safety glasses and leather steel safety toe shoes. This policy has reduced personnel injuries for employees and subcontractors.

Environmental Hazards: Environmental hazards are given extra consideration to verify that all dangerous procedures and materials are handled in accordance with Occupational Safety and Health Administration (OSHA) and Environmental Protection Agency (EPA) guidelines. Successful operation of Team R&R depends not only upon exemplary construction, but the careful execution of safety policies and procedures from construction to closeout. There is no task so important, nor any service so urgent, that cant we take time to work safely

All Switches, Outlets, Fire Alarm Strobe and Pull Station Boxes will be installed as per ADA requirements, and in accordance with National Electric Code, (NEC), National Fire Protection Association, (NFPA), and UBC. v. Domestic Water and Hydronic Systems if applicable will be installed within the Light Gauge Frame System. All Valves, Water Hammer Arrestors, Control Devices and similar will be installed as per UBC. vi. Drywall, Sheathing may be completed on one side, with Insulation installed in applicable areas that do not conflict with MEP activities. No MEP systems will be covered until the Quality Control Team has verified that the appropriate pressure test, or hydrostatic test have been performed on wet systems, and all MEP systems are correctly fastened and connected. 7. Tape, Bed, Texture and Paint a. Preparatory Phase Meetings for Tape, Bed, Texture and Paint will be held prior to work to verify Finish Texture and Color b. Areas that are determined complete with framing, MEP, Drywall and Insulation will be Taped, Bedded and Textured as per the Approved Design Requirements. c. When the required finish is achieved, the Area will receive Primer and Paint to the required mil thickness. 8. Ceiling Grid, HVAC Duct, Grilles and Registers, Electrical and Special System(s), Conduits, Lighting, Insulation, and Ceiling Tiles a. Prior to installing the Ceiling Grid the following will be completed; i. Preparatory Phase Meetings and Hazard Analysis for definable features of work ii. Install all Special System, Electrical, Life Safety and Plumbing Piping and Conduit iii. Install all Sheet Metal Duct iv. Prime and Paint Perimeter Wall at Ceiling Grid Ledger Elevation b. Ceiling Grid will be installed level using Laser Level or manometer and will be suspended in accordance with all regulating requirements. c. All HVAC Flexible Connections, Registers, Grills Diffusers will be installed. d. Lay-In Light Fixtures will be installed as per approved lighting plan. e. Special System Devices will be installed, and Ceiling Tiles will be provided for devices that are surface mounted to Lay-In Ceiling. f. Prior to installing Lay-In Ceiling Tiles and Insulation, the QC Team will verify all items are installed as per approved design criteria and all suspended grid elements are connected as per UBC and applicable requirements. Ceiling Tiles and insulation will be installed concurrently. 9. Hollow Metal Door and Window, Steel and Wood Doors, Glass and Glazing a. Prior to Installation, Preparatory Phase Meetings and Hazard Analysis for Hollow Metal Door and Window, Steel and Wood Doors, Glass and Glazing will be held to; i. Verify Material is in Conformance with Final Design criteria and Approved Submittals ii. Coordinate with Framing and Drywall and Flooring Trades

b. Hollow Metal Door Frame installation will be concurrent with Framing and Drywall c. Hollow Metal Window Frames will be coordinated with the Demolition activity as well as the Light Gauge Metal Framing to ensure the penetrations remain closed and weather tight to minimize security concerns and water intrusion during installation. d. Steel Doors will be installed as soon as practicable and will be coordinated to minimize security concerns e. Wood Doors will be installed after Painting and Suspended Ceiling activities, but before the flooring is installed, with adequate finish floor clearances. f. Glass and Glazing will be installed as soon as practicable and will be coordinated to minimize security concerns and water intrusion 10. Mechanical, Electrical, Special Systems, and Plumbing a. Mechanical i. Preparatory Phase Meetings and Hazard Analysis for Mechanical will be held to; 1. Coordinate with other Trades 2. Verify Material Conformance with Final Design criteria and Approved Submittals 3. Verify HVAC Controls and Fire Detection System are integrated appropriately 4. Coordinate with Electrical Scope to verify proper circuit capacity and disconnects are provided 5. Verify Condensate lines are to be installed appropriately 6. Verify Roof Curb, Frame, Flashing and Counter Flashing are appropriate ii. Remove and Replace the HVAC Units as per Approved Design Criteria iii. Install Duct, Registers, Grills and Insulation b. Electrical i. Preparatory Phase Meetings and Hazard Analysis for Electrical will be held to; 1. Coordinate with other Trades 2. Verify Material Conformance with Final Design criteria and Approved Submittals 3. Verify HVAC Controls, Special Systems, and Fire Detection System are provided conduits and integrated appropriately 4. Coordinate with Mechanical Scope to verify proper circuit capacity and disconnects are provided 5. Verify Grounding to be installed appropriately ii. Install Electrical Conduits in Framing, above Suspended Ceiling, and Penetrate as required for HVAC Disconnects iii. Install Lighting and switches as required for new circuits iv. Install new subpanel for additional and over current circuits v. Install Fiber Optics and Hardware as per approved design criteria and as specified c. Plumbing i. Preparatory Phase Meetings and Hazard Analysis for Fire Alarm and Special Systems will be held to; 1. Coordinate with other Trades 2. Verify Material Conformance with Final Design criteria and Approved Submittals 3. Verify Telephone Lines are provided and installed appropriately 4. Verify Fire Alarm System is installed appropriately a. Pull Stations, Enunciators and Strobes are Located appropriately b. Smoke, Heat and Duct Detectors as applicable are located and integrated accordingly with HVAC Controls and Electrical Circuits. ii. Install Fire Alarm, Telephone lines, Fiber Optics and Hardware as specified in the scope of work and design criteria 11. Flooring; Carpet and VCT a. Prior to Installation , a Preparatory Phase Meetings and Hazard Analysis for Flooring will be held to; i. Verify Material and Color is in Conformance with Final Design criteria and Approved Submittals ii. Verify all above floor work is complete, including Painting, MEP Trim, all Suspended Ceiling Work. iii. Verify floor flatness is in conformance with applicable standards, and apply Floor Leveler where required. b. Install Flooring in accordance with Manufactures Recommendations. 12. Concurrently with the Building Construction the site Improvements will be constructed a. Water tanks and all Utilities will be completed b. Fencing and Bollards will be installed c. Airfield and Access Road Concrete Paving will be placed d. Asphalt Paving and Berms will be completed e. Miscellaneous Concrete: Towers, Flagpoles, LP Fas Tanks, Well House and other ancillary items will be installed 13. Upon Substantial Completion of the Site Improvements, the Planting and Seeding activity will be executed. 14. Upon Substantial Completion of Base Bid Work; a. Team R&R will ensure that all deficiencies tracked through the CQC 3 Phase Process are completed, b. Team R&R will perform a CQC Pre-Final Inspection, and solicit input from the COR, and will rectify all contractually deficient work. 15. After completion/correction of the Pre-Final Inspection deficiencies, Team R&R will request and schedule Final Inspection.

16. Team R&R will document and correct all contractual deficiencies identified during the Final Inspection. 17. Close-Out a. Team R&R will provide bound copies and electronic paperless copy of all approved submittals and As-Built Incidental / Shop Drawings; b. Installation Instructions, O&M Manuals, and c. Warranties for material and workmanship as specified.

THREE PHASES OF INSPECTION


As part of our technical approach to this project, Team R&R will utilize a USACE standard 3-phase process which has been proven over many years to be successful in maintaining project quality control and safety. This process prepares the key performers of the major elements or definable features of the project scope and allows for discussions regarding site safety, potential interference or coordination with other subcontractors and generally brings out any potential problems before they become factors in the work.The process goes on to inspection of the work at key milestones to insure compliance with the work preparation phase and with the construction documents. The three phase process is further defined as follows: Preparatory Meetings Preparatory Meetings shall be performed prior to the beginning of any major definable feature of work. A meeting is held for each crew performing such feature or when members of the crew change. Preparatory Meetings are conducted by the Quality Control Manager and/or designee after a complete review of all applicable blueprints, specifications, shop drawings and related submittals has been made. A Preparatory Meeting Agenda is completed for each definable feature of work and distributed at the meetings. At the Preparatory Meeting, the Superintendent and Foreman (involved in this phase of construction) will coordinate with Quality Control and Safety personnel and introduce their plan for accomplishing the work. The Contracting Officer shall be notified at least 48 hours in advance of the Preparatory Meeting. The following items are typically discussed at such meetings: d. Review of the applicable specifications. e. Review of applicable blueprints and shop drawings. f. Review of related submittals and a check that all related submittals, shop drawings and materials have been tested (if applicable), submitted and approved. g. Review of the detailed sequence of the execution of the work. h. Discuss required testing and frequency. i. Review provisions to ensure controlled inspection and testing. j. Examination of the work area to ensure that all required preliminary work has been completed and is in compliance with the contract. k. Examination of the related material, review of the Receiving Inspection Reports and verification that the items received are in compliance with the contract and are properly stored. l. Review of the Activity Hazard Analysis Report to ensure that all safety precautions are met and the required safety equipment has been purchased and is available. m. Review and document the workmanship expected for the definable feature of work. n. Meeting Minutes shall be recorded and distributed within 48 hours of the conclusion of the meeting. Initial Inspections Initial Inspections shall be performed at the beginning of any definable feature of work and must be repeated at any time new workmen or new crews are assigned to the work or if the required standard of work is not being met. The government shall be notified at least 48 hours in advance. The same personnel who attended the Preparatory Meeting shall also attend the Initial Inspection. These should include: the

Superintendent and Foreman, Safety Personnel and the Quality Control Personnel. The following shall be accomplished during such meetings; a. Review the minutes of the Preparatory Meeting and verify that the work complies with the design documents; i.e., submittals, specifications, blueprints and/or shop drawings. b. Resolve all differences. c. Verify adequacy of inspection and testing. d. Establish a level of workmanship and verify that it meets the requirements. e. Check Safety and review the Activity Hazard Analysis Reports. f. Provide documentation of the previous inspection of the work area. g. Re-examine the work area for compliance. h. Meeting minutes shall be recorded and distributed within 48 hours of the conclusion of the meeting. Note: Preparatory Meetings and Initial Inspections are intended to prevent non-conformances from occurring. Follow-Up Inspections Follow-Up Inspections will be performed daily to ensure that the control established during Preparatory Meeting and Initial Inspection continues to provide a product that conforms to the contractual requirements. a. Construction activities shall be inspected by Quality Control in accordance with Quality Control Procedures. Forms for various activities will be filed and attached to the Contractors Quality Control Report. b. Installation and testing activities which do not comply with the requirements shall be documented on a Non-Conformance Report (NCR), Refer to Section 8 for Non- Conforming Items. c. Modifications, repairs and/or replacement of materials and/or parts performed subsequent to Final Inspection shall require re-inspection and/or retest to verify acceptability. Inspection and testing documents shall be filed and maintained in accordance with Section 5, Reports and Records and shall be available for review by the Contracting Officer.

Team R&R Project Manager John Very will work in tandem with Philip Hawkins AIA, LEED AP Design Manager (PHA) to provide comprehensive Design-Build Services in response to the requirements of this Project from project NTP for design phase through final close out. Team R&R members have many years of experience in this approach to the design-build delivery system which will ensure close communication among all team members and major subcontractors from the start of the USFS project. As an inclusive Design-Build Team, we will work together with the Forest Service on this project from inception to completion to ensure the entire scope for schedule, budget, and quality are maintained throughout. This highly effective specific and flexible management approach has been developed and refined by our Team and successfully utilized to complete recent USACE and USAF Projects. This concept further allows DesignBuild Cost Control to be monitored and maintained from project inception through the Project Managers involvement and review of the various design stage submittals. Value engineering, if required, can thus occur during design progress. The Design Manager will ensure that project quality control is detailed both in the drawings and specifications, as well as, monitored during the construction progress through regular on site construction meetings and observation. Ultimately, Team R&R Key Personnel; Project Manager, Design Manager, Project Superintendent, and Quality Control Manager and Safety Manager are responsible for all that happens or fails to happen during the design and construction of this project. Early integration of Team Members and Major Subcontracting Partners into the Design Process ensures a successful outcome. This process of early integration of the construction team and subcontractors into the design process has already been highly successfully achieved by Team R&R Members over several years through its development of the inclusive design-build team concept. Design-Build projects have to begin with a cost estimate that becomes the final contract price, exclusive of change orders to the owner, unless involving unforeseen conditions or owner requests. Team R&R works with the Major Subcontracting Partners during the design phase and utilization of materials and methods of construction are forecasted in their initial bids. This process has been developed and refined by the PHA Design-Build and the R&R Construction Teaming Partnersthrough the completion of several designbuild primary contracts for the DoD recently and through PHA providing the architectural support services to other general contractors on DoDdesign-build projects over multiple years. The Design Manager is involved in the construction review process from submittals to actual field observation. As both an Architect and General Contractor, Mr. Hawkins involement will provide the Forest Service with a more comprehensive design-build approach, rarely found within or fully understood by our competitors. As part of the overall Project Management Plan (PMP), Team R&R empowers its workforce, and all Major Subcontracting Partners to operate within the framework of the PMP. The following principles will be followed as part of the PMP AND the Quality Control Plan to ensure success of this Project: One project, one team, and clear communication lines Manage all work with the USACE/NAVFAC 3 Phase Process, Project Controls, Standardized Forms, Best Management Practices required by the DOA / USFS The Team is responsible for project success One project, one team, and clear communication lines Once this Project isplaced in the hands of Team R&R, the on-site Construction Team consisting of: Superintendent, Quality Control Manager, Safety Manager, Trade Supervisors, Foremen, Tradesmen, Laborers, Sub-Contractors, and Suppliers will all work under the ultimate direction of the Project Manager.

The on-site Construction Team understands that working with the Forest Service management personnel is an integral part of the success of the Project. The General Superintendent is Team R&Rs primary interface with the Forest Servicefor daily coordination, scheduling and issues regarding production. All Submittals and Request for Information will be processed through and transmitted to the Forest Service Contracting Officer CO or Contracting Officer Representative COR. R&R anticipates the Forest Service will assign an Engineering Tech / Inspector / COR for daily coordination issues, and expects written and/or verbal communications at the Project Site will be directed / transmitted to the Team R&R QC Manager and/or Superintendent, and either QC Manager or Superintendent will communicate through the COR / Engineering Tech / Inspector. The Project Manager will be Team R&RsConstruction Phaseprimary interface with the Forest Service for issues regarding contractual matters and contractual modifications to scope, schedule and/or budget, but the signature will be either the Project Manager or an R&R Principal.

Quality Control Plan


Upon Award, Team R&R will provide a fullQuality Control Plan (QCP) describing policies, organizational responsibilities, procedures, and the means of ensuring that all items of work are in conformance; the processes, procedures and details of reviews and checks that will be performed on the design of the project components, and the inspections and tests that will be performed on construction materials and workmanship to ensure the overall quality of the constructed project. For this Design-Build Project, and as part of our design build process, Philip Hawkins, AIA LEED Design Manager will provide design coordination and leadership for the design team, comprised of qualified in-house architect and drafters, and Major Subcontracting Partner engineersand subcontractors as well asother support personnel necessary to fulfill the planning / investigative actions and preliminary and final designs to ensure that the civil/structural/lighting/lightening protection/electrical/IT and mechanical systems are all completed in conformance with appliciable codes/regulations and contract requirements. At Team R&R, the Architect communicates with subcontractors from the project beginning, a rarely encountered project approach but nevertheless one that Team R&R partner PHA Design Build has successfully put into practice on multiple Department of the Air Force projects at Beale AFB. PHA will prepare the initial key design submittals including cementitous shiplap or faux Rigid Rock schematic design submittals, which will be reviewed by the Project Manager and QC Manager and send to USFS for approval. The approved schematic designs will be released to Rigid Global Buildings to provide structural engineering design and caluclations per specifications.Simeoultaneously, PHA will begin preparing final architectural design and engineering coordination for the building interior improvements and site feature development. All design submittals will be reviewed by PHA staff and directly supervised by The Design Manager, Philip Hawkins. All design submittals will also be reviewed by the Project Manager, QC Manager Estimator, and Key Subcontracting Partners for compliance with initial contract cost guarantees. This process will ensure theproject will meet schedule, cost, and quality results for the Forest Service as the project develops through the design phases. This process also will avoid the need redrawing and redesign delays at the 100% submittal phase, when costs are often discovered to be out of line. Construction submittals will be principally performed under the direction of the Project Manager. However, the Design Manager will review all key construction submittals for compliance with the original design documents.Construction submittals will be prepared utilizing daily QC Reports and other necessary records as comprised by the QC Manager for a scheduled submittal. Each submittal will be reviewed and signed by the

Project Manager. The QC Manager will document each submittal through a signed submittal register which will be maintained electronically, and in the field office. The Design Manager (PHA), working in tandem with the ProjectManager, and as a Teaming Partner holds greater responsibility and authority in ensuring a higher quality of project outcome. PHA, also having the benefit of being a general contractor, understands the means and methods of construction and has a stronger knowledge of the building process than many architects, having built many of its own projects. The full QCP will describe and define participant roles; quality review processes, responsibilities and activities; quality requirements for design/construction integration; the specific quality measures and application instructions; and necessary quality assurance (QA) documentation and verifications to which technical reports, project documents, design drawings, engineering calculations and construction documentation must comply. The QCP will identify the design quality requirements, including: 1. Certification that the design satisfies the requirements of the ContractStatememt of Work and specifications. 2. Accuracy, adequacy, conformance to standards of practice, compliance with codes and standards, cost effectiveness, quality, and fitness for purpose and functionality to construct a fully operational Helibase for USFS. Team R&R partners have all developed a strongreputation for consistent quality and areexperienced in the three (3) Phases of Contractor Quality Control Reviews for Construction (Preparatory, Initial, and Follow-Up) as well ashaving successfully met the requirements and controls required for Quality Management Plans of the US Army Corps of Engineers Design-Build Projects. Team R&R Manages all work with the USACE/NAVFAC 3 Phase Process, Project Controls, Standardized Forms, Best Management Practices. The QC Manager will perform the Three-Phase Inspection Program. The three phases of the Inspection Program adequately cover both on-site and off-site work and include the following for each definable feature of work (a definable feature of work is a task which is separate and distinct from every other task and requires separate inspection requirements). The Forest Service and Team R&R will agree on the definable work features for which preparatory meetings are required; a memo of understanding will be signed by both parties. The preparatory meeting will define any special quality control items that are not addressed in the QC Plan. These changes shall be submitted in writing to the Contracting Officer. No changes will be implemented before they are approved by the Contracting Officer. 1) Preparatory Phase:The Contracting Officer will be notified at least 24 hours in advance of each preparatory phase; the preparatory phase will be conducted with the Superintendent and/or QC Manager, Design Manager, and the Project Manager responsible for the definable feature of work. Results will be documented in the Daily Contract Quality Control Production Report. 2) Initial Phase:The Contracting Officer is notified at least 24 hours in advance of each initial phase. When the construction crew is ready to start work on a definable feature of work, the initial phase for that definable feature of work is conducted. The initial segment of the definable feature of work is observed to ensure that the work complies with contract and/or design requirements. The results of the initial phase are documented in the Daily Contract Quality Control Production Report. 3) Follow-Up Phase:Follow-up actions are performed for on-going work on a weekly basis or until the completion of each definable feature of work. Actions will be documented in the Daily Contract Quality Control Production Report.

The Team is responsible for project success Team R&R and the Major Subcontracting Partners who compromise the entire Construction Team are responsible and accountable for delivering a quality project to USFS. The General Superintendent is empowered and supported by the Project Manager to make project decisions within the bounds of the approved Project Management Plan (PMP). Team R&Rs vast resources ensure the Project Manager can provide that the Team has the resources, tools, skills and experience needed to deliver a quality project to USFS. Everyone is responsible for Safety. Team R&R is committed to Safety First, and the Safety Manager is empowered with the authority and responsibility to ensure that the Safety Program for this Project as well as the Injury Illness Prevention Plan for are followed, as well as all other Federal and State Safety Requirements are met such as OSHA, Cal OSHA, following the more stringent, and inconformance with the requirements of the Forest Service.
