This standard sets out requirements for the following; a) The design and use of safety signs intended for use in the occupational environment to regulate and control safety related behaviour, to warn of hazards and to provide emergency information. a) The control of access to dangerous or high risk areas or equipment through the use of barricades. b) The use of certain colours for the marking of physical hazards and for the identification of certain equipment in industry.

1.2

Scope
Where a workplace hazard has been identified and cannot be eliminated, signage, barricading and demarcation appropriate to the hazard will be installed. This standard applies to all employees, contractors and visitors to Queensland Nickel and associated facilities, including borefields, bulk fuel facility and raw materials handling facility at Townsville Port.

2.

ACCOUNTABILITIES
All Employees o Learn the meaning of and comply with the requirements of all safety signs o Report to their manager or supervisor all missing, deteriorated, obscured or obsolete signs o Understand plant demarcation colour codes and use as per YHSS061b. o Erect barricades to control access to areas in which a hazard has been identified o NEVER ENTER A BARRICADED AREA WITHOUT AUTHORISATION. Area Superintendent/Responsible Supervisor o Ensure that safety signs are erected as required and are kept in good order and condition o Ensure that the type of sign used is suitable for the intended purpose o Ensure that all employees understand safety signs and observe them o Ensure barricades are erected and maintained to prevent unauthorised or inadvertent access into areas of high risk o Ensure the barricade type are appropriate to the level of protection required o Ensure that barricades are removed when works have been completed o Know demarcation codes and ensure relevant equipment/areas are demarcated as specified in this standard o Ensure demarcated lines and equipment are maintained and always clearly visible.

DEFINITIONS

Barricade

Bunting / Flagging Gateway Demarcation

Includes all forms of permanent and temporary structures, devices and equipment designed to prevent access to restricted areas or protect personnel from hazardous processes, falling objects, equipment, traffic hazards and high risk areas Nylon rope with high visibility orange flags used to form temporary barricading A designated barricade entry point for controlling access. The marking of physical hazards and equipment used in connection with accident prevention in accordance with a specified colour code. High visibility plastic tape, usually with yellow and black diagonal stripes, used to highlight the existence of a physical barricade or hazard.

4. 4.1

PROCESS General Requirements

Where a workplace hazard has been identified, signage, barricading and demarcation appropriate to the hazard will be installed.

4.1.1

Signage
Personnel should know the meaning of symbolic, safety and road signs, and take appropriate actions as required. Signs should be located where the messages are legible, and so that they attract the attention of, and are clearly visible to all concerned. They should be mounted as close as practicable to the observers line of sight and positioned so as to give the viewer ample time to heed the warning. Signs shall be constructed and erected so that they dont create a hazard. For maximum effectiveness, signs should be maintained in good condition, kept clean and well illuminated.

4.1.2

Barricading
Barricades will be erected and maintained where there is risk of a person falling, being struck by falling objects, or where there is risk of injury from equipment, processes or area hazards. The use of barricades to control access to an identified hazard or hazardous area is the responsibility of all site personnel. Barricades will be erected to separate the affected area either by integration with existing structures or stand-alone installation. Barricading tags must be attached in sufficient numbers to ensure visibility under all circumstances. Barricade tags/information signs shall provide the contact person/responsible supervisors name and phone number in addition to the expected duration of the barricade. Where appropriate, they should also have attached specific hazard signs i.e. Danger no access-persons working above.

4.1.3 Work Area and Equipment Demarcation

Specific colours will be used for the marking of physical hazards and equipment used in connection with accident prevention, as per Plant demarcation Colour Codes YHSS061b and Australian Standard AS 13181985 (SAA Industrial Safety Colour Code). Safety colours should be applied so as to be clearly visible in all circumstances, and generally on a limited scale, so as not to interfere with colour schemes in which colours are applied to relatively large surfaces.

4.2.1

Selecting Hazard Controls

Notices and Signs
Safety signs are classified and shall be used according to their function. See Appendix 1 YHSS061a Example Symbolic Sign Types. All signs, including road signs, will be designed in accordance with AS1319-1994 Safety Signs for the Occupational Environment. Symbolic signs will be used in preference to word only or combined word-symbol signs. Written messages will be concise, clear and in the same colour combination as symbolic signs. Wherever practical, multiple individual signs will be accumulated at prominent locations in the form of a single board, with like symbols grouped at safe entry routes of each area.

4.2.2

Barricading
Barricades shall be constructed of an approved type (see Appendix 3) and will be used to provide a visual barrier; to restrict access to unauthorized persons to worksites and restricted areas; to contain equipment and materials; to prevent interaction with an identified hazard; The type, design and construction of all barricading will follow a risk management approach. This should include an assessment of the nature and of the hazard, the likelihood and consequence of interaction as well as the potential impact to workers and the work environment as a result of its placement i.e. the blocking of safe entry routes. Barricades should be installed in such a way as to eliminate the possibility of accidental entry into barricade zone. The person erecting the barricade is responsible for attaching amber-flashing lights at intervals, where the risk assessment indicates the need to warn people of the presence of a barricade during darkness. This ensures that the barricade is visible from the road or access ways. 4.2.2.1 Soft Barricading Soft barricading will be used to prevent entry of personnel and equipment as an immediate and short-term control. This barricade type will be used where risk assessment indicates that the associated risk is low. Soft barricades may be constructed of Scissor/Expanding Barricade

Chain Flag type bunting Scaffold tubing Plastic mesh barriers 900 mm high supported by capped star pickets or other upright structures at maximum spacing of 3 metres or Hazard tape can be used to highlight the presence of a hazard or barricade but shall not be used as barricading by anyone other than the ERC during an emergency situation. Note: Where star pickets are to be used to support barricading an excavation permit must first be obtained. 4.2.2.2 Hard Barricading Hard barricading will be used to prevent entry of personnel and equipment to areas where a risk assessment indicates that physical protection of hard barricading is required for a short to medium period. The purpose of solid barricades is to provide a physical barrier to entry. Hard barricades may be constructed of Construction Site Barricade i.e. mesh or hoarding fence panels Road Traffic Control barricade i.e. water filled plastic barricades Free Standing Barricade Where solid barricades are used they shall be erected by a competent person and accompanied with signs to communicate the hazard information and the contact details of the responsible supervisor. Designated entry and exit points or gateways can be included in the barricade design where entry to the controlled area is required.

4.2.3

Colour Codes
To standardise demarcation codes for equipment, Safe Entry Routes and work areas across site the following colour convention shall be followed as per attachment YHSS061b, Plant Demarcation Colour Codes. Sunflower Yellow: Used alone or with black to indicate caution is required. Examples include; Barricade markings and temporary barricades Machine and traffic guards Fixed ladders and cages Sunflower Yellow with black stripes: Hazard identification, for best attention in difficult environments. Examples include; Low head room, beams and low pipes etc. Pillars, posts or columns that might be struck Road speed humps Signal Red: Used to identify or indicate danger, fire protection equipment and emergency stop controls. Examples include; Stop buttons and similar applications Flashing red lights at rail crossings Fire pumps Signal Red with White 450 Stripes: Fire equipment wall markings

4.3

Maintenance
The responsible supervisor or area superintendent must ensure that all signage, barricading and/or demarcation under their responsibility are kept in a condition that doesnt hinder their effectiveness. Safety signs should remain; Legible (day & night if appropriate) Correctly attached Correctly positioned Contain accurate information Barricading should be; Type appropriate as per risk assessment, Signed appropriately (see above) Effective at preventing accidental contact Visible (day and night if appropriate) Removed when no longer required Demarcated Areas should: Adhere to the plant demarcation colour code Remain visible Be correctly positioned

5.

TRAINING
All employees and persons working on the Queensland Nickel will complete the site induction that contains information on barricades, Signs and work area demarcation.
