MEDICAID (MKD) TRANSPORTATION LIST Updated 1/31/14

If you do not have insurance and you need a ride to your MD appt. you must contact your social worker. They can coordinate Red Cross Transportation. Red Cross must have 3-4 weeks notice.
If you have MHS hoosier healthwise, please call (877)647-4848 to schedule your ride. If you have MDwise hoosier healthwise, please call (317)630-2831 to schedule your ride.

If you have Traditional Medicaid, please see this list below to select your transportation provider: 1. 3 Cs transportation (317)455-1709 Katherine a. Yes they are accepting new patients b. Mileage distance if trip is over 50 miles they will need a pre-authorization c. If the trip is long distance please request transportation at least 4 days prior to the appointment. If its a local appointment, she requests you call at least 2 days prior to the appointment. d. Handicap accessible e. No pick up after 3:30 or 4 pm most days g. Same day appointments are difficult, but will consider on a case by case basis h. Hours: 6 am- 3:30 pm
