CrossroadsFootballLeague Waiver,ReleaseofLiability,Indemnification, ConsenttoMedicalAttention,andStatementofPhysicalCondition Inexchangeformybeingallowedtoparticipateasamemberofthe(team),heretoforeknownastheteama nonprofitorganizationundertheLawsoftheStateofIndiana,Michigan,orOhio,Iagreetobeboundbyeachofthefollowing:

1. Definitions: a. ForthepurposesoftheWaiverandReleaseofLiabilityFootballPracticesandGamesshallbeconstruedtomeanall eventswhicharetooccurasadirectresultofmyparticipationwiththeteam,toinclude,butnotlimitedto,practices,games, mediaevents,classes,camps,andthelike. b. ForthepurposesofthisWaiverandReleaseofLiabilitycoachesshallbeconstruedtomeantheheadcoach,theoffensive coordinator,defensivecoordinator,and/oranyoneelseinvolvedwiththeteaminsuchacapacity. 2. Voluntaryparticipation.Iunderstandandconfirmthatmyparticipationwiththeteamisvoluntary.Iunderstandthatalthoughmy coacheswillendeavortoplayeveryplayerineachgamefortheenjoymentofallmyteammates,butthatthefinaldecisionstoplayme inanygamewhatsoeverarethedecisionsofthecoachesandonlythecoaches.Myplayerfeeandparticipationinpracticedoesnot guaranteeplayingtimeinanygame. 3. IdentificationandAssumptionofRisks.IunderstandthatseriousaccidentsmayoccurduringFootballPracticesandGamesand thatparticipantsinFootballPracticesandGamesoccasionallysustainmortalorseriouspersonalinjuries,aswellaspropertydamage, asaconsequencethereof.IunderstandtheserisksandknowthatmyparticipationinFootballPracticesandGamesmayinvolveriskof suchpersonalinjuriesandpropertydamage,including,permanentdisabilityanddeath.IunderstandthatthisWaiverandReleaseof LiabilityisintendedtoaddressalloftherisksofanykindassociatedwithmyparticipationinanyaspectoftheFootballPracticesand Games,including,particularly,suchriskscreatedbyactions,inaction,carelessness,ornegligenceonthepartoftheTeamorits directors,officers,employees,agents,volunteers,successors,orassigns.Iassumeallrisks,knownandunknown,foreseeableand unforeseeable,inanywayconnectedwithmyparticipationinFootballPracticesandGames.Iacceptpersonalresponsibilityforany liability,injury,loss,ordamageinanywayconnectedwithmyparticipationinthefootballpracticesandgames. 4. ReleaseandWaiver.IreleasetheCrossRoadsFootballLeague,thisteamanditsdirectors,officers,employees,agents,volunteers, successors,andassignsfromanyandallliabilityforandwaiveanyandallclaimsforinjury,loss,damage,orexpense,including attorneysfees,inanywayconnectedwithmyparticipationintheFootballPracticesandGames(aClaim),whetherornotcaused inwholeorpartbythenegligenceorothermisconductoftheteamoranyoftheindividualsmentionedabove. 5. Indemnification.Iagreetoindemnifyandtoholdharmless(inotherwords,toreimburseandtoberesponsiblefor)theTeamand theirdirectors,officers,employees,agents,volunteers,successors,andassignsfromallClaims(includingthecostofdefendingany ClaimImightmake,orthatmightbemadeonmybehalf,thatisreleasedorwaivedbythisinstrument)inanywayconnectedwithor arisingoutofmyparticipationintheFootballPracticesandGames,whetherornotcausedinwholeorinpartbythenegligenceor othermisconductoftheteamoranyoftheindividualsmentionedabove. 6. BindingEffect.Thisinstrumentshallbebindinguponmyrelatives,personalrepresentatives,heirs,beneficiaries,nextofkin,or assignsandshallinuretothebenefitoftheTeamandtheirsuccessorsandassigns. 7. ConsenttoMedicalTreatment.IauthorizetheTeamtoprovidetome,throughmedicalpersonneloftheirchoice,customary medicalassistance,transportation,andemergencymedicalservicesshouldIrequiresuchassistance,transportation,orservicesasa resultofinjuryordamagerelatedtomyparticipationintheTeam.ThisconsentdoesnotimposeadutyupontheTeamtoprovide suchassistance,transportation,orservices. 8. StatementofPhysicalConditionandCompliance.IaminproperphysicalconditiontoparticipateintheFootballPracticesand Games.IhavereadandwillcomplywiththetrainingguidelinesandthemedicalrulesandpolicieswithrespecttotheFootball PracticesandGamesthathavebeensuppliedtomebytheTeam. 9. TeamProperty.IunderstandthatIamfullyresponsibleforallTeampropertywhenitisinmypossession.IunderstandthatIamto returnallTeampropertyimmediatelyafteritsuse,unless,IhavetheverifiedconsentofaTeamDirector,Officer,orCoachandthatall TeampropertymustbereturnednolaterthanNovember1ofthisyear.IfIlose,damage,orcauseanexpensetotheTeamproperty,I understandthatTeamshallbepermittedtopursueallcollection,relief,includingattorneysfeesandcourtcosts. 10. ApplicableLaw.Thisinstrumentshallbegoverned,construed,andenforcedinaccordancewiththelawsoftheStateofIndianaor Ohio.
