Survey Monkey Guidelines

Revised September 2010 This is a benefit of ANCHASL membership. The ANCHASL subscription to Survey Monkey may only be used by the ANCHASL member. You should not ive the pass!ord to anyone else" not even your staff members. #f you !ant your entire library or institution to use it" you !ill need to buy your o!n subscription or use the limited free version. This should primarily be used for library purposes !ithin your institution. #f your institution !ants you to do a survey for the institution that has nothin to do !ith the library" they should be encoura ed to purchase their o!n subscription. ANCHASL members may also use it for their o!n personal non$!ork related surveys. %lease do not look at others& surveys !ithout first askin permission. 'emember that your survey is in the list of surveys" and it is technically possible for others to look at it. So if you !ant to make absolutely sure that your survey is confidential" you mi ht !ant to consider buyin your o!n subscription. Al!ays put your surveys into a folder that identifies it as your survey in some manner. #nstructions follo!. (nce a year a call !ill o out for everyone to clean up the survey list. #f the survey is not in a folder and has not been used !ithin the previous )* months" it !ill be deleted. You !ill receive notification of lo in information and pass!ord after you pay your annual dues" via email from our Membership Chair +see http,--!!!.anchasl.or -committees.shtml.. /or additional information about Survey Monkey" contact the SurveyMonkey %ro0ect Mana er listed on the Committee pa e above. The pass!ord !ill be chan ed every 1anuary ) to coincide !ith membership rene!al and e2piration.

Caveat:
Our license allows for up to 1000 responses per mont ! totaled from all open surveys" ). #f you e2pect to et more than 344 responses from your survey" in a sin le calendar month" you should purchase your o!n subscription. Subscriptions can be purchased on a monthly basis for 5)6.63-month. *. #f you do a survey and et more than )"444 responses" you !ill be billed for everythin over )"444" at the same rate that SurveyMonkey char es" 5.43 per survey response. 7. #f you e2pect to do a survey that !ill et many responses" please send a messa e to the ANCHASL$ L email list" lettin others kno!" so that there !ill not be t!o lar e surveys oin at the same time. 8. #f you e2pect to do a survey that !ill et many responses" start it in the middle of the month and end it in the middle of the follo!in month. That !ill break up the responses into t!o months" rather than one. 3. As a contribution to the profession and to ANCHASL" please consider postin your e2periences !ith SurveyMonkey to the ANCHASL email list. 9veryone !ill be interested in !hat !orks" !hat doesn:t" tips and tricks" and this kind of information.

There is a feature in SurveyMonkey that enables you to make a folder in !hich to put your surveys. This makes it much easier for you to roup all your surveys to ether so that you don&t have to bro!se throu h all the surveys to find yours. #t also identifies !ho o!ns the survey" so that there is someone to contact if you have >uestions about a survey. Click on @Create folder@" put in a folder name that can identify you as the o!ner of the survey +in case someone !ants to contact you about it.. Click the @Add A@ button. No! click on @Mana e /olders@ a ain and o throu h the list of surveys and usin the drop$do!n menu" select your folder name for all your surveys. Then click on <pdate. 8. The ne2t time you o into SurveyMonkey to !ork on a survey" all you have to do is click on the drop$do!n menu on the @Current /older@ bo2 to find your folder" select that" and only your surveys !ill be listed.
