1. Start with biographical summary of yourself (e.g., origins, family background, education, training, etc.) I was born this way. I am Ordequito Prudente Lumactod, Jr, and I proudly live the life I wanted. On the eve of 29 th of June 1989, my parents were very happy when they gave birth to their 4 th child, and that was me. My parents are Lydia Doseo Prudente from Iloilo City and Ordequito Sedayon Lumactod, Sr. who lived in Libertad, Escalante with his origins. We were 6 in the family and I have 3 siblings, being the youngest among siblings, I am much loved by my parents. Weve been raised by our parents with good education and values as inculcated in our hearts and minds. Presently, I am the only single sibling and with me are my parents and my niece. At 6, I went to Dian-ay Elementary School and eventually graduated in 2001 as 1 st Hon. Mention. I completed my secondary education at Dian-ay National High School in 2005 as Class Salutatorian. I was determined to pursue my tertiary education and I applied for a scholarship program given by the local government of Escalante City for four years. I enrolled at Mount Carmel College, Escalante City and took Bachelor in Secondary Education Major in Mathematics. On my 2 nd year in college, I changed my course to BEED, General Education. I successfully graduated in 2009. Because of perseverance and dedication to work, I am now a Teacher 1 of Binaguiohan Elemetary School, Division of Escalante City. In our school, I am the Grade VI-1 Adviser handling Science Subject in Grades 4-6. I am also the SPG Adviser, Science School Coordinator, School Testing Coordinator, and School Paper Adviser. Recently Ive been recognized in the Div isionfor obtaining 82.56% MPS in Science VI in NAT 2013. Currently studying at NONESCOST taking up MAED and preparing for my Comprehensive Exam. I am happy and looking forward to be a successful teacher or school administrator in the future.

9. Briefly discuss a model or exemplary individual whom you will edify in becoming a SUCCESSFUL HEROIC SERVANT LEADER (School Administrator). Some people are born more naturally to leadership than others. Most people don't seek to be a leader, but many more people are able to lead, in one way or another and in one situation or another, than they realize. People who want to be a leader can develop leadership ability. Leadership is not the exclusive preserve of the wealthy and educated. Leadership is a matter of personal conviction and believing strongly in a cause or aim, whatever it is. Leadership sometimes comes to people later in life, and this is no bad thing. Humanity tends to be generational characteristic. There is no real obstacle to people who seek to become leaders if leadership is approached with proper integrity. Anyone can be a leader if he/she is suitably driven to a particular cause. And many qualities of effective leadership, like confidence and charisma, continue to grow from experience in the leadership role. Even initially surprised modest leaders can become great ones, and sometimes the greatest ones. If there is one person whom I wanted to follow her steps, it would be our Principal, Mrs. Annie R. Tuto. She possessed the qualities I mentioned above.
