First and foremost, the researchers would like to pay gratitude to God, who gave

the researchers the knowledge and brilliance, the will and strength, the talents
and skills which he bestowed upon the researchers. Next, the researchers would
like to show their appreciation to their ever kind and wonderful professor who had
given them the chance to experience what community health nursing is and guided
the researchers throughout the program. Those times which Mr. Lazarte spent to
them were priceless and indeed fun which will marked to the minds of the
researchers. The researchers would also like to be grateful to the personas in
the locale where the researchers carried out their programs. The researchers would
like to thank Kagawad Jerry P. Matabang and Chairman Dick Wilson which must be
recognize, for not for them this program will not be a success. They are the one
who assisted the researchers and provided them with all the pre-requisites they
need. During the researchers’ nutrition program, the researchers are in debt to
the wonderful and hospitable teacher Mrs. Grace F. Domalaon and her assistants for
allowing the researchers to interact with their students. Last but not the least,
this study would not reach its pinnacle if it has not been for their respondents,
the people of Chacon St. Tondo who willingly and invitingly accepted them. The

COMMUNITY NURSING
Many challenges influence the overall health of communities. Infectious diseases,
such as tuberculosis, can spiral out of control in many urban settings. Children's
health and well-being are still threatened by preventable diseases, environmental
toxins, violence, accidents, and injuries. Unequal access to health care and
insurance coverage has notably increased morbidity and mortality among infants,
pregnant women, and other vulnerable populations. Amid these challenges, all
nurses can improve health outcomes and expand the infrastructure for monitoring
and managing diseases. In traditional public health settings, community health
nurses work to ensure that expectant and new mothers have the resources to
appropriately care for themselves and their children. Community health nurses are
valued for their adaptability and willingness to provide care in many settings,
including community health clinics, churches, homeless shelters, and schools.
These nurses provide comprehensive care to patients within their homes, at
organized events such as health fairs, and at agencies and institutions serving
people who have particular health needs. Advanced practice nurses in communities
improve access to care and lower costs at nurse-managed clinics on college
campuses and at primary and secondary schools. They develop and implement
corporate wellness programs, thereby supporting the health and productivity of
employees and their organizations.

COMMUNITY NURSING
Nurses in community health work with diverse partners and providers to address
complex challenges in the community. Nowhere is this more evident than in current
efforts to identify, reach, and treat people and in efforts to help the elderly
effectively manage their chronic health problems and remain at home. Community or
public health nurses may specialize in areas such as home care; case management;
clinical, school, or corporate nursing. Community health nurses with advanced
degrees can also find opportunities in higher education and clinical research. The
skills needed for these diverse areas can vary, but at a minimum, nurses should
have a bachelor's degree and sound clinical experience. Some public health
settings have relaxed their minimum requirements in response to the nursing
shortage. Nurses who choose traditional public health or home care should have a
broad understanding of health issues and be comfortable with autonomy, change, and
uncertainty. Nurses entering this specialty must highlight not only their clinical
skills, but also their critical thinking, advocacy, and analytical abilities.
Communities are dynamic, and nurses must adapt to be able to provide patients with
whatever care they need.

health in our subjected area. Be it, change, recreation, development, as long as
we can withstand and solve the problems that might occur. And our goal must be
specific, measurable, attainable, realistic and time bounded.

It was July 7, 2009, Tuesday, when we started our first duty on our community
health nursing together with our clinical instructor Mr. Francis Carlos Lazarde in
Lorenzo Chacon St. Tondo , Manila. We aknowledged firsr the Brg. Chairman, Mr.
Dick Wilson about the interview we are about to conduct. We was so kind that he
even let one of his kagawads to go with us. It was a small community with almost
60 families living. At first we were a bit scared because their community was
crowded. We conducted an interview to some of the families and assessed their
lifestyle. We discovered many things such as there is one spot in their community
in where most of the families shares with others in order for them to take a bath,
and wash their dishes as well as their clothes. We also had free blood pressure
check up for those who are interested. On the second day, July 8, 2009, Wednesday,
we conducted a little program for the families in the community about sanitation
and proper hygiene, drugs and it’s effects, and hypertension. We even taught them
how to use the blood pressure apparatus, how to check the temperature using a
thermometer, and how to check the respiratory and pulse rate. We were so happy
because many have

On the third day, July 13, 2009, a rainy Monday, we went back to the Barangay hall
to conduct a Nutrition Program for the children of their day care center but
unfortunately there were less students because of the rainy season that’s why we
decided to conduct the program on the next day. We went back to the community and
gathered some more information to those families we haven’t interviewd yet. Even
though it was a rainy day, it was fun because the families accepted us on a nice
way. Then the fourth day cam, July 14, 2009, it was our last day. We prepared for
our nutrition program in their day care center. We asked the teacher if we could
lend some time for us to be able to teach few things for the children. We had a
little play which all the kids enjoyed. We even tesch them about the Go, Grow,
Glow foods. We again went back to the community to give some donation such as
unused clothes and old shoes to the families as a sign of appreciation for their
kindness. Even though we only had a few time to spend with them, we were so
grateful to have an experience just like that because we get to discover and learn
