JOINT AFFIDAVIT OF ARREST

We, PO1 Mark Yeen and PO1 Greyson Purisima both of legal ages, bonafide members of the Philippine
National Police and presently assigned at Police Station 1, stationed / based at Sto Rosario St. Angeles City
after having been duly sworn to in accordance with law, do hereby depose and state:
1. That on March 10, 2013 at about 1530H our Office received information from Raewel Tuazon of
Brgy. San Jose Sn. Fabian St. Angeles City that there was a shooting incident committed at the
corner of 711 Sto. Rosario St. Angeles City;
2. That, acting on the said information, our Captain Joel Bagoisan directed us to respond/proceed at
the reported place of incident to verify the veracity of the said information and if affirmative, to
conduct an ocular investigation;
3. That, upon arrival at the place of the incident, we were personally informed by the
witnesses/bystanders that victim one Aaron Samadan was already rushed to the AUF Medical
Center for medical treatment ; and that, we were likewise informed that the suspect escaped after
the incident with the firearm he used in the commission of a crime;
4. That, I PO1 Mark Yeen immediately informed our Office/Unit confirming this incident; thereafter,
we, together with one Mark Carpio who witnessed the incident and volunteered to help us ,
proceeded at the residence/house of the suspect at Villa Julia Subdivision City of San Fernando
Pampanga; and that upon arrival there at an about of the same date, our witness positively identified
the suspect to us who was then walking in front of the gate of his residence/house.
5. That, I PO1 Greyson Purisima immediately identified ourselves to him (suspect) as police officers
and members of Miranda Doctrine and arrested the same; and that , I likewise informed him the
reason of his arrest and of his constitutional rights in a dialect known to and understood by him
such as: You have the right to remain silent and anything you say will be used against you in court
You have the right to consult with an attorney and to have that attorney present during questioning
and if you are indigent, an attorney will be provided at no cost to represent you.However during
his arrest , the firearm he used was no longer in his possession and the same cannot be ascertained /
located;

6. That, he (Aaron Samadan) submitted himself to us peacefully; and further identified himself of
Villa Julia Subdivision City of San Fernando Pampanga and that, we immediately brought him to
our Office/Unit for medical examination, investigation and proper disposition; and
7. That, we executed this affidavit/ statement to attest the veracity of theaforementioned facts and to
support the filing of appropriate charge /s in court against the arrested suspect.
