Application for grant of Learners Licence (Form 2)


Self declaration as to the physical fitness (Form 1)

Age Proof (Ex.: Birth Certificate, SSLC)

Note: Every applicant for a learners licence shall present himself before the
licensing authority on such date, place and time.
Preliminary Test:
Knowledge Required for the applicant:
1. He/she must know the traffic signs, traffic signals and the rules of the road
regulations.
2. He/she must know the documents to carry with him/her while driving a
motor vehicle.
3. He/she must know the duties of a driver when his vehicle is involved in an
accident.
4. He/she must know the precautions to be taken while passing an unmanned
railway crossing.
Note: The applicant must possess "adequate knowledge" means answering
correctly at least 60% of the questions put to him/her.
Outcome: A Learners Licence issued by the Licensing
Authority will be in Form 3.

For Driving Licence:

(Fees: Rs. 250/- only)

Rs. 200/- (for Licence Card) + Rs. 50/- (for Driving Test)
Documents Required:
1. Application for Driving Licence (Form 4) / For International Driving
Licence (Form 4-A)
2. Learners Licence issued by the authority (Form 3)
3. Medical Certificate, if necessary (Form 1-A)
4. Passport size photograph (3 Copies)
5. A driving certificate in issued by the school, if any (Form 5) Mandatory
for transport vehicles only
6. Address Proof (Ex.: Ration Card, Voter ID, LIC Policy)
7. Age Proof (Ex.: Birth Certificate, SSLC)
Note 1: Application to Driving Licence shall be made only after a minimum
of 30 days of issue of Learners Licence.
Note 2: Applicant shall bring a vehicle by himself with all its necessary
documents updated.
For International Driving Licence:

(Fees: Rs. 500/- only)

Documents Required (In addition to the above requirements):


1. Valid proof of Indian Nationals; if valid proof of passport.
2. Valid proof of visa, wherever applicable.

Note 2: If licence is required in book format (Form 6), the fee shall
be Rs. 40/- only.
Outcome: A Driving Licence issued by the Licensing
Authority will be in Form 7 (Laminated card/Smart Card).

For Addition to Driving Licence:

(Fees: Rs. 200/- only)

Documents Required:
1. Application for the addition of a new class of vehicle to a driving licence
(Form 8)
2. Learners Licence issued by the authority (Form 3)
3. Old Driving Licence (Form 6/Form 7)
4. Medical Certificate, if necessary (Form 1-A)
5. A driving certificate in issued by the school, if any (Form 5) Mandatory
for transport vehicles only
Note 1: Application to addition to Driving Licence shall be made only after a
minimum of 30 days of issue of Learners Licence.
Note 2: Applicant shall bring a vehicle by himself with all its necessary
documents updated.
For Renewal of Driving Licence:

(Fees: Rs. 250/- only)

Documents Required:
1. Application for the renewal of driving licence (Form 9)
2. Old Driving Licence (Form 6/Form 7)
3. Medical Certificate, if necessary (Form 1-A)
For Duplicate of Driving Licence:

(Fees: Rs. 250/- only)

The Duplicate licence shall be issued within a maximum period of 60 days from
date of application.

Outcome: A Driving Licence issued by the Licensing


Authority will be in Form 7 (Laminated card/Smart Card).

For Registration of new motor vehicle: (Fees: Rs. 60/- only)


Rs. 60/- (for motor cycle) or Rs. 200/- (for imported motor cycle) or Rs.
200/- (for non-transport LMV) Rs. 300/- (for LCV) Rs. 400/- (for medium
goods/passenger vehicle)
Documents Required:
1. Application for registration of a motor vehicle (Form 20)
2. Sale certificate issued by the manufacturer/seller/department of defense in
case of Ex-army vehicle (Form 21)
3. Address proof (Ex.: Ration Card, Voter ID, LIC Policy)
4. Initial certificate of compliance with pollution standards, safety standards
of components and road-worthiness form the manufacturer (Form 22)
5. Customs clearance certificate - for imported vehicles only
6. Certificate of fitness (Form 38) for transport vehicles only
Note: Application should be made within 7 days from the date of delivery
excluding the period of transport.
The certificate of registration shall be issued within a period of 30 days from the
date of application.
For renewal of RC:

(Fees: Rs. 60/- only)

Rs. 60/- (for motor cycle) or Rs. 200/- (for imported motor cycle) or Rs.
200/- (for non-transport LMV) Rs. 300/- (for LCV) Rs. 400/- (for medium
goods/passenger vehicle)
Documents Required:
1. Application for renewal of certificate of registration of a motor vehicle,
other than a transport vehicle (Form 25)
2. Certification of Pollution under control (PUC)
3. Certificate of fitness (Form 38) for transport vehicles only
Outcome: Certificate of registration issued by the registering
Authority will be in Form 23 / Form 23A (Smart Card).
