That I am familiar with the facts surrounding the birth of my Daughter VANDANA

That I was born on 12th December, 1957 in [HALOL, PANCHAMAHAL, GUJARAT,
INDIA], Age-55, Occupation: Housewife, That I presently reside at [Parthbhumi
2,Block no. 99/989, GIDC, VADSAR ROAD, VADODARA, GUJARAT, INDIA]
Do here by solemnly affirm on oath and declare as under:-

1. I am the deponent herein and a citizen of India.

2. I was married to Mr. PANCHAL PRAVINBHAI MOHANLAL on MAY 5,
1974 in HALOL, PANCHAMAHAL, and GUJARAT, INDIA.
3. As it is customary, I had gone to my parents place at HALOL,
PANCHAMAHAL GUJARAT, INDIA for the delivery of our first child in
December, 1975. That this child was born to me and my husband.
4. VANANDA was born on Dec 16, 1975 in HALOL, PANCHAMAHAL,
GUJARAT, INDIA. I have the personal knowledge of her birth because I
am her blood mother and I gave her birth.
5. VANDANAs full name at the time of birth was given as PANCHAL
VANDANABEN PRAVINKUMAR.
6. VANDANAs birth was registered later because it was not prevalent at
that time to register the birth of children. (DATE OF REGISTRATION:
DEC 25 1975, REGISTRATION NO: 533, AT HALOL NAGAR
PALIKA, PANCHAMAHAL, GUJARAT, INDIA.)
7. As it is customary in our region to add the suffix tag BEN for females
for giving respect purpose, when she was admitted in the school, her
first name was provided as VANDANABEN

mother and I gave his birth.

5. DHYEYs birth was registered later because it was not prevalent at
that time to register the birth of children.
