In the name and by the authority of the State of Texas:
I~ David Biggar, do solemnly swear that t have good reason to believe and do believe that,
Jacob Barry Lewis on or about the 14th day of, January 2015, and before the making and
filing of this complaint, in Bowie Countyt Texas did then and there: intentionally. knowingly, or
recklessly cause bodily injury to Brian Jones by stabbing him in the face with an ink pen, and
the defendant did then and there know that the said Brian Jones was then and there an
employee of LaSalle Corrections. a person who had contracted with government to perform a
service in BiState Jail, to-wit: Correctional Officert and that the said Brian Jones was then
and there performing said service, and the defendant knew that the said Brian Jones was
authorized by government to provide the service.
Against the peace and dignity of the State

Affiant's belief is based upon the following:


On 01 ..14-15 LaSatre Corrections Captain Brian Jones reported that he had been stabbed
in the face by inmate Jacob Barry Lewis (WIM 07-27-92). Captain Jones explained that he
was sitting behind the front desk just prior to a session of jail court when Lewis stabbed him
just below his left eye with an ink pen. Jail footage of the assault and activity prior to and after
the assault was burned to a CD and placed into property as evidence (item #1). The blue ink
pen was also placed into property as evidence (item #2). Captain Jones was wearing a
LaSalle Corrections uniform when he was assaulted.
Written statements were made by several witnesses to include Kim Watson from Bowie
County PR Bonds. In her statement Watson advised Lewis was brought in to the court room

for his magistrate warning at which time she gave him paperwork to fill out to apply for a
Public Defender. When Watson told Lewis to sit down, he began to complain that he needed

to urinate. Watson told him that he would have to wait. Lewis cussed Watson and exited the
courtroom and walked to the desk at zone one. Watson saw Lewis rummaging through the
desk and called Captain Jones who responded with Sergeant Ivan Mendoza and spoke to
Lewis.
lieutenant Guy Wheat stated that Capt. Jones told Lewis that he would be placed in
lockdown after court for his actions.

Jail footage shows Lewis leave the courtroom and then urinate in two cups that he had
obtained from behind the desk in Zone 1. Lewis is then escorted back into the courtroom
where he resumes filling out his paperwork. At 8:07:56 Lewis can be seen as he stands and
approaches the desk where Captain Jones, Kim Watson~ and Lance HaU are seated. Lewis

leaped across the desk and stabbed Captain Jones in the face with the pen. Officer James
Combs then tackted and subdued Lewis at the door as he attempted to flee the court room.
I believe that probable cause exists in this case for the arrest of Jacob Barry Lewis for the
charge of Assault on a Government Contract Employee and I request a warrant be issued for
him.
