COMES NOW, petitioner, EMMANUEL PACQUAIO, by and through the undersigned
counsel, and unto this Honorable Court, respectfully alleges:
1. Petitioner is of legal age, Filipino, married and a resident of Tuguegarao City, Cagayan,
while respondent is impleaded in her capacity as the incumbent Municipal Civil Registrar
of Tuguegarao, Cagayan, with office address

Municipal Hall, Tuguegarao City

where she maybe served with court processes;

2. That the fact of name, EMMANUEL PACQUAIO is duly registered in the Registry
Books of Live birth of the Local Civil Registrars evidenced by his Authenticated
Certificate of Live Birth, issued by the National Statistics Office, photocopy of which
is hereto attached and marked as Annex A, of this petition
3. That as appearing in Annex A, the name EMMANUEL PACQUIAO be changed to
Manny Pacquiao
4. The ground for filing this petition is that his name Manny Pacquiao habitually and
continuously used and he is publicly known in the community with that first name.
5. The following documents in support of his petition:
a) A certified true machine copy of the Registry book containing his name sought to
be changed, herein referred as Annex A;
b) A copy of his BIR Tax ID, Company ID and Drivers License indicating the name
Manny Pacquiao is hereto attached and made an integral part hereof as Annexes
B, C, and D, respectively;
c) A copy of his NBI clearance issued by the National Bureau of Investigation,
Tuguegarao City that He has no criminal record, herein referred as Annex E.

WHEREFORE, after due trial, it is respectfully prayed of this Honorable Court, that an
order be issued:
Directing the Local Civil Registrar of Tuguegarao City to cause to be made the
change of my firt name, as appearing in his Authenticated Certificate of Live Birth
(Annex A) from EMMANUEL PACQUIAO to MANNY PACQUIAO as
petitioned herein, upon payment of the fees required/prescribed by law in the
premises.

I, EMMANUEL PACQUIAO, of legal age, Filipino Citizen and residing at Carig Sur
Tuguegarao City, Cagayan after first having duly sworn to in accordance with law depose and
say:

2. I have caused the preparation of the foregoing complaint
3. The contents therein are true and correct to the best of my personal knowledge
and based on authentic documents.
4. I hereby certify that I have not commenced any action or proceeding involving the
same issue in any tribunal or agency, to the best of my own knowledge, no such
action or proceeding is pending in any other tribunal or agency and should I
thereafter learn that a similar action or proceeding has been filed or is pending in
Court, I will undertake to report such fact within five (5) days therefrom to the
court wherein this Petition and this certification have been filed.
