ADMISSION PROCEDURE
1. Apply at the following Guidance
Offices and accomplish the
Application Form. (HS Guidance
Office: for Junior High School,
Elementary Guidance Office: for
Elementary Applicants)
2. Pay P 150.00 at the cashier
(Window 6 or 7) for the testing fee.
3. Submit Application Form, Receipt,

and all other requirements to the

Guidance Staff.
4. Report to the Guidance Office on
the scheduled date for the Test &
Interview.
5. If qualified, you can enrol on the
scheduled dates. Please bring the
requirements upon enrolment as
stated below.

ADMISSION REQUIREMENTS
ELEMENTARY KINDER: Birth or Baptismal Certificate (Original & Photocopy,
Application Form).
GRADE 1 TO 6: Report Card (Original & Photocopy), Recommendation Letter from
the Class Adviser or Guidance Counselor (for Grades 2 to 6)
JUNIOR HIGH SCHOOL GRADE 7 (NEW)/ TRANSFEREES: Report Card, Application
Form, Recommendation Form (Grades 8 to 10).

REQUIREMENTS UPON ENROLMENT:

Report Card (original) Notice of Application Result (Qualified) Birth Certificate
(NSO copy original) Certificate of Good Moral Character from the Principal
Copy of Govt Permit/Recognition from previous school (case-to-case only).
