EMERGENCY MEDICAL & REPATRIATION ASSISTANCE (BTI)

For Home Country employees (across all SLB businesses)


Home Country employees, from the following countries, are covered
under this Emergency Medical and Repatriation Assistance Policy
when they are required to travel outside of their home country for
business/training purposes.

Zealand, Papua New Guinea, Philippines, South Korea, Taiwan, Thailand,
Vietnam.
Middle East: Bahrain, Egypt, Ethiopia, India, Iran, Iraq, Jordan, Kuwait,
Oman, Qatar, Pakistan, Saudi Arabia, Sudan, Syria, UAE, Yemen.
EUM: Algeria, Austria, Azerbaijan, Belgium, Cyprus, Finland, Germany,
Hungary, Ireland, Israel, Italy, Kazakhstan, Libya, Mauritania, Morocco,

Emergency medical expenses (incl. emergency dental

Medical repatriation to home country
This plan will cover the cost of emergency medical treatment which is
necessary to take whilst on a business trip abroad for up to 180
consecutive days. The policy has a limit of Euro 1,000,000 per
person per year.
Non urgent or mild medical incidents that can be treated on your
return to home country under your local medical plan will not be
reimbursable under this plan.

You are required to call Axa Assistance prior to


receiving any emergency medical treatment to check
that it is covered under the plan.
In an emergency
If an emergency medical situation occurs whilst you are outside of
your home country travelling on Company business you should
immediately call the AXA Assistance Helpline (details above). By
describing your symptoms a medical professional will direct you to
the nearest facility where you can access treatment and they will
advise whether the illness/incident is covered under the Emergency
Assistance plan.
In the case of medical evacuation a responsible SLB Manager should
contact the AXA Assistance helpline to inform of the circumstances.
With the exception of a critical illness/accident where you are
immediately hospitalized, you must first seek approval from AXA
Assistance. In the case of emergency hospitalization, where the
costs are not covered by the government of the country in which you
are taking the treatment, you should ask someone to notify AXA
Assistance within 48 hours of being admitted for treatment.
This plan is a second Euro plan meaning that the financial
commitment of the AXA Group Emergency Medical and Repatriation
Assistance covers only the excess part of the reimbursement of the
medical expenses whilst you are abroad above that which you may
have reimbursed from your local medical insurance and/or the
government healthcare plan in your home. You may be asked to
provide details to AXA Assistance if any such plans exist.

Payment for treatment

HOW TO MAKE A CLAIM FOR PRE-APPROVED

OUTPATIENT
TREATMENT:

In the case of hospitalization, AXA Assistance will make payment


directly with the facility. AXA Assistance will then contact you to
determine whether you have any other medical insurance (local plan,
private insurance, government plan) which may cover part or all of
the
costs.

plan,
AXA Assistance will pay the amount above that reimbursed locally. If
there is no other plan AXA Assistance will pay the full medical cost.
In the case of pre-approved
approved outpatient medical costs, these should
be paid by you to the facility. If you have any other medical insurance
you should claim first from that plan. Claims for part or full
reimbursement should be sent to AXA Assistance

Where Axa has settled costs directly with the medical facility you
need to complete the Axa BTI Validation form and send it to your
Personnel Manager to sign also. It should then be returned to Axa
(address and fax number on the form).
To claim reimbursement

need to complete the Axa Claim Form and the Axa BTI Validation
form (also signed by your Personnel Manager). Send both forms to:
AXA Assistance, Service Gestion, 6, rue Andr Gide, 92321 Chtillon
Cedex, France.
These forms are available via your Personnel Manager.

Principle exclusions
AXA Assistance cannot replace local emergency services and will
not take over the expenses incurred by them.
The following claims are not covered:
Accidents intentionally caused or provoked by the insured
The consequences of suicide of the insured
The taking of drugs, consequences of alcohol (including
driving under the influence of alcohol) or related issues
Accidents as a result of practicing sport as a professional
or dangerous sport (e.g. aerial,, fighting etc)
Any medical expenses incurred in the insureds country of
employment
Expenses incurred by the insured without prior agreement,
except for proved cases of force majeure
