Introduction
Bushehr Petrochemical Company of IRAN wishes to develop Utility Units and Offsit
e/Offplot Facilities for production and supply of required utilities for whole c
omplex. The PLANT is located at Pars Special Economic Zone in Assaluyeh, Phase I
L
2.
Purpose
2.1
GENERAL
Nardis as basic designer of utility units in both sites 2&3 of Bushehr Petrochem
ical Complex is going to evaluate and critic safety issues of his basic design b
y a third party. The third party is expected to submit their proposal for perfor
ming HAZOP, SIL, HAZID, QRA, RAM and Consequence Analysis for the attached P&IDs
(ATTACHMENT 5).
It is expected that reviewing of P&IDs and its overall objective conducting to g
enerate recommendations aimed at
Improving safety
Reducing the environmental impact
Optimizing the design and the operability.
Complete report of each study shall be prepared and issue by the third party.
2.2
HAZOP
In the HAZOP Study normal operation systems are reviewed to ensure that the syst
em is safe and then the technique is used to assess whether deviations from the
design intention could occur and, if so, whether such deviations are likely to b
e hazardous.
So the bidders shall propose their proposal for performing a brain storming HAZO
P study for the scope in which is defined in following paragraph.

HAZOP STUDY DESCRIPTION

HAZOP Study shall observe all aspects of safety, health and environmental (I-ISE
) issues in relation to the UTILITY UNITS and OFFSITE/OFFPLOT with regard to loc
al and international measures.
The utility units and OFFSITE/OFFPLOT located in sites 2&3 (listed hereunder) ar
e subjected to HAZOP study:

The HAZOP Chairman shall be experienced person from Third Party Company. The cha
irman shall be responsible for ensuring that a thorough HAZOP study is performed
, that the HAZOP study report is satisfactory and for guiding the team to their
decisions, based on the Chairman's experience.
3.2 METHODOLOGY
3.2.1 GUIDEWORDS
The technique relies on the systematic application of Guide Words. to the variou
s systems comprising the P&IDs. The list of guidewords is contained in Attachmen
t 1. The approved list is in two parts. The first set of guidewords shall be app
lied to the P&IDs line systems requiring evaluation. The second set based on 'Ot
her Than shall be applied at the end of the unit or facility review to assess th
e overall unit or specific vessels as appropriate.
3.2.2 REVIEW PROCEDURE
HAZOP Study is to be scheduled on a UNIT by UNIT basis and in a brain storming m
anner. Typically this shall take the form in which:
The particular system under review shall be identified by marking the HAZOP Mast
er P&IDs with a series of colorful dashes. highlighted the nodes under study. On
ce the system review is completed the yellow dashes shall be made continuous.
Agreed safety changes, including all queries, shall be marked in red on the HAZO
P Master P&IDs and identified by a unique reference number. This number is to be
shown clearly and highlighted on the HAZOP Master P&IDs in the most appropriate
location. The details of the change shall be recorded also on the HAZOP Report

(HAZOP WORK SHEET Attachment 2).

The Chairman shall divide the P&IDs into a number of discrete systems for review
. At the start of the review sessions, scheduled for a complete UNIT, ''Lead Pro
cess Engineer" shall describe the process function briefly to the Review Team an
d indicate the nature of the process material hazards.
The Chairman shall lead the review by applying individual guidewords for each sy
stem. Where a potential hazard is identified, remedial action may be required de
pending on the likelihood of the event and its consequence. If an immediate solu
tion is available and acceptable to the team, the HAZOP Report and/or HAZOP Mast
er shall be marked up accordingly. Where solutions are unlikely to be derived, w
ithout a technical evaluation, the Chairman shall refer the problem for a separa
te assessment. Discussion of problems in the meeting shall be kept to a minimum.
The detailed sequence of HAZOP review is provided at Attachment 3.
3.2.3 RECORDING OF REVIEWS
HAZOP secretary shall fill in the HAZOP Report in accordance with Attachment 3.
If, after reviewing a system, no actions have been generated then a statement to
that effect shall be recorded. The reporting mechanism is by exception, recordi
ng only concerns needing action.
The HAZOP report shall contain all worksheets, A3 copies of the HAZOP master P&I
Ds, The recommendations identified, listed in each worksheet, will be summarized
in HAZOP Study "Action Sheets" (Attachment 4); some of these recommendations ar
e preceded by "consider the possibility" or"evaluate the possibility" that means
that further analysis shall be carried out to confirm or change the previous de
sign solutions.
3.3

HAZOP Review meetings were held in a dedicated room at Project Engineering Offic
