La Candida es la forma ms letal de las infecciones comunes del torrente sanguneo que producen la sepsis, una enfermedad potencialmente
mortal en la cual el cuerpo presenta una respuesta inflamatoria aguda, a una infeccin bacteriana o
mictica.
Los cientficos de la Facultad de Medicina Warren Alpert de la Universidad de Brown (Providence, Rhode Island, EUA, www.med.brown.edu),
quienes trabajaron con colegas de otras instituciones, realizaron un estudio clnico, en 12 centros,
del 31 de julio de 2013 al 24 de abril de 2014. Recogieron muestras de sangre de 1.801 pacientes
hospitalizados, con edades entre 18 y 95 aos y a
quienes se les haba ordenado un cultivo de sangre
segn la norma habitual de la entidad.
Los equipos utilizaron el instrumento T2Dx
que realiza automticamente todos los pasos establecidos para el panel T2Candida, despus de cargar la
muestra. Especficamente, el T2Dx lisa las clulas rojas
de la sangre, concentra las clulas patgenas y los restos celulares, lisa las clulas de Candida de forma mecnica con perlas, amplifica el ADN de la Candida usando una polimerasa termoestable (T2Biosystems, Inc.;
Lexington, MA, EUA; www.t2biosystems.com) y cebadores para la regin de intervencin del espaciador
transcrito 2, que se halla dentro del opern de ADN ribosomal de la Candida, para todos los tipos de Candida y finalmente, detecta el producto amplificado por
aglomeracin inducida con amplicn de partculas supermagnticas y hace la medicin por T2MR.
El Anlisis de Resonancia Magntica T2, que incluye el T2Candida y el instrumento T2Dx, demostr una
sensibilidad global del 91,1 % y una especificidad global
de 99,4 %. El tiempo medio para obtener un resultado
positivo para T2Candida fue de 4,4 horas, en comparacin con las 129 horas que se requieren con un cultivo
de sangre y la identificacin de la especie. El tiempo
medio para obtener un resultado negativo para T2Candida fue de 4,2 horas, en comparacin, con por lo menos, 120 horas que se requieren para un hemocultivo.
Eleftherios Mylonakis, MD, PhD, autor principal del
estudio, dijo: La capacidad para determinar la presencia o ausencia de Candida en cuestin de horas en lugar de das es un cambio de paradigma para los pacientes en riesgo de sufrir estas infecciones. Nos permitir
quitarnos la idea de mejor adivinemos, cuando hablamos del tratamiento de los pacientes con riesgo alto, como son aquellos con cncer o trasplantes y los pacientes que se encuentran en la Unidad de Cuidados Intensivos y seguir un proceso con mejor informacin, en el
cual podremos enfocar rpidamente el mejor curso del
tratamiento, con la posibilidad de obtener la mejor respuesta del paciente y de salvar vidas. El estudio fue publicado el 12 de enero de 2015 en la revista Clinical Infectious Diseases.

Los inmunoensayos basados en papel, por lo general en forma de pruebas de flujo lateral, son actualmente la plataforma estndar para el diagnstico casero. Sin embargo, las pruebas laterales convencionales
son, a menudo, complicadas por una adsorcin severa,
no especfica, de partculas detectoras cuando se aplican a muestras de anlisis que contienen fluido salival.
Cientficos del Instituto de Bioingeniera y Nanotecnologa (The Nanos; Singapur; www.a-star.edu.sg)
desarrollaron un dispositivo de flujo de apilamiento,
consistente en una tira de anlisis, una almohadilla de
muestra, una almohadilla de reactivo, una almohadilla
absorbente, y un regulador del flujo. Las almohadillas
de muestras y reactivos estaban hechas de fibra de vidrio, y la almohadilla absorbente estaba hecha de celulosa (Merck Millipore, Billerica, Massachusetts, EUA,
www.merckmillipore.com). La tira de anlisis de nitrocelulosa fue cortada en las dimensiones requeridas,
manualmente, con un rodillo cortador. La deteccin
de inmunoglobulina G (IgG) especfica para el dengue
fue realizada utilizando el mtodo de conjugado lquido. El conjugado lquido se compone de protena G
conjugada con nanopartculas de oro de 40 nm.
En el dispositivo, se crean diferentes trayectorias de
flujo para muestras y reactivos a travs de un sistema
apilado mltiple. Esto permite que la muestra de saliva fluya separadamente a travs de una matriz de fibra
de vidrio, que elimina las sustancias que puedan interferir con el sistema de deteccin basado en nanopartculas, antes de que se mezclen con las nanopartculas
sensoras. La configuracin del dispositivo tambin
ayuda a regular el flujo en la tira de anlisis, generando lneas uniformes de anlisis para obtener resultados
ms exactos.
El estudio fue publicado el 12 de enero de 2015, en

Los biomarcadores para la enfermedad cardiovascular futura fueron la fenilalanina, un aminocido comn, y la cantidad de grasa monoinsaturada
en la sangre; Las concentraciones ms altas se relacionaron con el riesgo mayor de enfermedad. Estos
dos biomarcadores eran predictores tan fuertes de
enfermedad cardiaca futura como las medidas del
colesterol malo o la presin arterial. Adems, los
niveles en sangre ms altos, de tanto los cidos grasos omega-3 como omega-6, estaban vinculados
con un menor riesgo de enfermedad cardiovascular. Todas estas molculas estn normalmente presentes en la sangre de todas las personas, pero era
la cantidad de estas molculas que se demostr
que eran el reflejo de la salud cardiovascular.

en la revista Circulation.

plasmticos bajos de apoE, se mantuvo significativamente asociado con un mayor riesgo de enfermedad de Alzheimer despus del ajuste para el genotipo APOE 2/3/4.
Los autores concluyeron que los niveles plasmticos bajos de apoE estn asociados con un mayor
riesgo de enfermedad de Alzheimer futura y para
todas las demencias en la poblacin general, independiente del genotipo APOE 2/3/4. Esto es
clnicamente relevante, porque no hay biomarcadores plasmticos implementados actualmente.

detectan el VHS-1/2
y Varicella Zoster
e compar el desempeo de dos reacciones en
cadena de la polimerasa en tiempo real (PCR),
mltiplex, con las PCR en tiempo real, individuales, para la deteccin del virus del herpes simplex1 (VHS-1), VHS-2 y del virus Varicella zoster
(VVZ), a partir de muestras clnicas. El cultivo viral ha sido considerado la prueba estndar de oro,
aunque los mtodos moleculares han demostrado
ser ms rpidos y sensibles y en la actualidad, la
mayora de los laboratorios realizan PCR en tiempo real, para la deteccin del VHS y VVZ.
Los cientficos de la Clnica Mayo (Rochester,
MN, EUA, www.mayoclinic.org) analizaron
26.186 muestras que haban sido enviadas a su laboratorio para anlisis de rutina mediante los reac-

moleculares. La sensibilidad del ensayo Quidel vari desde 92,0%, para VHS-1 a 97,7% para el VHS2, mientras que la especificidad para todos los objetivos fue del 100%. El ensayo Focus demostr
100% de sensibilidad para todos los objetivos, y la
especificidad por ciento oscil entre 96,8% para el
VHS-1 hasta 100% para el VHS-2 y VVZ.
Los autores concluyeron que para la deteccin
de VHS-2, en el lquido cefalorraqudeo (LCR), el
ensayo Focus mostr 100% de sensibilidad y especificidad, mientras que el mtodo Quidel VHS-2 exhibi una sensibilidad del 87,5% (7/8) y una especificidad del 100 % (18/18). Se necesitan ms estudios que evalen un nmero mayor de muestras de
LCR para definir mejor el desempeo de estas pruebas. Los ensayos multiplex para los VHS-1/2 y VVZ
ofrecen una opcin prometedora para aquellos laboratorios clnicos que buscan reducir los tiempos
de trabajo manual y de respuesta. El estudio fue publicado en lnea en la edicin de enero de la revista Diagnostic Microbiology and Infectious Disease.

Diagnostics, Inc., (SD); Yongin, Repblica de Corea; www.standardia.com), que se basa en dos glicoprotenas de superficie variables nativas (VSGs).
Una RDT prototipo desarrollada por SD que se basa en antgenos recombinantes VSG expresados
en Baculovirus. La tercera RDT fue una RDT prototipo que se basa en los antgenos de glicosprotena superficial del dominio trans-membrana, de tipo-1(ISG65), no variada, recombinante de VSG
nativa y VSG nativa (MITat 1.4), desarrollados por
Soluciones BBI (Cardiff, Reino Unido; www.
bbisolutions.com) y llamados BBI.
Se calcul la sensibilidad y la especificidad de
cada RDT para cada lector y cada prueba por duplicado. Los dispositivos prototipo no eran inferiores en sensibilidad o especificidad a la SD BIOLINE HAT en los mrgenes de 5%, mientras que
uno de los dispositivos (BBI) present una sensibilidad significativamente superior. El anlisis del
desempeo de los antgenos individuales fue utilizado para modelar las nuevas combinaciones de
antgenos a ser exploradas en el desarrollo de la
prxima generacin de RDTs para la HAT. El mo-
delo mostr que una RDT utilizando dos antgenos recombinantes (rLiTat1.5 y rISG65) daba un
desempeo similar al de los mejores dispositivos
en el estudio, y que tambin ofreca el desempeo
ms robusto en las condiciones del campo en deterioro.

Identifican agentes causantes del eumicetoma de grano negro mediante mtodo molecular
e ha presentado un mtodo molecular rpido,
simple y altamente eficiente, para la identificacin del agente causante del eumicetoma de grano
negro, con el fin de mejorar el diagnstico en las
reas endmicas.
La identificacin precisa del agente causal del
micetoma es una prioridad para el tratamiento; Sin
embargo, las herramientas de identificacin actuales estn lejos de ser satisfactorias tanto para el
diagnstico fiable como para las investigaciones
epidemiolgicas y los mtodos moleculares disponibles en la actualidad, basados en la secuenciacin de ADN, aunque especficos son extremadamente caros.

C. difficile toxignico
lostridium difficile es una bacteria anaerobia
intestinal y la causa patgena ms comn para la diarrea asociada a la atencin sanitaria y, por
lo tanto, la deteccin rpida de C. difficile toxignico es muy importante en los laboratorios clnicos. El mtodo estndar aceptado para la deteccin de C. difficile es el cultivo anaerobio seguido
de la identificacin de toxigenicidad por inmunoensayo enzimtico (EIA), cultivo de clulas y
prueba de neutralizacin de la citotoxina o un ensayo de amplificacin de ADN. Sin embargo, existen limitaciones asociadas con el mtodo de cultivo toxignico tales como un tiempo de respuesta
largo y la complejidad tcnica. Los microbilogos
de la Facultad de Medicina de la Universidad de
Sungkyunkwan (Sel, Repblica de Corea;
www.skku.edu) examinaron un total de 302
muestras de heces diarreicas consecutivas para determinar la infeccin por C. difficile (CDI). Todas
las muestras de heces, sin ningn tipo de tratamiento previo, fueron inoculadas en una caja con
agar cromognico, ChromID C. difficile (bioMrieux SA; Marcy lEtoile, Francia; www.
biomerieux.com) y fueron incubadas en condiciones anaerobias. Se realiz una prueba de reaccin
en cadena de la polimerasa casera (PCR) para detectar los genes de la toxina de C. difficile.

lgunas infecciones despus de la biopsia de
prstata debidas a una cepa resistente a los medicamentos de Escherichia coli, se pueden evitar
mediante simples cultivos de hisopados rectales antes del procedimiento y las pruebas de cultivo para
la E. coli, resistente a los antibiticos se pueden usar
para dirigir la seleccin de la profilaxis antimicrobiana, usada para el procedimiento.
La biopsia de prstata guiado por ecografa transrectal (TRUSBx) es uno de los procedimientos urolgicos ambulatorios realizados con mayor frecuencia y la infeccin es un riesgo bien establecido, incluyendo la infeccin del tracto urinario (ITU),
prostatitis, bacteriemia, sepsis, con complicaciones
bien reconocidas como hematuria , sangrado rectal
y hematospermia. Los mdicos del Hospital de Rhode Island (Providence, Rhode Island, EUA, www.
rhodeislandhospital.org) realizaron un estudio retrospectivo de 487 pacientes en total, a quienes les
practicaron biopsias de prstata entre febrero de
2013 y febrero de 2014; en 314 se hicieron cultivos
rectales previos al procedimiento y en 173 no se hicieron. Las edades promedio fueron de 62,7 y 64,1
aos, respectivamente. No hubo diferencia en el valor
promedio del antgeno prosttico especfico, puntuacin de comorbilidad de Charlson u origen tnico.
Los hisopados rectales fueron recolectados en el consultorio, por lo general, menos de un mes antes de la
TRUSBx. Para las pruebas de resistencia
a la fluoroquinolona, los hisopados fueron cultivados directamente en agar
MacConkey tanto con 1 mg/mL de ciprofloxacina como en placas de agar
sangre y fueron incubadas a 35C durante 48 horas. Las susceptibilidades
antibiticas fueron determinadas utilizando la mquina automatizado VITEK 2 (bioMrieux, Inc. Hazelwood, MO, EUA, www.biomerieux-usa.com).
Los cientficos encontraron que el 12,8% de
los pacientes tenan E. coli preexistente, resistente a los medicamentos en los cultivos de hisopados rectales. Tambin observaron que exista un riesgo menor de infecciones post-biopsia y
una disminucin del riesgo ajustado de complicaciones infecciosas en pacientes que recibieron
profilaxis con base en los resultados del cultivo
del hisopado rectal pre-biopsia en comparacin
con aquellos pacientes que se sometieron al procedimiento sin los cultivos de hisopados rectales
prebiopsia, pero las diferencias no alcanzaron
significacin estadstica debido al tamao del es-

tudio. A partir de los cultivos de dos de estos pacientes se pudo aislar Acinetobacter junii y
Pseudomonas aeruginosa resistente a las fluoroquinolona.
Los pacientes a quienes no les hicieron cultivos
de hisopados rectales prebiopsia y que desarrollaron
una complicacin infecciosa despus de la biopsia
tuvieron hospitalizaciones ms largas para la infeccin post-procedimiento: de tres das frente a 1,6 das y en el 40% fue necesario continuar el tratamiento ambulatorio con antibiticos intravenosos para
esas infecciones, en comparacin con el 16,6% para el grupo tratado con antibiticos dirigidos por el
cultivo.

Datos refuerzan prueba VPH para deteccin del cncer de cuello uterino
nos datos recientemente publicados de un
ensayo de tres aos, apoyan nuevas guas
profesionales, provisionales, de las sociedades mdicas sobre el uso de la prueba del virus del papiloma humano (VPH), como una prueba de deteccin, de primera lnea, para el cncer de cuello
uterino (CCU), para las mujeres a partir de los 25
aos.
La infeccin persistente por el VPH ha sido implicada en ms del 99% de los casos de cncer de
cuello uterino en todo el mundo. Roche Diagnostics Corporation, del grupo Roche (Basilea, Suiza;
www.roche.com), ha anunciado que los datos de
final del estudio de 3 aos del ensayo ATHENA,
publicado ahora en la revista Gynecologic Oncology (enero de 2015), refuerzan las directrices
emitidas por la Sociedad de Oncologa Ginecolgica (SGO) y la Sociedad Americana de Colposcopia
y Patologa Cervical (ASCCP) (desarrollado con el
aporte de 5 organizaciones mdicas adicionales)
para ayudar a los mdicos a que incorporen efectivamente la prueba de VPH en los protocolos de
deteccin del cncer de cuello uterino. La gua clnica provisional recientemente emitida (publicada
simultneamente en las revistas Gynecologic On-
de ms de 47.000 mujeres que mostr una inesperada alta carga de la enfermedad de cuello uterino en las mujeres de 25-29 aos. Los datos del
estudio ATHENA demostraron que ms de un tercio de todas las mujeres con NIC 3, el grado ms
alto de pre-cncer, tenan entre 25 y 29 aos.
Clnicamente validada por el estudio ATHENA,
la prueba cobas HPV es actualmente la nica
prueba el VPH aprobada (abril de 2014) por la
FDA para su uso como una prueba de deteccin
primaria principal para el cncer de cuello uterino
en las mujeres mayores de 25 aos. Tambin es la
nica prueba de VPH aprobada por la FDA que
proporciona informacin especfica de genotipificiacin para el VPH 16 y 18, los tipos de mayor
riesgo, y a la vez, informando sobre los otros 12 tipos de VPH de alto riesgo, como resultado agrupado, todo en un procesamiento y a partir de una
muestra de la paciente.

IFCC, its Regional Federations and many of its Members towards a greater focus on the clinical effectiveness of
laboratory medicine. For IFCC this has been exemplified by the work of many of the Task Forces and by the increasing collaboration with international clinical organizations. A nice summary of this topic was published in the
January 2015 issue of the electronic journal of IFCC. This trend will surely gain further momentum in the years
ahead for laboratory medicine is truly central to clinical decision making in healthcare.
The IFCC Council Meeting. An excellent attendance occurred at the triennial Council meeting in Istanbul in June
2014. The agenda included reports on IFCC activities after which Council debated two Motions on the future composition of the Executive Board. These Motions were subsequently put to formal electronic vote resulting in an
outcome that will strengthen Regional Federation representation in future Executive Boards.
Congresses and Conferences. Congresses and conferences provide an opportunity to update our scientific
and clinical knowledge, skills and experience and they also provide a great forum to meet and make friends and
to build long lasting professional networks. During 2014 we held a successful IFCC WorldLab congress in Istanbul, which was attended by >5000 delegates.
Finances. The annual accounts for 2014 were audited in March 2015 and copies are available upon request from
the IFCC Office. IFCC had another solid year financially and our finances are sound as we prepare for the future. The only dark cloud is the continuing fall in value of the Euro against other currencies, notably the Swiss
Franc (the formal currency of IFCC).
Increasing the Focus on Patients. Over the past three years there has been a significant shift in emphasis by

commitment and inspiration. Our excellent team in the IFCC Office in Milan (Paola Bramati, Silvia Colli-Lanzi,
and Silvia Cardinale) has again demonstrated their dedication and professionalism in supporting our Members.
Above all, however, IFCC owes a great debt to the many scientists and medical doctors who work on a voluntary basis to give freely of their time and expertise in the interests of improving the quality, delivery and relevance of clinical chemistry and laboratory medicine on a worldwide basis. It is gratifying that we are seeing an
increase in the number of experts who want to work for IFCC. These people are the true strength of IFCC as
we move forward. The incoming Executive Board inherits an active and confident IFCC I wish them success
and pleasure in their role.
TREASURERS REPORT
The Executive Board held three meetings during 2014. The first meeting was held in the headquarters of the
American Association for Clinical Chemistry (AACC), Washington DC, USA, March 21-23; the second was held
in Istanbul in association with the WorldLab Congress, June 20-21; during this meeting was also held the IFCC
Council with a great debate about the future of IFCC and of Laboratory Medicine. Moreover an important motion
on the presence of one Regional Federation representative on all future Executive Boards was discussed and
approved later by electronic ballot with 98% of votes.
The third EB meeting was held in Rome in November 15-17, in association with the Joint Symposium IFCC-Roche
Biomarkers in Alzheimer Disease. The newly elected EB members and the resigning ones participated to the meeting. The outgoing members of the EB were invited to reflect on the last three years and suggest topics of interest for
the future of IFCC, to be considered by the incoming Board. The new EB Members were invited to ask EB for suggestions about their new role and their working relationships with Divisions, Committees and the IFCC Office. At the
end of the joint EB meeting, many words were spoken to remember and appreciate the role of the outgoing friends.
It was a really exciting and touching moment that reminded everyone of crucial advancements for IFCC.
New members joined the IFCC family in 2014: the Macedonian Society of Medical Biochemistry and Laboratory Medicine (MSMBLM) and the Saudi Association for Clinical Chemistry as Full Members; Diatron (HU),
Guangzhou Wondfo Biotech Co. (CN), Sonic Healthcare Europe (DE) as corporate members.
A letter was sent to the Full and Affiliate Members National Societies, requesting their annual reports; 44 forms
were received to be included in the IFCC Annual Report 2014, which is available on the IFCC website
(www.ifcc.org). Regional Federations, as essential part of the IFCC, also presented their summary that will be
part of the report published in the IFCC website.
The 2014 IFCC Annual Report closes my first term as IFCC Secretary. I would like to personally thank the EB
members finishing their term at the end of 2014: Jocelyn Hicks, Graham Beastall, Bernard Gouget, Howard Morris, Larry Kricka, Ulisses Tuma, Thomas Brinkmann, and the IFCC staff of the IFCC office in Milan.
Now my 2nd term begins, I wish to thank the confidence showed by the National Representatives, hoping to
live up their expectations.

During these last three years, considerable efforts have been made to make more efficient the day-to-day processing of accounting records with IFCC national societies, IFCC regions, Corporate Members and the banks in
charge of the IFCC accounts. All this effort could not have succeeded without the momentum from the IFCC-EB
and a very close collaboration based on trust and friendship with the IFCC office which now counts 2.1 FTE. A
big thank you to the entire team: Paola Bramati, Silvia Colli-Lanzi, and Silvia Cardinale. Thanks also to Silvia Cattaneo who worked in the IFCC office until June 2014 before taking over the EFLM office independently.
This 2014 budget reflects the desire to have the most realistic budget possible seeking to promote the smooth
progress of the scientific activities underway while making room for funding of innovative projects. The total operational revenues and financial income are CHF 2.078.225 (Fig 1), for total operational costs and financial
charges of CHF 1.515.308 (Fig 2), or a profit of CHF 562.917, 2014 is the second consecutive year with substantial profits, as 2013 also closed with a positive result (+ CHF 442 498).
The division budgets remained constant relative to 2013. It can be noted that the SD received specific sponsorships (CHF 264,667), which gave them a certain flexibility conducting projects.
This year, expenses appear to be increased relative to the provisional budget of CHF 1,519,984 due to the
latest payouts of EuroMedLab 2013 to the Italian society at the start of the year. Likewise, the revenue takes into account the financial inflow from WorldLab 2014 Istanbul managed by the IFCC. According to current guidelines, the IFCC has received 20% of the global income, i.e.: EUR 547,035.9 + EUR 15,910 from satellite symposiums, the TBS (Turkish Biochemical Society) EUR 232,968 and the PCO (MZ Congressi) EUR 232,968. The
Congress operating balance (EUR 2,735,178/EUR 1,721,905.94) remains positive showing the importance of the
success of an IFCC conference like WorldLab which contributed 32% of 2014 IFCC annual resources. We thank
MZ for their energy and professionalism in the organization and success of IFCC congresses and events.
The price of the full member dues is set according to the World Bank classification which distinguishes difference categories according to gross national product (GNP) per capita (Fig 3). Therefore, economies with low
revenue, medium revenue and high revenue are distinguished (with medium revenue ones being subdivided into lower medium revenue and upper medium revenue). Figure 4 reflects full member dues by region for a total
amount of 183,754 CHF. It may be noted that the EFLM with 40 countries contributes CHF 95,321.00, and that
others (including AACC), contribute CHF 44,110. The contribution of the 10 affiliate members was CHF 3,584.
The IFCC Strategic Plan. The Executive Board continued to working effectively during 2014 and succeeded in
completing all 26 action points in the Strategic Plan for 2012-14. Notable achievements during 2014 included:

A meeting with Regional Federation Presidents
An increase in the number of clinical organizations with which IFCC collaborates
Surveys of Member opinion on a range of issues
Support for a horizon scanning review published by Larry Kricka and colleagues
We can only thank the members for their loyalty and attention in paying dues at the start of the year, which helps
the cash flow.
The grant to the regions is maintained at CHF 10,000 except for EFLM which received the balance from EuroMedLab 2013 Milan, in late January 2014, EUR 157,289.00.
Despite some old corporate members leaving and new ones coming in (50 in 2012, 51 in 2013 and 52 in
2014), the number of CM remains approximately constant and the received dues are CHF 330,422 (Fig 5). The
actual received amount shows a higher sum compared to the budgeted one, due to the catch-up contributions
from outstanding companies. The IFCC is very grateful to the iVD industry for their support and active participation in the IFCC scientific events.
The latest strategic focus concerning finances consisted of optimizing account management with Credit Suisse, as well as that of the portfolio entrusted to the LGT Group. As of January 26, 2015, the date of switching
from one treasurer to the other, the current account had a balance of CHF 788,534.90 (CHF 430,660.24, EUR
276,213.83 and USD 81,660.83) which allows operating with the usual financial income to cover 2015 without
difficulty despite the change in the CHF exchange rate. The Swiss National Bank (SNB) having announced in
mid-January that it was terminating the CHF rate floor. This decision resulted in a surge in the Swiss franc against
the euro, which positions the Swiss franc and the euro at almost parity. The distribution over three currencies allows partially cushioning the practical consequences of this increase.
During the last 3 years IFCC established very close relationship with M. Jean Michel Daniele, LGT portfolio
manager to find the best possible investment based on a thorough analysis of qualitative and quantitative factors. The core element of The IFCC/LGT investment philosophy was the disciplined asset allocation across a full
range of all asset classes and the permanent risk monitoring of the portfolio. In 2014, we choose the Growth
profile (Figure 6) to achieve an optimized long-term risk/return profile through broad diversification with an expected growth rate around 5%.
In terms of geopolitical developments and global economy, 2014 was a disappointing year. The problems in
the Middle East have been exacerbated with the sudden rise of the Islamic State, while the Ukraine conflict and
the cooling of relations between Europe and the US on the one hand and Russia on the other have been a bitter economic and political setback. While the economic development within the euro area was disappointing once
more, at least the recovery in the US economy remained in line with expectations.
Against the background of the observed changes we have reduced our investment risks in the euro area
where LGT recommends caution in particular with regards to the respective currencies and bonds. IFCC Portfolio remained diversified in terms of how the various asset classes were weighted according the economic environment. A close monitoring of the IFCC portfolio got us out of those tricky geopolitical tensions with a sound performance for 2014.(+4.56% (EUR 2,867,931.14 ) on December 31 and 5.52% (EUR 3,026,211.88) on January
26, 2015) (Figure 8).
Figure 8 shows that in three years, the collaboration with LGT has proven fruitful since we went from EUR
1,981,330 in late 2011 to EUR 3,026,211.88 on January 26, the date on which the keys and passwords were
turned over to the new treasurer. This amount is equal to two fiscal years of operation to respond appropriately
to the IFCC missions.
As usual, the 2014 financial statements were certified by Mr. John Gates, Partner with Humphreys and Gates
Audital who provided valuable guidance and expertise.
At the end of this Treasurer mandate, we would like to wish all the best to Prof. Tomris Ozben, the new IFCC
treasurer.

Corporate Representative at IFCC Executive Board: Thomas Brinkmann (Sonic Healthcare). Second
term: 2012-2014. Corporate Members contribute significantly to the overall IFCC activities in all three Divisions
and in their Executive Committees. Furthermore, Corporate Members organizations continued in sponsoring
IFCC workgroups and Scientific Awards. In addition numerous delegates from Corporate Members actively collaborated in most of the scientific workgroups..
Number of Corporate Members. By the end of 2014 the total number of Corporate Members increased to 52
with a total annual dues actual income of CHF 330,422 representing 15.90% of the IFCC total annual income
(fees). 3 IVD companies joined IFCC as Corporate Member (Diatron, Hungary; Guangzhou Wondfo Biotech Co.,
Ltd., China; Sonic Healthcare Europe, Germany); 4 Corporate Members were lost: BG Medicine (US), Perkin
Elmer (FI), Immunodiagnostic Systems (UK), and Biomerieux (FR).
Corporate Members' Meeting. In 2014 two Corporate Members meetings were organized in conjunction with
international conferences: IFCC WorldLab, Istanbul, Turkey (June, 2014) and AACC in Chicago, IL, USA (July,
2014). Meeting minutes and ToDo lists have been distributed.
IFCC Committees. Corporate Members contributed in the following three IFCC divisions: Scientific Division, Joseph
Passarelli as 2nd term 2013-2015 (Roche Diagnostics); Education & Management Division, Christoph Ebert as 1st
term 2013-2015 (Roche Diagnostics); Communications & Publications Division, Bruce Jordan as 2nd term 20142016 (Roche Diagnostics). In addition Corporate Members worked in many IFCC committees and workgroups.

The congress was successful in regards to the number of participants, exhibition, scientific program, social
events, and the facilities of the congress center. The scientific program included a wide range of high quality and
innovative topics through one opening lecture, 4 plenary lectures, 39 symposia, 117 lectures and 42 short oral
presentations selected from the abstracts, 1,552 poster presentations and 22 industry sponsored workshops. The
number of delegates was 3,258; visitors 1,875 and the total number of participants 5,133 from 116 countries.
There were 13 sponsors and 48 exhibitors. IFCC provided 19 Travel Scholarships, Prof. Jocelyn Hicks one and
Turkish Biochemical Society ~100 reg. fees.
Three satellite meetings were organized before the congress: XIII International Congress of Pediatric Laboratory Medicine ICPLM 2014; POCT Satellite Meeting, and LC-MS/MS in Clinical Laboratory

IFCC WorldLab Durban 2017 South Africa (22-26 October, 2017). The 23rd WorldLab Congress will be held
at the Durban International Convention Center. The Organizing and Scientific Committees of the Congress were
formed. The members of the International Scientific Committee are as follows: Prof. Tahir Pillay, President and
Chair, Members: Prof. R. Erasmus, Prof. T. Matsha, Prof. K. Adeli, Dr. M. Turzyniecka, Prof. J. Delanghe, and
Prof. S. Sandberg. First announcement of the Congress has commenced along with various promoting activities.
IFCC WorldLab 2020, Seoul South Korea. The Korean Society of Clinical Chemistry (KSCC) as the Hosting
Society of IFCC 24th WorldLab 2020, started to activate the Local Organizing Committee (LOC) by nominating
Prof. Junghan Song (Seoul National University) as the Chairman of Local Organizing Committee. The WorldLab
Congress began to be promoted during the Autumn Symposium of KSCC on November 6, 2014.
IFCC Regional Congresses of Clinical Biochemistry and Laboratory Medicine
APFCB 2016 Congress - Taipei, Taiwan (November 26-29, 2016). The 14th APFCB Congress will be held at
Taipei International Convention Center (TICC), Taipei, Taiwan. The Organizing committee has been formed. The
chairman is Woei-horng Fang. All the relevant information regarding the meeting is provided on the website of
the congress (www.apfcbcongress2016.org/committee.html). The program and the speakers have been defined.
EuroMedLab Congress 2015 France, Paris (21-25 June 2015). The scientific program for 21st EuroMedLab
has been finalized.
The congress will start on Sunday with the Opening Ceremony and the Welcome Cocktail. The morning program from Monday to Thursday includes a series of symposia organized by the Scientific Committee or the
IFCC/EFLM. A plenary lecture will conclude the morning program. The early afternoon program from Monday to
Wednesday includes poster sessions and poster walks. The restaurant will be located in the poster area in order
to attract delegates to the posters. Symposia and Workshops are also scheduled during this time. The late afternoon program from Monday to Wednesday includes an IFCC/EFLM symposium and a series of Educational
Workshops proposed and funded by the sponsoring companies. The exhibition area will be open on Monday,
Tuesday and Wednesday from 10:00 to 17:30 hrs. The Congress will end on 13:00 hrs on Thursday.
Delegates with full registration are allowed to submit an unlimited number of abstracts. About 1,2001,300
abstracts are expected. All posters will be displayed with panels divided day by day, on Monday, Tuesday, and

C-Nomenclature, Properties and Units (C-NPU)
During 2014 ongoing progress towards C-NPU terms of reference included:
Term of Reference 1. Provide advice for the management, updating and publishing of the NPU terminology. Accomplishments include: A new NPU Steering Committee was created consisting of NPU committee members
and key stakeholder representatives (from the IFCC, IUPAC and user countries) The NPU Steering Committee
(IUPAC subcommittee) met June 24-26, 2014, in conjunction with IFCC WorldLab, Istanbul, Turkey. The C-NPU
Steering Committee held teleconferences in 2014 as follows: September 29, August 22, June 19, May 7, March
11, and January 14; 2) Regularly updated versions of the NPU terminology continue to be available on the IFCC
website; 3) The majority of these revisions are made or coordinated by NPU members belonging to the Danish
e-Health Authority; 4) An application for a Finnish release center is being considered.
Term of Reference 2. Make recommendations on NPU for reporting clinical laboratory data that conform to or
adopt current standards of authoritative organizations and that will improve their utilization for health care. Accomplishments include: 1)Project ongoing for definition of poorly identified NPU terms - Project for establishing definitions for non-referenced terms in the NPU terminology ongoing (IUPAC Project 2010-035-700 ) (Chair:
Helle Moller Johannessen); 2) Proposed work to map against SNOMED-CT discussed with IHTSDO, with limited success to date; 3) Silver Book revision 2 (Compendium of Terminology and Nomenclature of Properties in
Clinical Laboratory Sciences) in final review (joint IFCC CPD project + IUPAC 2008-033-3-700); 4) Prepared initial strategic plan and governance arrangements for NPU Steering Committee.
Term of Reference 3. To provide a connection with other organizations concerned with NPU, such as the Bu-

Term of Reference 5. Reference materials for UA and urine creatinine (funded by JSCC and NIST). Accomplishments include: SRM 3666 is currently being prepared based on the specifications developed by the WGSAU and the LWG of the NKDEP. The NIST Albumin in Human Frozen Urine standard reference material (SRM
3666) is pooled urine collected from human subjects with urinary albumin levels ranging from the following stages
of albuminuria: normoalbuminuria (0-25 mg/L), microalbuminuria (25-200 mg/L), and macroalbuminuria (>200
mg/L). All four levels of pooled urine will be available frozen, in 1 mL aliquot volume. A contract to a commercial
vendor for the production of SRM 3666 was awarded in September 2014. A candidate secondary urine albumin
reference material prepared by the Japanese Society of Clinical Chemistry is being characterized. The material
is a buffer based purified monomeric human serum albumin in lyophilized form. The assigned value (225.1 mg/L)
was by transfer from ERM-DA470 using qualified immunoassay procedures.
WG - Standardization of Pregnacy-Associated

participating in the QA/QC program have been collected.
Term of Reference 4: To test the implementation in clinical laboratories of quantitative mass spectrometry analyses for peptides and proteins, using the examples of hepcidin and apolipoproteins. Accomplishments include:

Term of Reference 5. Develop educational materials regarding commutability for manufacturers, laboratories
and users of laboratory results. Accomplishments include: No activity for this Term of Reference.
Term of Reference 6. Advise IFCC Committees and Working Groups on how to assess the commutability of materials on which they are working. Accomplishments include: Members of WG consulted with the WG-TNI to
collaborate on commutability assessment for the reference materials being developed by that group.

Term of reference 1. To provide education and training on the various aspects of analytical quality in the clinical
laboratory, which include: 1. Methods and Instrument validation; 2. Traceability concepts; 3. Measurement uncertainty; 4. Internal quality control procedures; 5. External quality assessment programs. Accomplishments include: Directory of EQA services: A previously established worksheet to collect essential information (on: country,
organizing body, EQA, website, e-mail, contact, open to international participation, disciplines) has been send out
via the IFCC office to all National Societies. 21 National Societies responded and provided the requested information. The information received has been compiled in an EXCEL worksheet and is presently being consolidated
by C-AQ members. Once finalized, the Directory of EQA services will be published on the C-AQ web site and will
be a resource in providing help, guidance and contacts when setting up EQA schemes by any interested party.
Term of reference 2: Education and training will be provided in many ways including: 1. Written material; 2. Electronic teaching; 3. Workshop and seminars; 4. Invited lectures; 5. Consultation and advice. Accomplishments
include: Develop e-learning Presentations; Aim: to work with the Committee on Distance Learning to develop
presentations of Principles of EQA and Principles of IQC. Annette Thomas provided two new documents
(Procedure for the Planning of a new EQA Scheme and Pilot Design (a planning template). Annette has still
to confirm permission for C-AQ website utilization. Once provided, both documents shall be placed on the C-AQ
website. The Principles of EQA Power Point presentation is currently being worked on and discussions with CDL about recording the presentation will begin when this is ready.
Term of reference 3: To ensure proper control of pre-analytical and post-analytical variables. Accomplishments include: Planning an EQA Scheme; Aim: to develop a checklist of items that need to be considered by
countries that are planning to design and set up their own EQA scheme. The EQA set up checklist was finalized and was put on the C-AQ website under Resources in 2014.
Term of reference 4: To address the educational and training needs of emerging nations on analytical quality. Accomplishments include: Resource Table for EQA Website; Aim: to collate a directory of various relevant resources
for pre & post analytical QA, IQC and EQA which will reside on the C-AQ section of the IFCC website. This work
has been started in 2013 and is still being worked on. Finalize the Resource Table for EQA Website in 2015.
Additional Projects: Aim: upon request of an IFCC member country, assist them to set up an EQA program by:
providing expertise on EQA organization and management, providing assistance with designing an EQA program, liaising with EQA providers and with IFCC corporate members to provide suitable materials.

Dr. L Sciacovell, is the chair of this WG. Project: The adoption of Quality indicators (QIs) has prompted the development of tools to measure and evaluate the quality and effectiveness of laboratory testing, first in the hospital setting and subsequently in ambulatory and other care settings. While Laboratory Medicine has an important role in
the delivery of high-quality care, no consensus exists as yet on the use of QIs focusing on all steps of the laboratory total testing process (TTP), although the International Standard ISO 15189:2012 for Accreditation of Medical
Laboratory requires their implementation. In order to promote the harmonized use of QIs and reduce errors in laboratory testing, the IFCC Working Group on Laboratory Errors and Patient Safety (WG-LEPS) developed a project on QIs. The purpose of the project is to design a routine, formal, proactive system of monitoring that uses validated measures to focus strictly on laboratory performance creating a common reporting system based on standardized data collection, and to define the state-of-the-art and quality specifications for each QI independent of: a)
the size of organization and type of activities; b) the complexity of processes undertaken; c) different degree of
knowledge and ability of the staff.
The achievement of a consensus on the typology and the limits of acceptability for quality indicators, above all
for the extra-analytical processes, should allow a reliable comparison to be made between the data collected from
the different laboratories and the achievement of effective benchmarking at international level, for the development
and the application of standardized operative procedures and scientific recommendations to manage the various
critical processes. The final goal is to define a Model of Quality Indicators (MQI) that will be proposed to, and applied by, all clinical laboratories in order to monitor processes and encourage improvement in performances so as
to decrease the error rate in the total testing process.
There has been one C-PR meeting in Istanbul in June, to which all the Committee Members, as well as some of
the Corresponding Members and Federations Advisors were in attendance; in September 2014 the EFLM nominated Dr. Graziani, replacing Dr. Webster who had resigned in 2013.
PR Deliverables. The brochures Understanding Laboratory Medicine and the IFCC Brochure, as well as the
Laboratory Medicine slide kit have been revised and updated for the 2015-2017 edition. The new template has
been adopted for the slides presentation and it is continuously updated by the IFCC office.
LABS ARE VITAL Consortium. Several new blogs have been submitted to Kapler Communications for publication on the web. Each article is to be reviewed by Richard Kents team, before going online.
The overall perception is still that the Labs are Vital Proposal website is not visible and that there is a communication problem. In September 2014 the National Societies individual members have been encouraged to login the LRV website and participate to the discussions on the posted blogs.
Thanks to Ellis Jacobs suggestions, visibility to the LRV initiatives for the Pathology Day celebration has been
given via the IFCC website, and on the IFCC eNewsletter.
Task Force on the Impact of Laboratory Medicine on Clinical Management and Outcomes (TF-ICO). In
agreement with Mike Hallworth, the TF-ICO chair, the data coming from an article on the impact of clinical laboratory on patient care and on cost/benefits stemming from the research that the group had performed, will be
used in a short form for PR-Brochures and in existing or new PP presentations, and that these be translated into many languages much like the existing brochures, and commented as a blog, if in line with the mission of the
LAV consortium.
Worldwide visibility of IFCC. In order to increase the worldwide visibility of IFCC Committees, WG and task forces,
at different levels (National Associations, Regional Federations, Professional Associations, Decision makers) the CPR is evaluating the possibility that its Members be assigned to liaise with National Societies or Associations that are
within their broad geographical region and/or linguistic realm to make known the activities and services of IFCC.

de virus del bola
e ha desarrollado un laboratorio porttil para el
diagnstico del virus del bola, y el nuevo mtodo es seis a 10 veces ms rpido que el actual, igualmente sensible y pronto ser ensayado en frica.
Actualmente, las pruebas de laboratorio para el virus del bola utilizan una tcnica llamada reaccin
en cadena de la polimerasa en tiempo real (PCR) que
busca la firma del genoma del virus en la muestra, y
aunque las mquinas de PCR son cada vez ms pequeas, no son adecuadas para ubicaciones remotas
sin electricidad y refrigeracin.
Los cientficos del Centro Alemn de Primates
(DPZ; Gttingen, Alemania; www.dpz.eu) han desarrollado el Diagnstico-en-una-Maleta, que contiene
todos los reactivos y equipos para detectar el virus
del bola, en 15 minutos, en el punto de necesidad.
Los Diagnsticos-en-una-maleta se basan en la tecnologa de Amplificacin de Recombinasa Polimerasa (RPA), desarrollada por TwistDx Ltd. (Cambridge,
Reino Unido; www.twistdx.co.uk). Este mtodo fue
adaptado para ser utilizado para el diagnstico del virus del bola por cientficos del Centro Mdico Universitario de Gttingen (Alemania; www.med.
uni-goettingen.de). La RPA es tan sensible como la
PCR, pero muy rpida y funciona a una temperatura
constante, es decir, no se requiere ningn termociclador rpido. Adems, los reactivos utilizados en el ensayo de RPA son independientes de la cadena de fro,
lo que les permite ser utilizados y transportados a
temperatura ambiente.
Christiane Stahl-Hennig, DMV, quien dirige la Unidad de Modelos de Infeccin en la DPZ, dijo: La deteccin precoz de los pacientes infectados con bola
conducir a un control de virus ms eficaz ya que el
personal mdico puede identificar y aislar los casos confirmados de bola ms rpidamente. Los diagnsticos
contribuirn a un mejor manejo durante los brotes de
bola. La idea es que el diagnstico en una maleta
permitir la rpida deteccin del virus bola, no slo en
el campo sino tambin en otros puntos de necesidad,
como aeropuertos y estaciones de cuarentena.

Anlisis que mide ocho marcadores de citoquinas diferencia tuberculosis activa y latente
n nuevo anlisis de PCR de
transcripcin reversa, en tiempo
real (rt-RT-PCR) permite que los diagnosticadores diferencien entre la tuberculosis primaria activa (TB) y la
forma latente de la enfermedad (LTBI), que son estados de la enfermedad que no se pueden diferenciar por
la prueba drmica de la tuberculina
(TBT) o los anlisis de liberacin de
interfern gamma (IGRAs).
Investigadores de la Universidad
de Yonsei (Repblica de Corea; www.
yonsei.ac.kr) desarrollaron un PCR,
de transcriptasa reversa, en tiempo
real TaqMan para la determinacin
de los niveles de ocho objetivos humanos [IFN-gamma, factor de necrosis tumoral (TNF) alfa, IL-2R, IL-4, IL10, CXCL9, CXCL10, y CXCL11].
Los ensayos TaqMan se basan en
sondas de oligonucletidos que constan de un fluorforo unido covalentemente al extremo 5 de la sonda y de
un extintor en el extremo 3. La molcula extintora apaga la fluorescencia emitida por el fluorforo cuando
es excitado por la fuente de luz del ciclador a travs de FRET (Transferencia de Energa de Resonancia Fluorescente). Mientras el fluorforo y el extintor estn en proximidad, la extincin inhibe cualquier seal de fluorescencia. Las sondas TaqMan estn

nea de mltiples objetivos. La combinacin de IFN-gamma, TNF-alfa e IL2R, y la combinacin de TNF-alfa, IL2R, CXCL9 y CXCL10 mostr el mejor desempeo para la deteccin de
PTB activa (ambos con 100% de positividad) y LTBI (86,36% y 81,82% de
positividad, respectivamente).

Anlisis de
hemoglobina con sensor
ptico fototrmico
n sensor ptico, llamado reflectmetro de coherencia ptica de dominio espectral fototrmico (PT SD-OCR) es capaz de la medicin directa de
la concentracin de hemoglobina (Hb), sin necesidad de usar sustancias qumicas y el pre-procesamiento de la sangre.
El PT SD-OCR OCR mide la Hb mediante la
cuantificacin de los cambios del ndice de refraccin de las muestras de sangre debido al efecto fototrmico bajo la irradiacin de la luz de 532 nm y su
exactitud diagnstica y la precisin fueron evaluadas en pacientes anmicos.
Cientficos de la Universidad de Yonsei (Sel, Repblica de Corea; www.yonsei.ac.kr) y sus colegas
utilizaron la PT SD-OCR para medir la Hb en 50 pacientes anmicos, y examinaron su exactitud contra
un analizador de hematologa.
Se examin la exactitud relativa del sensor PT
SD-OCR y las comparaciones de la PT SD-OCR con
el sistema Hemocele 201+ (ngelholm, Suecia;
www.hemocue.com) contra los resultados del analizador de hematologa, ADVIA 2120i (Siemens AG,
Erlangen, Alemania; www.healthcare.siemens.
com). Se observ una excelente correlacin entre
los resultados del analizador de hematologa y los
del sensor PT SD-OCR. La precisin del sensor PT
SD-OCR tambin fue comparable con la del analizador ADVIA 2120i y con la del detector HemoCue
201+.
Los autores concluyeron que la PT SD-OCR, de
operacin simple y sin necesidad de reactivos qumicos permiti la medicin rpida y exacta de la Hb
en los pacientes anmicos. Se espera que el sensor
facilite los procedimientos clnicos relacionados con
los trastornos hematolgicos en el cuidado de los
pacientes. Por consiguiente, el sensor PT SD-OCR
se puede utilizar fcilmente en entornos clnicos para la deteccin de la hemoglobina y la determinacin de las estrategias de tratamiento adecuadas para los pacientes anmicos. El estudio fue publicado
en la edicin de enero 2015 de la revista Clinica
