MUNICIPAL ORDINANCE NO. 05-2014

AMENDING SECTION 114 OF THE GUINOBATAN REVISED MUNICIPAL TAX CODE OF 2008,
THEREBY ADJUSTING SOME FEES OF THE MUNICIPAL HEALTH SERVICES IN THE MUNICIPAL
HEALTH OFFICE OF GUINOBATAN, ALBAY.

Be it ordained by the Sangguniang Bayan of Guinobatan, Albay in session assembled

that:
Section 1. Amendatory Provision. Section 114 of Municipal Ordinance No. 011-2008,
otherwise known as the GUINOBATAN REVISED MUNICIPAL TAX CODE OF 2008, is hereby
amended to read as follows:
Section 114. Local Health Service Fee The following service fees shall be collected for
the services rendered by the Municipal Health Office, to wit:

Stool Exam
Complete Blood Count
Blood Typing
Pregnancy Test
Vaginal Smear
Tooth Extractions
a. Molar Tooth
b. Incesior and Premolar
Dental Filling
a. Temporary
b. Permanent

Section 2. Separability Clause. If, for any reason/s, any part/s of this Ordinance shall be
held to be unconstitutional or invalid, other parts or provisions thereof which are not affected
thereby shall continue to be in full force and effect.
Section 3. Repealing Clause. Any other ordinance/s or part/s thereof which are
inconsistent with any of the provisions of this Ordinance are hereby repealed, amended or
modified accordingly.
Section 4. Effectivity. This ordinance shall take effect immediately upon its approval.
Enacted: May 12, 2013 on Third and Final Reading.
