pating and/or taking any part or program at Family Fun Inc dba SkyHigh Party Zone, the undersigned hereby
acknowledge and agrees that:
I(We) have read, understand and agree to be bound by the stated and customary Rules of Safety and Conduct
governing my (our) conduct while using equipment in the arena, including but not limited to the inflatable FOR:
devises, karaoke stage area and party rooms. If, I (We) observe any violation of the Rules of Safety and
Conduct or other hazard during my (Our) participation, I (We) will bring such violation or hazard to the
attention of the nearest official or employee of Family Fun Inc dba SkyHigh Party Zone. I (We) understand GUEST’S NAME:
the Rules of Safety and Conduct are strictly enforced.
I (We) am aware and understand that the risks, hazards and dangers which can occur as a result of my (our)
participation in any activity, include but are not limited to, falling, jumping, landing, performing tricks or
maneuvers, colliding with other users, staff, media personnel and spectators. I (We) also understand that 26670 MADISON AVE

As to my (our) participation in any activity, and as to any and all liability for damages which I may suffer or
incur due to any cause whatsoever, while in the arena or on the premises, I (We) hereby agree as follows: a)

personal injury, disability, death, medical and any other type of expense or property damage or loss caused by
any negligence Family Fun Inc dba SkyHigh Party Zone, its officers, directors, stockholders, associates,
agents, contractors, managers, employees, volunteers, sponsors, vendors, and any other person or entity in
any way associated with it, and promise not to sue or exercise any legal right to seek damages from Family TIME:
Fun Inc. b) To RELEASE , absolve and discharge Family Fun Inc dba SkyHigh Party Zone, its officers,
directors, stockholders, associates, agents, contractors, managers, employees, volunteers, sponsors, vendors,
and any other person or entity is any way associated with it, from any and all liability for any active or
passive negligence whatsoever, and for any damages I (we) may suffer or incur, or that our next kin may
suffer, as a result of our participation in any activity, including but not limited to, the risks described herein.
c) To HOLD HARMLESS AND INDEMNIFY Family Fun Inc dba SkyHigh Party Zone, its officers, RSVP:
directors, stockholders, associates, agents, contractors, managers, employees, volunteers, sponsors, vendors,
and any other person or entity is any way associated with it, from any and all liability for the damages to any
Third Party resulting from our participation in any activity.
THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THIS RELEASE OF ALL
CLAIMS AND INDEMNITY FOR ALL THIRD PARTY CLAIMS, and further agrees that no oral
representations, statements, or inducements apart from this agreement have been made by Family Fun Inc
dba SkyHigh Party Zone , or anyone else with regard to the subject matter of this agreement. By signing
below on behalf of myself and my child(ren), and or my spouse, I also agree to the above should I decide to
participate.
