Tuition for the 3 day camp is

$35.00 for applications
received by June 10th.
$40.00 for those received
after. For families with more
than one player attending, a
$10.00 discount will be
given.
Tuition, Player information,

received prior to the players
participation. Applications
can be brought to the high

With my signature below, I
acknowledge that:
My son has permission to attend
Henry County High Schools
Wildcat Youth Football Camp.
I have no knowledge of any
physical impairment that would
affect my sons participation
(unless listed on player info.).
In the event of my child requiring
medical care, I authorize the camp
staff to act for me and to obtain
for him whatever medical
treatment the staff deems
necessary and appropriate. I
specifically consent to such
treatment including hospitalization
and surgery and will be
responsible for any medical or
other charges in connection with
said treatment.
I acknowledge that at the Wildcat
Youth Camp my son will
participate in a sport that may
involve physical contact with other
persons or objects that could incur
a risk of injury. I specifically
waive and give up and release the
Wildcat Youth Camp, its staff,
Henry County High School, and
the City of New Castle from
liability for any claim for damages
which I or my son may have for
injuries or illnesses that he may
sustain at Wildcat Youth Football
