ELGIBILITY Entrants must be a full member of a CDGA Affiliated Club. CDGA GHIN # Club__________________________________________________________________
must be on application. Team participants must be full, non-restrictive golfing members
of the club they are representing. TEAM 1
RULES USGA rules will govern all play except for local rules approved by CDGA.

90% of course handicap will be used.

ENTRY Entry is subject to approval or rejection at any time by CDGA Tournament Com-

ENTRY FEE $320 per team. Entry includes cart, range and buffet lunch. Fee will not TEAM 2
be refunded after deadline unless team submits a written medical excuse. Riviera GC

Part C, Section 3 of the Rules of Golf. “A player is prohibited from having a parent, guardian or
another player in the same competition serve as his caddie during the stipulated round.”
TEE TIMES 9:00 a.m. or 1:00 p.m. Shotgun Start. Copies of starting times will be sent Entry Fee: $320/team
to CDGA Pro Shops approximately one week in advance of event. Contestants may
also check OHIOGOLF.ORG for tee times. Members of Riviera Golf Club may enter this event for $288/team
(20% discount as a host club member)
